RESUMEN

AIM: To evaluate the efficacy and safety of itopride used to treat the symptoms of functional dyspepsia (FD) of the upper gastrointestinal tract. SUBJECTS AND METHODS: A prospective, open-label, multicenter trial using as a control the placebo response obtained in the previous investigations enrolled 96 adult patients. The diagnosis of FD corresponded to its Rome II criteria. Patients received itopride (Ganaton) oral tablets (50 mg) 3 times daily for 8 weeks. When included into the trial, the patients were orally given itopride (ganaton) tablets (50 mg) thrice daily before meals for 8 weeks. The patients' status was evaluated during (at weeks 4 and 8) and after (at week 12) treatment. Treatment response was assessed using the Global Patient Assessment (GPA) and the Leeds Dyspepsia Questionnaire (LDQ). To evaluate the safety of itopride use, the investigators studied the frequency of adverse events and carried out laboratory tests (renal and liver function tests) and electrocardiography (ECG). RESULTS: The GPA showed that 53.76, 85.71, and 82.22% of the patients achieved a therapeutic effect of itopride at weeks 4, 8, and 12, respectively. The proportion of the patients who achieved the therapeutic effect (86%) at week 8 was higher than the historical placebo controls in the previous studies--45% (86% vs 45%; X2 = 68.868, df = 3; p < 0.001). The mean LDQ score at week 8 was significantly lower than that at baseline (2.09 and 9.36 scores; p < 0.001); 6 nonserious adverse events occurred in 3 (3.12%) of the 96 patients. During the follow-up period, there was a mild adverse event that was related to the test drug (atrial extrasystole as evidenced by ECG) and resolved a few days later. CONCLUSION: Itopride is an effective and well-tolerated drug in the treatment of functional dyspepsia in the Russian patients.

AIM: To assess efficacy of treatment of patients with atherogenic dyslipidemia (ADL) with beekeeping products (honey, pollen, bee bread). MATERIAL AND METHODS: ADL parameters were examined in 157 patients (64 males and 93 females) aged 39 to 72 (mean age 61,7 + 8,5 years) with ADL. Products of beekeeping were given in the absence of allergy and individual resistance to honey, pollen, bee bread. The patients were divided into four groups: patients on hypolipidemic diet only, on diet and honey or pollen, on bee bread, combined treatment - diet, honey, pollen. RESULTS: A significant hypolipidemic effect was registered in patients taking honey in combination with pollen (total cholesterol decreased by 18,3 %, LDLP cholesterol by 23,9 %) and bee bread (total cholesterol decreased by 15,7 %, LDLP cholesterol by 20,5 %). CONCLUSION: Improvement of blood lipid composition in taking honey and pollen in overweight (body mass index - BMI 25 - 30) and obese (BMI over 30) patients occurs only in loss of body mass.

This study examines gastric acetic ulcer healing in the rat after administration of honey, royal jelly and propolis into the stomach. Chronic gastric ulcers were induced in male Wistar rats by the application of 100% acetic acid to the serosal surface of the stomach on 60 sec. Bee-keeping products were administrated into the stomach from 2nd to 7th day after acetic ulcer induction. On 7th day animals were killed, and ulcer area was measured in mm2. In gastric juice pH and activity of pepsin were measured. The healing of acetic ulcers is accelerated with the administration of honey, royal jelly or propolis during six days. The largest healing effect was demonstrated with propolis and royal jelly, smaller one with the honey. It was revealed decrease of stomach acid secretion in the rats, which have received bee-keeping products versus the rats of control group.

RESUMEN

Natural bee honey is one of compound natural products in which structure more than four hundred various components are revealed, including enzymes, organic acids, vitamins and microelements. One of the basic biological properties of honey is the ability to slow down processes of aging, because there are vitamins E, C, enzymes with antioxidative properties and a succinic acid in its structure. Examination of 193 beekeepers daily using honey in quantity of 57.2 +/- 8.6 gram with definition of their biological age was carried out. The received results have been compared to results of examination of 35 workers who are doing manual labour in the same degree, as the beekeepers, but do not use products of beekeeping. The research has shown that the biological age of 70% of beekeepers is lower than that of the average in population, 15% of beekeepers are of the same and 15% are of higher biological age than that of the average in population. The biological age of people in the group of comparison is lower than the average in population only in 28.6% of cases, corresponds in 31.4% and is higher than the average in population in 40.0% of cases. The biological age of beekeepers appeared not only less, than of the persons who are not using products of beekeeping, but it also is less than biological age of the population as a whole.

RESUMEN

When studying the state of metabolism of the main substance of the connective tissue (glycosaminoglycan (GAG) and one of gag-hydrolases) in case of stomach and duodenal ulcers in patients under in-patient clinic conditions, it was shown that the amount of GAG in the urine approximately doubles in both groups in the state of exacerbation, and the given index reduces to the norm in the state of remission with a insignificant fluctuation of the b-glucuronidase level in the blood serum.

The results of the examination and 7 days treatment of the patients with duodenal ulcer associated with Helicobacter pylori (HP) by rebeprasol (20 mg 2 times per day), clarithromycin (500 mg 2 times per day) and Amoxicillin (1000 mg also 2 times per day) are presented. After treatment the patients did not receive any therapy. Before and after treatment of the patients the HP sowing of stomach mucosa was determined by Clo-test and histological method. The adhesion of bulb and duodenal ulcers is marked in all 10 patients on the data of esophagogastroduodenoscopy (EGDS) just after treatment. The second control EGDS (4 weeks after treatment) spending in 8 patients showed the adhesion of ulcers and HP eradication (in all cases). The research is continued. The obtained results on the adhesion of bulb and duodenal ulcers and HP eradication testify the efficiency of suggested scheme of the 7-days antihelocobacter therapy of the non-complicated duodenal ulcer associated with HP.

The results of one-week eradication therapy of uncomplicated duodenal ulcer disease associated with Helicobacter pylori by Ezomeprasol, Clarithromycin and Amoxicillin (accordingly 20 mg, 500 mg and 1000 mg twice per day) are presented. The results of the adhesion of uncomplicated ulcers of duodenal bulb in 4 weeks after termination of eradication treatment course--the adhesion of the ulcers was observed in 17 of 19 patients (89.5%). The eradication of Helicobacter pylori according the data of fast urease test and 13C-urease respiratory test was determined in all 19 patients (100%), on the data of histological method--in 18 of 19 patients (94.7%).

Apitherapy is treatment of diseases with biologically active products of bee-keeping (BAPB), which is developing in an intensive way all over the world. The interest in apitherapy is explained, on the one hand, by a great number of natural compounds produced by bees as a result of their vital functions and having high physiological activity, and on the other hand, by the universal nature of bees occurrence and comparative simplicity of getting the bee-keeping products. In apitherapy literature many authors point to the fact that honey has an impact on gastric secretion: a cold honey solution stimulates, and a warm one inhibits acid excretion. Yet there are no results of studies confirming this action in all publications.
