AIM: There is controversy in the literature regarding clinical outcomes of CAD/CAM laminate veneers. The aim of the present study was to assess the impact of different levels of CAD expertise and different software programs on the reliability and reproducibility of digital wax patterns of laminate veneers and single crowns. MATERIALS AND METHODS: The present preliminary in vitro study was performed on 10 prepared maxillary central incisors available in dental study models. Of the total, five central incisors were prepared with shoulder finish lines for single crowns, whereas the other five underwent incisal shoulder preparation for laminate veneers. The models were scanned using an intraoral scanner. Four dentists (group DENT) and four CAD dental technicians (group CAD) with expertise in different software programs performed digital diagnostic waxing on all prepared teeth. The resulting digital wax patterns (n = 80) were exported as standard tessellation language (STL) files and superimposed on gold standard digital wax patterns (obtained from the original shape of the teeth before preparations). 3D mesh deviations at the cervical margins as well as distal, mesial, and incisal/palatal surfaces between each STL and the gold standard digital wax patterns were calculated in millimeters using a CAD software program. The mean time required by each operator to perform digital waxing was also recorded. Paired comparisons between groups DENT and CAD as well as between nondental and dental software programs were performed for the crowns and laminate veneers using the Wilcoxon signed-rank and paired t tests (α = 0.05). RESULTS: For group DENT, median deviations for single crowns were 0.15 mm (range: 0.08 to 1.05 mm) and for laminate veneers they were 0.15 mm (range: 0.08 to 0.76 mm). For group CAD, median deviations for single crowns were 0.16 mm (range: 0.09 to 0.73 mm) and for laminate veneers they were 0.10 mm (range: 0.06 to 0.53 mm). The Wilcoxon signed-rank test revealed a statistically significant difference between groups DENT and CAD (P = 0.041) and between the software programs (P = 0.029) for laminate veneers, but not for single crowns (P > 0.05). Furthermore, mean times required for group CAD and for dental software programs were significantly shorter than those for group DENT (P = 0.001) and for nondental software programs (P = 0.001), respectively. CONCLUSION: Within the limitations of the present study, the findings suggest that CAD expertise and the software program significantly affect digital wax patterns for laminate veneers, but not for single crowns. RESUMEN

A method is presented for obtaining a virtual 4-dimensional patient that replicates the intended esthetic treatment. The process involves facial and intraoral scanning to acquire records and software manipulation to enable a virtual waxing of the smile. Once the digital design is complete, patient information can be merged to generate an animated video of the projected rehabilitation, displaying movement and smile dynamics. This strategy provides a noninvasive and reliable diagnostic tool for predicting clinical outcomes.

Resumo A Atenção Domiciliar (AD) promove desospitalização de usuários em condição clínica estável e humanização da atenção à saúde em contraposição à valorização exclusiva do saber médico; organizada pelo Serviço de Atenção Domiciliar (SAD), coaduna-se ao Sistema Único de Saúde (SUS) e dá especial importância à participação ativa de usuários e cuidadores. Este artigo deriva de estudo que analisou efeitos da interface equipe SAD-cuidadores na gestão do processo de cuidado em município do interior paulista. Investigou-se a experiência de cuidadores e profissionais na parceria assistencial dedicada a usuários do serviço: foram entrevistados dez cuidadores e sete profissionais participaram de grupo focal. Análise de Conteúdo Temática foi utilizada para compreensão dos dados, permitindo identificar quatro categorias: hierarquização da relação de cuidado; efeito terapêutico da relação equipe-usuário; infantilização do usuário; e influência do gênero no cuidado. Considerações da equipe no grupo focal corroboram percepção dos cuidadores entrevistados, legitimando necessidade de estratégia para aproximação entre equipes e cuidadores. A gestão compartilhada do cuidado, modalidade que pretende promover retomada da autonomia e apropriação do processo de trabalho pelos profissionais, tende a diminuir a fragmentação e favorecer interdisciplinaridade e integração, promovendo ativamente condições para integrar saber de usuários e cuidadores no cuidado.


Abstract Home Care (HC) promotes dehospitalization of users in a stable clinical condition and humanization of health care in opposition to the exclusive valorization of medical knowledge; organized by the Home Care Service (SAD), it is in line with the Unified Health System (SUS) and gives special importance to the active participation of users and caregivers. This article derives from a study that analyzed the effects of the SAD-caregivers interface on the management of the care process in a city in the interior of São Paulo. The experience of caregivers and professionals in the care partnership dedicated to service users was investigated: ten caregivers were interviewed, and seven professionals participated in a focus group. Thematic Content Analysis was used to understand the data, allowing to identify four categories: hierarchy of the care relationship; therapeutic effect of the team-user relationship; infantilization of the user; and gender influence on care. Considerations of the team in the focus group corroborate the perception of the interviewed caregivers, legitimizing the need for a strategy to bring teams and caregivers closer together. Shared care management, a modality that aims to promote the resumption of autonomy and appropriation of the work process by professionals, tends to reduce fragmentation and favor interdisciplinarity and integration, actively promoting conditions to integrate the knowledge of users and caregivers in care.

RESUMEN

A Paralisia Cerebral é a principal causa de incapacidade motora na infância. O avanço tecnológico tem permitido a sobrevida de crianças prematuras extremas, e conseqüentemente, espera-se o aumento das morbidades advindas da prematuridade. O objetivo deste trabalho é descobrir a prevalência de paralisia cerebral em pacientes prematuros nascidos em nosso serviço. No período estudado houve 1184 partos, incidência de prematuridade de 12,9%, mortalidade de 9,8% em prematuros, e foi encontrado apenas um caso de paciente com paralisia cerebral, no grupo de prematuro extremo (<31 semanas), demonstrando prevalência de 0,7% em prematuros.

Data from the Brazilian Ministry of Health shows that the breast cancer is one of the leading causes of women's death, despite the enhancements in treatment and diagnosis of this disease. Successive explanatory advertising campaigns emphasized the methods available for prevention: auto examination, clinical examination by a health care professional and mammography. Regardless of their relevance, only these strategies did not reduced incidence of breast cancer, since they do not impact on the oncogenesis factors. The objective of this article is to develop a health promotion activity through the reinforcement of the previous knowledge and aggregation of new information about breast cancer of a basic health unit. The methodology used was a transversal study of pedagogical intervention in the waiting rooms of a basic health unit. The pedagogical intervention was composed by an interactive questionnaire, lecture and discussion about the topic associated with a qualitative and quantitative evaluation of the activity. The proposed activity had positive results since the participants had qualitative and quantitative improvement in their knowledge and also a good interactivity with the researchers.

No Brasil, dados do Ministério da Saúde apontam o câncer de mama como principal causa de morte em mulheres, apesar dos avanços no diagnóstico e tratamento desta enfermidade. Sucessivas campanhas de esclarecimento à população enfatizaram os meios disponíveis para prevenção: autoexame, exame clínico por profissional da saúde e a mamografia. Apesar da sua importância, estas estratégias isoladamente não reduziram a incidência deste agravo à saúde, já que não atuam nos fatores predisponentes para sua iniciação. O objetivo desse estudo foi desenvolver uma atividade de promoção à saúde através do reforço de conhecimentos existentes e agregação de novas informações sobre o câncer de mama no contexto de Unidade Básica de Saúde. Trata-se de um estudo transversal de intervenção pedagógica em salas de espera de Unidade Básica de Saúde. A intervenção pedagógica foi realizada através de questionário interativo, palestra informativa e discussão livre sobre o tópico, associados à avaliação quantitativa e qualitativa desta intervenção. A abordagem proposta foi positiva, uma vez que houve melhora quantitativa e qualitativa da compreensão do público sobre os temas abordados, boa interatividade entre os palestrantes e o público, e criação de agentes multiplicadores de conhecimento.
