RESUMEN

Bilateral robotic rehabilitation has proven helpful in the recovery of upper limb motor function in patients with stroke, but its effects on the cortical reorganization mechanisms underlying recovery are still unclear. This pilot Randomized Controlled Trial (RCT) aimed to evaluate the effects on the interhemispheric balance of unilateral or bilateral robotic treatments in patients with subacute stroke, using Quantitative Electroencephalography (qEEG). 19 patients with ischemic stroke underwent a 30-session upper limb neurorehabilitation intervention using a bilateral upper limb exoskeleton. Each patient was randomly assigned to the bilateral (BG, n=10) or unilateral treatment group (UG, n=9). EEG evaluations were performed before (T0) and right after (T0+) the first treatment session, after 30 treatment sessions (T1), and at 1-week follow-up (T2), in both eyes open and eyes closed conditions. From the acquired EEG data, the pairwise-derived Brain Symmetry Index (pdBSI) was computed. In addition, clinical evaluation was performed at T0 and T1 with validated clinical scales. After the treatment, a significant improvement in clinical and EEG evaluations was observed for both groups, but only the BG showed reduced pdBSI in delta and theta bands. In the cluster of sensorimotor channels, there was no significant difference between groups. The observed changes were not maintained at follow-up. No significant changes were observed in the pdBSI after a single rehabilitation session. Results suggest that balancing of interhemispheric symmetry comes along with a clinical improvement in the upper extremity and that the pdBSI can be used to investigate the mechanisms of neuronal plasticity involved in robotic rehabilitation after stroke.

RESUMEN

BACKGROUND AND METHODS: On behalf of the Gynecologic Oncology Group, we performed a randomized trial of radiotherapy in combination with three concurrent chemotherapy regimens -- cisplatin alone; cisplatin, fluorouracil, and hydroxyurea; and hydroxyurea alone -- in patients with locally advanced cervical cancer. Women with primary untreated invasive squamous-cell carcinoma, adenosquamous carcinoma, or adenocarcinoma of the cervix of stage IIB, III, or IVA, without involvement of the para-aortic lymph nodes, were enrolled. The patients had to have a leukocyte count of at least 3000 per cubic millimeter, a platelet count of at least 100,000 per cubic millimeter, a serum creatinine level no higher than 2 mg per deciliter (177 micromol per liter), and adequate hepatic function. All patients received external-beam radiotherapy according to a strict protocol. Patients were randomly assigned to receive one of three chemotherapy regimens: 40 mg of cisplatin per square meter of body-surface area per week for six weeks (group 1); 50 mg of cisplatin per square meter on days 1 and 29, followed by 4 g of fluorouracil per square meter given as a 96-hour infusion on days 1 and 29, and 2 g of oral hydroxyurea per square meter twice weekly for six weeks (group 2); or 3 g of oral hydroxyurea per square meter twice weekly for six weeks (group 3). RESULTS: The analysis included 526 women. The median duration of follow-up was 35 months. Both groups that received cisplatin had a higher rate of progression-free survival than the group that received hydroxyurea alone (P<0.001 for both comparisons). The relative risks of progression of disease or death were 0.57 (95 percent confidence interval, 0.42 to 0.78) in group 1 and 0.55 (95 percent confidence interval, 0.40 to 0.75) in group 2, as compared with group 3. The overall survival rate was significantly higher in groups 1 and 2 than in group 3, with relative risks of death of 0.61 (95 percent confidence interval, 0.44 to 0.85) and 0.58 (95 percent confidence interval, 0.41 to 0.81), respectively. CONCLUSIONS: Regimens of radiotherapy and chemotherapy that contain cisplatin improve the rates of survival and progression-free survival among women with locally advanced cervical cancer.

RESUMEN

A case of amphicrine carcinoma of the uterine cervix is described in a 27-year-old woman whose initial cervical biopsy specimen showed small cell undifferentiated neuroendocrine carcinoma. Despite an initial complete clinical response to VP-16 and carboplatin, residual tumor was noted in the cervical cone biopsy specimen and she was treated with external-beam irradiation and cesium implants. The neoplasm subsequently metastasized and required additional VP-16 and carboplatin for control. The pathologic features and biologic behavior of this unusual type of neuroendocrine neoplasm are described in this case report.

RESUMEN

An episode of unilateral ureteral obstruction secondary to sarcoidosis is described. Corticosteroid treatment resulted in prompt and complete resolution of the obstruction and associated lymphadenopathy. We believe this represents the first reported case of ureteral obstruction secondary to sarcoidosis.

RESUMEN

Small concentrations of methemoglobin are present in the blood of normal individuals. Increased concentrations of methemoglobin can be formed by the action of certain chemicals or drugs, or in individuals with specific genetic defects. There is little information available concerning the validity of postmortem methemoglobin concentration as an indicator of antemortem methemoglobinemia. We measured blood concentrations of methemoglobin in 49 autopsy specimens. We conclude that postmortem methemoglobin concentrations are not valid indicators of antemortem methemoglobinemia.

RESUMEN

The treatment of patients with stage A prostatic carcinoma demands precise pathological review. In an attempt to evaluate these patients thoroughly earlier reports from our institution recommended repeat transurethral biopsy. Recent studies have reaffirmed the benign nature of stage A1 disease. To examine the efficacy of this procedure a referee pathologist reviewed 8 years of experience with patients with stage A prostatic carcinoma from our institution. Clinical followup was more than 5 years in most cases. Of the 40 patients who underwent repeat transurethral biopsy the tumor in 38 remained stage A1 pathologically. While 1 patient died of the tumor the the remainder are without clinical disease. This low incidence of upstaging does not support the routine use of repeat resections.

RESUMEN

Benign testis tumors are reported to be extraordinarily rare. Recent clinical experience has shown a high prevalence (7 of 12 patients) of these tumors, which, although small, is much higher than other reports. We herein present 4 illustrative cases and a practical categorization of the varied pathologic events causing these tumors.

Cytochemical, ultrastructural, and terminal deoxynucleotidyl transferase determinations in three cases of acute lymphocytic leukemia with azurophilic granules and punctate alpha-naphthyl acetate esterase (ANAE), alpha-naphthyl butyrate esterase (ANBE), and acid phosphatase (AP) activities further described a cytochemical and morphologic variant of acute lymphocytic leukemia. The ANAE, ANBE, and AP activities appeared specially localized within cytoplasmic granules. Electron microscopy substantiated a prominence of membrane-bound dense-core cytoplasmic granules. The variegated appearance of these dense-core granules and the cytochemical profile suggest that the aberrant granules are lysosomal in nature. Both the prominent cytoplasmic granules and the older age distribution led to early misdiagnosis of myelocytic leukemia. The distinctive cytochemical/morphologic profile and the ease of misdiagnosis suggest an important morphologic variant of acute lymphocytic leukemia.

A breast tumor with the morphologic features of a carcinoid tumor and containing large amounts of estrogen receptor protein was associated with areas of typical in situ and infiltrating lobular carcinoma. The carcinoid areas were argyrophilic and ultrastructurally contained electron dense granules on which silver grains were localized. Of 21 other examples of ordinary breast cancer, five showed histologic similarities to carcinoid tumors. Focal argyrophilia was observed in 11 invasive tumors. The presence of argyrophilic granules could not be correlated with the presence of "neurosecretory" granules, although groups of such granules were found in one case of in situ lobular carcinoma. Argyrophilic and "neurosecretory" granules imply the presence of hormonal substances, although in breast tumors these granules have not yet been chemically or immunochemically characterized. Until such evidence becomes available it would appear that most cases reported as primary carcinoid tumors of the breast have much more in common with conventional breast cancer than with the usual carcinoid tumor. Within this context it can be acknowledged that some breast cancers may focally exhibit a carcinoid-like differentiation.

RESUMEN

Melanosis involving the duodenum is extremely rare. We present endoscopic, microscopic, and histochemical studies of a patient with melanosis of the duodenum associated with a gastric ulcer and a folic acid deficiency. The pigment is similar to that observed in melanosis coli and histochemically does not stain like melanin or lipofuscin. The pigment disappeared with correction of the folate deficiency and healing of the gastric ulcer, suggesting a possible relationship with these entities.
