RESUMEN

BACKGROUND: Improving the level of self-management by people undergoing hemodialysis is an effective way to reduce the incidence of mortality and complications and improve quality of life. A better understanding of what influences an individual's level of self-management can help nurses find appropriate ways to improve self-management. OBJECTIVES: To examine self-management levels, and discuss the factors influencing self-management, in a sample of patients undergoing hemodialysis in Beijing. DESIGN: A descriptive study design was chosen. SETTINGS AND METHODS: A convenience sample of 216 patients undergoing hemodialysis was recruited from dialysis centers in three tertiary hospitals in Beijing from September 2010 to January 2011. Questionnaires were used to examine the variables: level of self-management; knowledge of hemodialysis; self-efficacy; anxiety and depression; and social support. Data analysis involved descriptive statistics, including frequency, percentage, mean and standard deviations, while Spearman correlation, non-parametric Z and χ(2) and multiple linear regression were used for comparative purposes. RESULTS: The number of returned questionnaires was 198 (91.67% response rate). The overall score of self-management was 56.01 (SD=10.75). The average item scores for each of the four self-management subscales were 3.02 for partnership, 2.98 for problem-solving skills, 2.74 for self-care and 2.47 for emotional management. Multiple linear regression analyses for overall self-management and the four subscales indicated that knowledge, self-efficacy, the availability of social support and depression were the main influencing factors which explained 34.1% of the variance of self-management. CONCLUSION: The level of self-management by those undergoing hemodialysis in this study was less than ideal, varying from 'rare' to 'sometimes' for use of self-management behaviors. For the different subscales of self-management, partnership was the most used and emotional management the least used strategy. Patients' knowledge, self-efficacy, the availability of social support and depression were the main factors influencing self-management.

OBJECTIVE: To study the difference of histologic examination and type II collagen expression in the rib cartilage of patients at different age. METHODS: The patients were divided into three groups, as children group (5 to 10 years old), teenager group (11 to 17 years old), and adult group (18 to 29 years old). Histologic examination of rib cartilage and quantitative analysis of type II collagen with immunohistochemical stain were carried out in the three groups. Motic Med 6.0 A system was used to quantitate immunohistochemical image through measurement of mean photodensity. The results were statistically analyzed. RESULTS: Among the three groups, the children's rib cartilage had the most blood vessels, the most chondrocytes, well-distributed stain of matrixes, and the type II collagen was expressed actively and highest in photodensity. The rib cartilage of teenager group had less blood vessels, unhomogeny distributed stain of matrixes, the enlarged and separated cartilage lacunae. The rib cartilage in adult group showed the least blood vessels, the least chondrocytes, the hyalinization of perichondrium, the most deposition of calcium salt, and the type II collagen was expressed at the lowest level in photodensity. Statistical analysis showed significant differences among the three groups (P < 0.01). CONCLUSIONS: Histologic structure in human rib cartilage changes during the aging process and the type II collagen is decreased with the increase of age.

Abstract The base composition pattern (BCP) in the putative promoter region (PPRs) up to 5 Kb lengths of 682 human genes on Chromosome 22 (Chr22) was examined. Two-dimensional (2D) and three-dimensional (3D) functions were designed to delineate the DNA base composition, with four major patterns identified. It is found that 17.6% genes include TATA box, 28.0% GC box, 18.9% CAAT box and 38.4% CpG islands, and approximately 10% genes have one of four putative initiator (Inr) motifs. The occurrence of the promoter elements is tightly associated with the base composition features in the promoter regions, and the associations of the base composition features with occurrence of the promoter elements in the promoter regions mediate tissue-wide expression of the genes in human. The occurrence of two or more promoter elements in the promoter regions is required for the medium- and wide-range expression profiles of the human genes on Chr22. Thus, the reported data shed light on the characteristics of the PPRs of the human genes on Chr22, which may improve our understanding of regulatory roles of the PPRs with occurrence of the promoter elements in gene expression.
