RESUMEN

As many as 208 patients with carcinoma of the stomach were examined, with 98 of these, having been victims of Chernobyl accident. It has been established that in spite of the fact that the clinical picture was practically identical in both case groups those patients having been exposed to mixed radiation demonstrated more clearly manifest endogenous intoxication, impairement of erythrocytic membrane function, which fact calls correction of these changes in the preoperative period.

RESUMEN

Tramal, the drug produced by the "Grunenthal" firm (FRG) was used in the treatment of 478 patients with malignant neoplasms. In 24.46% patients, the side effects were noted, which were characteristic of the majority of analgetics and didn't threaten the life. The authors recommend to use Tramal for cupping off the pain syndrome at the postoperative period, and in incurable patients as well.

RESUMEN

Changes of cytochemical indices and phagocytic activity of neutrophil granulocytes of the peripheral blood were revealed in patients with cancer of the rectum at different stages of combined treatment. These changes evidence an increase of vascular permeability and catabolism, disorders of energy resources, oxidation-reduction processes and functional activity of granulocytes. The dynamics of changes of cytochemical indices of leucocytes reflect the efficacy of different methods of treatment and prophylaxis of postoperative purulent complications.

RESUMEN

The study including 414 patients aged over 60 years suffering lung esophageal, gastric and colorectal cancer was carried out to assess the possibility of preventing postoperative complications. Preoperative preparation, anesthesiologic medication and drug treatment in the early postoperative period included vitamins, eleutherococcus, sodium oxybutyrate, retabolil, splenin, curantyl, heparin, panangin, sulfalen and trichopol. In that group, the overall postoperative complication rate was 1.5 times lower and that of purulent complications, cardiovascular disorders, pneumonia and pancreatitis--2 times lower than those in 642 controls. The duration of postoperative stay in the hospital was shorter and the postoperative lethality tended to decrease. Topical treatment with methylsiloxane-immobilized gentamycin and 5-fluorouracil was shown to prevent development of purulent complications and pancreatitis.

RESUMEN

The kallikrein-kinin system of the blood was studied in 59 patients with tumours of the abdominal cavity and small pelvis. In 33 patients, the postoperative course was complicated by wound suppuration, formation of intraabdominal and pelvic abscesses. It was established, that the development of purulent complications was accompanied by regular activation of the system of proteolysis and increase in the blood level of the middle molecular mass substances. The changes in the indices of kallikrein-kinin system can serve as an additional criterium of the effectiveness of treatment and prophylaxis of postoperative complications.

RESUMEN

On the basis of the analysis of the results of treatment by the methods of cryosurgery, roentgeno-endovascular embolization, local use of immobilized chemo-preparations and systemic administration of a high-molecular platinum compound of 218 patients with IV stage tumours of different location, it is shown that the mean lifetime of the patients increases 12-fold, the number of postoperative complications and lethality decrease.

RESUMEN

A long-acting dosage form for local use of gentamicin immobilized on polymethylsiloxane, a silicon organic adsorbent was developed. It combined the antimicrobial spectrum of gentamicin and the local sorption-detoxication action of the matrix. In acute and chronic experiments on 5 species of laboratory animals it was shown that polymethylsiloxane had no general toxic action on the animals, no damaging action on their internal organs, did not affect their functions and the state of the biological fluids, had no pyrogenic or allergenic effect. During gentamicin immobilization on polymethylsiloxane there was observed no increase in the antibiotic toxicity as compared to the nonimmobilized dosage form of the antibiotic. Further study of the immobilized dosage form of gentamicin is advisable.

RESUMEN

The study was concerned with postoperative course and dynamics of biochemical indexes for liver function and degree of intoxication in 47 cancer patients suffering mechanical jaundice. The patients received hemosorption preoperatively. Hemosorption was found to significantly increase the efficacy of detoxifying therapy and to contribute to prevention of postoperative complications and lethality.

RESUMEN

Hemosorption was used in complex treatment of postoperative diffuse peritonitis in 69 cancer patients. A pronounced detoxicating effect manifested itself in removal of toxic metabolites from blood matched by an improvement in liver, kidney and cardiovascular function and blood rheologic properties. As a result, lethality rate fell to as low as 69.6% versus 88% in a group of 73 patients in whom the procedure was not used.
