The hearts were examined from 125 patients with isolated ventricular septal defects (VSD) who had died at the age of 0-14 years before or without operation. One hundred twenty five heart showed 178 defects: 103 with isolated, 22 with multiple VSD. Five major defect types were identified: (1) perimembranous; (2) sinus; (3) central trabecular; (4) boundary muscular trabecular; (5) infundibular. According to the relevance of fibrous skeleton and cardiac conduction system, the defects of various sites are divided into 2 large groups: 1) unadjacent to them (muscular) and 2) adjacent. In terms of septal geometry, there are also two large groups: 1) those located in the inflow portion of the septum and 2) those located in the outflow portion.

At modelling a congenital heart failure--defect of the interatrial septum during late stages of the experiment, dilatation of the pulmonary artery and certain changes in the microcirculatory bed of the lungs, oesophagus, stomach and colon are observed. It has been demonstrated histologically and morphometrically that in adaptive reactions of the blood bed an important role belongs to the arteriolar part and metabolic link of the microcirculatory bed of the organs investigated. The changes in the pulmonary vessels are insufficiently expressed and possibility of hypertension in the lesser circulation is not great.

Thirty-one hearts with anomalous conotruncus (common arterial trunk, hypoplasia of the aorta with transposition, atresia of the pulmonary trunk) have been studied. There are some peculiarities in anatomy and topography of the coronary sinus, concerning its sources, that is veins forming the venous sinus, position and interrelations with the venous sulcus and with the interatrial septum, size and form of the ostium and valve of the coronary sinus. The most amount of the anatomical peculiarities of the sinus are observed in the preparations, where the anomalous conotruncus is combined with absence of one or both cardiac septa.

Twenty-two preparations of hearts with preserved atrioventricular canal have been investigated. Primary defect of the interatrial septum is a constant component of this pathological state. The size of the defect defines participation of the atria in formation of the atrioventricular canal. At the same time it is possible to distinguish the following anatomical varieties of abnormal development of the interatrial septum; oblique cleft in the oval fossa, partial absence of the valve in the oval orifice, perforation of the valve in the oval orifice, complete absence of the valve in the oval orifice and certain defects of the interatrial septum situating beyond the oval fossa limits.

Twenty-eight preparations of hearts having three chambers with two atria have been investigated. The age of the deceased was from several hours up to 21 years. Absence of the interventricular septum results in a complex rearrangement of the vessels in the basis cordis. Only in three cases the aorta and the pulmonary trunk situated typically, in others there was a complete transposition. Since the only ventricle, as a rule, is accompanied with a presence of a small chamber--"emissary", the topography of the vessels is mainly determined by the latter. The "emissary" is situated either along the right or left contour of the base of the common ventricle, its dimensions are variable. When the "emissary" is situated along the anterior-right contour of the basis cordis, as a rule, the aorta takes origin from the latter, its bulb is situated ventrally, initial parts of the venous arteries are visible. The cuspides of the aortal valve sag into the "emissary" lumen. When the aorta is situated normally, it can get off the cavity of the common ventricle, or the "emissary". The state of the children is determined first of all by the character of the pulmonary circulation. If there is no stenosis in the pulmonary trunk and it takes its origin from the common ventricle, or from the "emissary" of a great size, the prognosis is more favourable. Cases of early death among the children are connected with small size of the "emissary" chamber and a small diameter of the pulmonary trunk.(ABSTRACT TRUNCATED AT 250 WORDS)
