RESUMEN

Objective:To explore the mechanism of zoledronic acid (ZOL) affects osteogenic differentiation and bone formation through the regulation of sirtuin 3 (SIRT3) / P53 expression.Methods:Bone marrow mesenchymal stem cells (BMSCs) were induced to differentiate into osteogenic cells, the expression of SIRT3 in the cells was detected, and the targeting regulation relationship between SIRT3 and P53 was analyzed. The intracellular expressions of SIRT3 and P53 were intervened and ZOL was used to treat the cells. MTT method, Western blot method and kit were used to detect cell viability, osteogenesis-related genes Osteoprotegerin (OPG), runt-related transcription factor 2 (Runx2) expression, alkaline phosphatase (ALP) activity and alizarin red S (ARS) staining, respectively. Ovariectomy (OVX) was used to construct a rat model and explore the effect of ZOL on the progression of osteoporosis (OP) in vivo.Results:ZOL promoted osteogenic differentiation of BMSCs. The expression of SIRT3 was down-regulated in the serum of OP patients (0.78±0.23) compared with that of healthy subjects (1.00±0.26 vs. 0.78±0.23. t=3.85, P<0.001). During the osteogenic differentiation of BMSCs, the expression level of SIRT3 gradually increased with the prolonged induction of osteogenesis. Compared with the p53 protein expression and BMSCs activity in the control group, SIRT3 knockout could increase the expression level of p53 protein (0.59±0.05 vs. 1.01±0.11. t=6.02, P=0.004) but inhibited the activity of BMSCs (100.00±8.41 vs. 51.26±5.59. t=8.36, P=0.001). After ZOL treatment, the inhibitory effect of SIRT3 on cell viability (49.61±5.11 vs. 87.61±7.31. t=7.38, P=0.002) and osteogenesis was relieved, and the level of P53 was inhibited (1.10±0.10 vs. 0.69±0.04. t=6.59, P=0.003). P53 overexpression partially offseted the effects of ZOL on cell viability (84.61±6.52 vs. 66.54±5.47. t=3.68, P=0.021) and osteogenesis. Compared with the sham surgery group, the OVX group showed inhibition of osteogenesis in rats, and ZOL treatment significantly improved osteogenic inhibition. ZOL treatment increased the expression level of SIRT3 protein in bone tissue of OVX rats, but inhibited the expression level of P53. Conclusion:ZOL promoted osteogenic differentiation and bone formation of BMSCs by promoting the ubiquitination and degradation of P53 by SIRT3.

RESUMEN

Objective:To construct an anticipatory grief intervention scheme for family caregivers of advanced cancer patients based on narrative theory, and to provide reference for anticipatory grief nursing intervention.Methods:From October 2022 to May 2023, through literature research, semi-structured interview and brainstorming method, the first draft of nursing intervention plan was constructed, the Delphi method was used to conduct 2 rounds of correspondence consultation with 15 experts, and the indicators at all levels were modified according to the opinions of experts, and the final draft of intervention plan was formed.Results:The experts were all female, aged (49.67 ± 5.83) years old. The authority coefficient of the two rounds of experts was 0.87. The Kendall coordination coefficients of the first, second, and third level indicators after the first round of expert inquiry were 0.195, 0.113, and 0.093, respectively. The Kendall coordination coefficients of the first, second, and third level indicators after the second round of expert inquiry were 0.200, 0.119, and 0.101, respectively. The differences were statistically significant ( χ2 values were 8.76-107.21, all P<0.05).Finally, a nursing intervention plan based on narrative theory was formed, which included 4 primary indicators, 19 secondary indicators and 72 tertiary indicators. Conclusions:The anticipatory grief intervention scheme for family caregivers of advanced cancer patients is scientific, practical and feasible, and can be used for psychological nursing of family caregivers.

RESUMEN

In the context of modern medicine, it’s difficult for doctors lacking narrative thinking to get into the hearts of patients and offer high-quality medical service. Precise classification terms and decontextualized abstract language have become communication barriers between doctors and patients, creating an atmosphere of unusual indifference and fear during the medical process. William Osler, the father of modern medicine, emphasized that doctors should respect the individuality of patients, and advocated that doctors should use life-oriented language to provide humanistic care to patients. This echoes the concept of the “doctor-patient narrative community” in the construction of the Chinese narrative medicine system. As a brand new clinical humanistic practice path, narrative medicine focuses on the keyword “narrative” and revolves around the theme of “inter-subjective relationship” in the medical context, advocates that doctors switch flexibly in the two references of science and life, achieve visual fusion with patients and their families, pay attention to patients’ life experiences, and establish narrative connections with them. On this basis, it can achieve two-way narrative interaction, build a harmonious doctor-patient narrative community, and thus enhance the patient’s medical experience.

RESUMEN

Non-infectious uveitis, an autoimmune disease that can cause severe visual impairment, can be difficult to treat. According to the prevailing hypothesis, the immune-mediated imbalance that contributes to non-infectious uveitis is primarily driven by CD4+T cells. However, recent research has shown that B cells also play a significant role in this process, participating in various ways such as antibody production, antigen presentation, and cytokine secretion in both human uveitis and experimental autoimmune uveitis models. Therapies targeting B cells have been used extensively in various autoimmune diseases. Rituximab, a B-cell inhibitor, is effective in treating noninfectious uveitis that is unresponsive to conventional corticosteroid and immunosuppressive therapy. This paper provides an overview of the involvement of B cells in non-infectious uveitis and their potential use in cellular therapies, aiming to further investigate the mechanisms and develop more effective strategies for prevention and treatment.

Objective To find,screen and describe indicators for hospital quality evaluation.Methods Scoped framework reviews were conducted using the PRISMA flowchart.The search period was from the establishment of the database to January 2023.Six databases were searched.EndNoteX10 software was used to select references,and inclusion and exclusion criteria were established for literature screening.Extracts,analyzes and summarizes data according to research questions,and makes inductive analysis of indicators.Results 39 literatures were included in the analysis,including 18 Chinese literaturesand 21 foreign literatures,the earliest of which was published in 1992.A total of 8 indicator theoretical models,5 indicator selection methods,and 12 indicator weighting methods were involved.Donabedian model is the most commonly used evaluation indicator model.Literature research method is the most frequently used indicator selection method.Analytic hierarchy Process is the most frequently used method of indicator weighting.Among the top ten indicators of hospital quality evaluation at home and abroad,the mortality rate and the utilization rate of beds both ranked the first three.The cleanliness of hospital environment ranked second and the professional spirit of doctors ranked tenth in the quality indicator of foreign hospitals.The top ten domestic cost,average length of stay,intensity of antibacterial drug use and case mix index were not mentioned in the quality indicator of foreign hospitals.Conclusion Medical quality is the most concerned core dimension of hospital quality evaluation at home and abroad.The cleanliness of hospital environment,professional spiritofdoctors,understanding and communication between doctors and patients can be included in domestic hospital quality evaluation indicators.

RESUMEN

Objective:To preliminarily develop a fertility motivation scale for infertile women (FMS-IW) and test its reliability and validity.Methods:The FMS-IW was developed according to the theory of self-determination and Maslow's hierarchy of needs, and the original item pool was established through clinical psychological counseling practice experience, literature review, consulting clinical psychotherapists, interviewing with infertile women and open-ended questionnaires. The original scale was constructed on item analysis and exploratory factor analysis in 257 infertile women. The formal version of FMS-IW was further adapted by Delphi method and tested in another 392 women to conduct confirmatory factor analysis and reliability test. Furthermore, 56 participants of 392 women were randomly retested with FMS-IW after two weeks.SPSS 25.0 and Amos 24.0 software were used for Spearman analysis, exploratory factor analysis and confirmatory factor analysis.Results:Exploratory factor analysis showed that the FMS-IW was composed of 16 items consisting of two factors: autonomous fertility motivation and controlled fertility motivation. The cumulative variance contribution rate was 64.18%. Confirmatory factor analysis showed that the model fitted well ( χ2/ df=3.292, RMSEA=0.077, SRMR=0.055, GFI=0.902, AGFI=0.871, IFI=0.938, CFI=0.938, TLI=0.928). The Cronbach's α coefficient of the FMS-IW was 0.908. The Cronbach's α coefficient for autonomous and controlled fertility motivation was 0.911 and 0.928, respectively. The parity split-half coefficient of the formal version of FMS-IW was 0.870. The test-retest reliability of the formal version of FMS-IW was 0.823. Conclusion:The FMS-IW has good reliability and validity, and can be used as an effective tool to measure fertility motivation in infertile women.

OBJECTIVE@#To explore the clinicopathological features and types of genic mutations in DNA mismatch repair (MMR) in colorectal cancer (CRC).@*METHODS@#Immunohistochemistry was used to determine the expression of MMR proteins in 1394 patients with CRC, and PCR-capillary electrophoresis (PCR-CE) was used to detect microsatellite instability (MSI) in 106 cases of defective MMR (dMMR), 46 cases of proficient MMR (pMMR) with heterogeneous expression and 147 randomly selected cases of pMMR. The relationship between the expressions of MMR proteins and the clinicopathological features of the patients was evaluated. The consistency between the results of immunohistochemistry and PCR-CE was assessed.@*RESULTS@#Immunohistochemical staining showed an incidence of dMMR of 7.6% in the patients. The main type of dMMR was co-deletion of MLH1 and PMS2, accounting for 55.7% of the total dMMR cases. The deletion of MMR proteins was significantly correlated with the patients' age, tumor location, tumor size, gross type, histological type, degree of differentiation, lymph node status and TNM stage (@*CONCLUSIONS@#The main type of dMMR is co-deletion of MLH1 and PMS2 in patients with colorectal cancer. dMMR colorectal cancer has typical clinicopathological features and a lower incidence in China than in Western countries. The results of immunohistochemistry and PCR-CE are highly consistent for detecting dMMR in colorectal cancer patients.

RESUMEN

Objective To observe the early clinical outcomes of iliotibial band with Gerdy's tubercle transplantation for irreparable rotator cuff tears (IRCT).Methods A total of 16 patients with IRCT who underwent iliotibial band with Gerdy's tubercle transplantation from June 2014 to June 2015 and with a minimum follow-up of 2 years were retrospectively analyzed.There were 4 male and 12 female with a mean age of 56.1±6.6 years old (range 44-67 years old).The X-ray and MRI were used to evaluate the improvement of acromiohumeral interval (AHI),rotator cuff healing and final outcome of bone graft postoperatively.Functional outcomes were assessed by visual analogue scale (VAS),American Shoulder and Elbow Surgeon (ASES) and Constant-Murley score.Results The average follow-up duration was 25.3±1.5 months (range 24-48 months).At the final follow-up,the forward flexion,external rotation and internal rotation were 159.37°±8.51°,35.31°±8.26° and 11.00°±2.52°,respectively.There was statistically significant difference between preoperative and postoperative joint range of motion (P < 0.05).At the 3 months postoperatively,all patients had complete bone union between the Gerdy's tubercle and the greater tubercle.However,at the final followup,partial or complete bone absorption was observed in 6 patients.Twelve patients had complete rotator cuff healing,and 4 patients had partial healing.No patient had rotator cuff retear.The AHI increased significantly from 3.63±1.25 mm preoperatively to 8.37± 1.02 mm postoperatively (P < 0.05).The postoperative VAS,ASES and Constant-Murley score were 1.06±0.93,81.75±6.80 and 77.12± 6.72,respectively.The differences between preoperative and postoperative VAS,ASES and Constant-Murley score were statistically significant (P < 0.05).There was significant difference in postoperative VAS between bone absorption group and non-absorption group (P < 0.05),while no significant difference was found in ASES or Constant-Murley score (P > 0.05).Conclusion All patients had complete or partial rotator cuff healing.However,37.5% of the patients had bone absorption and obtained a worse VAS score.The iliotibial band with Gerdy's tubercle transplantation for IRCT could significantly restore the joint function at 2 years postoperatively.

RESUMEN

Objective To assess the cesarean section scar morphology and size with transvaginal ultrasound and the healing of incision diverticulum after the repairing operation.Methods Forty cases with cesarean section scar defects needed repairing operation,40 cases of cesarean section without symptoms and 40 cases of vaginal delivery were involved.The scar condition and measured the size of cesarean section defects were observed.For the 40 cases needed repairing operation,the healing of the scar and measured the size of the defects were observed which still existed before and after the surgery.For the transvaginal delivery cases the thickness of uterine isthmus were measured.Results After the scar defects repairing operation,there were 9 cases who still had diverticulum,but the defects were smaller than that before operation (P <0.05).The symptoms were relieved.Among the 40 asymptomatic cases,there were 1 1 cases had defects,but the diverticulum were smaller than that of needed operation patients (P < 0.05 ). Conclusions The transvaginal ultrasound is a noninvasive and convenient method to observe the cesarean section scar.

Objective To explore the clinical effect of transvaginal repair of cesarean scar diverticulum (CSD). Methods Totally 64 patients of CSD in the First Maternity and Infant Hospital, Tongji University between Mar. 2013 and Sept. 2014 underwent transvaginal repair of CSD were reviewed retrospectively and followed. Result All the patients had a prolonged period, and the duration was (14.8± 3.5) days; all the patients were received the transvaginal repair of CSD, there was no intra-operative complications, the procedures were successfully performed in all patients. The mean operation time was (67± 12) minutes, the mean blood loss was (53±32) ml, and the mean length of hospital stay was (4.0±1.1) days. All patients were followed after the operation, the duration of menstruation was (8.1 ± 3.5) days shorter in average, which was statistically significant (P<0.01);the operation effective rate was 94%(60/64) to assess the clinic syptoms, the operation effective rate was 95%(61/64) for anatomic assessment. The distance of the CSD from the serosa became thicker after surgery significantly, the distance was thicker (3.4 ± 0.4) mm compared with preoperation (P<0.01). Conclusions Transvaginal repair of CSD offers minimal invasiveness, good exposure and accurate resection. It is worth to be popularized in the treatment of patients with CSD.

Objective To investigate the security characteristics of rural young boarding pupils and its influencing factors.Methods 210 boarding and 265 non-boarding rural young pupils of Huojia in Henan were tested by questionnaires,including of a self-developed questionnaire,security questionnaire and adolescent mental resilience scale.Results ①The scores of the interpersonal security (25.99 ± 5.53),the determine control sense factor (24.63 ±6.17) and the total security(50.62 ± 10.37) in the boarding pupils were statistically lower than non-boarding pupils (27.36 ± 6.38,26.00 ± 6.55,53.36 ± 11.77,P <0.05).②There were statistically significant difference in the scores of the interpersonal security,the determine control sense factor and the total security among the different grades(P<0.05),but there were no statistically sex difference in the scores(P > 0.05).③Group regression analysis showed that,no family factors in the young boarding pupils had influence on the score of the security sense factors and the total (P>0.05),and marital relationship of parents,only child,father' s and mother(')s education background of non-boarding pupils had certain influence on the score of the interpersonal security,the determine control sense factor and the total security (β =-0.43-0.27,P< 0.05).④The group regression analysis showed that,the problem solving and self-efficacy,parents'high expectations in mental toughness respectively had some effect on the interpersonal security of boarding and non-boarding pupils(β =-0.15,β =0.25,P<0.05),and social ability and family care,schools resilience respectively had influence on the determine control sense in boarding and non-boarding pupils (β =-2.91-2.87,P<0.05),and social ability,parents'high expectations respectively had influence on the total security in boarding and non-boarding pupils (β =-0.17,β=0.22,P<0.05).Conclusion The security-sense in boarding young pupils are lower than the non-boarding young pupils,and family factors and mental toughness have different effects on security-sense of two kinds pupils.

RESUMEN

Microbial components and the endogenous molecules released from damaged cells can stimulate germ-line-encoded pattern recognition receptors (PRRs) to transduce signals to the hub of the innate immune signaling network-the adaptor proteins MyD88/TRIF/MAVS/STING/Caspase-1, where integrated signals relay to the relevant transcription factors IRF3/IRF7/NF-κB/ AP-1 and the signal transducer and activator of transcription 6 (STAT6) to trigger the expression of type I interferons and inflammatory cytokines or the assembly of inflammasomes. Most pleiotropic cytokines are secreted and bind to specific receptors, activating the signaling pathways including JAK-STAT for the proliferation, differentiation and functional capacity of immune cells. This review focuses on several critical adaptors in innate immune signaling cascades and recent progress in their molecular mechanisms.
