RESUMEN

Infrared laser therapy using an arsenide-gallium laser was used in 54 patients aged 30-75 years who suffered from coronary heart disease. A positive clinical effect was achieved in 47 (87%) of 54 patients, as evidenced by the reduced number of nitroglycerin tablets used and improved bicycle ergometric, echo- and electrocardiographic parameters in the patients.

Radioimmunoassay was used to measure thyroid, somatotropic, parathyroid hormones and cyclic nucleotides in 40 patients with hypertrophic cardiomyopathy and 14 with hypertensive disease concurrent with asymmetrical myocardial hypertrophy. In the patients with hypertrophic cardiomyopathy, the mean blood concentration of thyroid, somatotropic, parathyroid hormones and cyclic nucleotides was generally within the normal ranges and similar to that in hypertensive patients with apparently healthy individuals. The patients with hypertrophic cardiomyopathy concurrent with supraventricular arrhythmias exhibited higher blood levels of total and free thyroxine and thyroxine/thyroxine-bound globulin ration than did patients without arrhythmias. The patients with hypertrophic cardiomyopathy concurrent with arrhythmias displayed increased blood levels of cyclic nucleotides. The findings suggest that thyroid hormones and cyclic nucleotides are likely to make contribution to the pathogenesis of arrhythmias in hypertrophic cardiomyopathy.

To study vascular responses and to choose a differential therapy, 51 patients with hypertensive disease underwent a pressor test combined with intravenous epinephrine. The test revealed two types of blood pressure control. In type I responses (normotensive responses), there was a good antihypertensive effect shown by beta-adrenoblockers and/or a course of physical trainings. In type II (hypertensive responses), alpha-adrenoblockers and diuretics proved to be more effective. A relationship was found between the type of responses and the activity of the renin-aldosterone system.

RESUMEN

To specify indications for isovolumic hemodilution, hemostatic parameters (fibrinogen, soluble fibrin, fibrin decomposition products, platelet aggregation, thromboelastograms) were compared with clinical manifestations of coronary heart diseases (CHD) and coronarographic and bicycle-ergometric findings in 41 coronary patients. The increased blood viscosity syndrome was found in 63% of the patients. Isovolumic hemodilution was performed to control the detected disorders in 17 patients. Hemodilution was shown to be an effective means of controlling hemostatic disorders in coronary patients, hemostatic normalization being accompanied with clinical improvement in part of the cases.

Echocardiographic investigation revealed changes in central hemodynamics and myocardial contractility, associated with lasting hypokinesia, in coronary patients after the amputation of a limb that differed significantly from those recorded in patients with intact limbs. Amputation of a limb, lasting hypokinesia, physical overstrain, negative emotions, disorders of myocardial electric and contractile functions, central and peripheral hemodynamic disorders all contribute to the risk of coronary disease, myocardial infarction and death in post-amputation patients.

RESUMEN

Echocardiography was conducted in 169 patients with ischemic heart disease (atherosclerotic and postinfarction cardiosclerosis, angina pectoris, unstable angina pectoris, myocardial infarction) to determine the diagnostic importance of changes in the contractile function of the interventricular septum (IVS). It is shown that no essential changes in IVS contractility detectable by echocardiography are encountered in patients with ischemic heart disease in a period clear of exacerbation. During an attack of angina pectoris, a decrease of the mean normalized rate of IVS systolic displacement is recorded, which suggests indirectly a lesion of the left anterior descending artery. IVS contractile function increases in the pre-infarction period, which is a compensatory reaction to the changes in the contractile function of the left ventricle.

RESUMEN

Simultaneous study was conducted of the state of central hemodynamics and indices characterizing the rheologic blood properties in 25 patients with acute myocardial infarction. Patients with left ventricular insufficency were found to have considerably more marked disorders of rheologic blood properties. Experimental research conducted on dogs with a model of acute myocardial infarction confirmed the important role of disorders of rheologic and platelet-forming properties of blood in the prognosis and course of the acute period of the disease.
