RESUMEN

UNLABELLED: The Orthopaedic Nurses Certification Board (ONCB) conducts a role delineation study (RDS) every 5 years. Results identify tasks performed by orthopaedic registered nurses, nurse practitioners, and clinical nurse specialists, and musculoskeletal health conditions commonly seen by patients under their care. PURPOSE: The purpose of the study was to define current practice patterns among orthopaedic nurses and validate content for future certification examinations. METHOD: An online survey methodology was used to identify task and knowledge statements that represented orthopaedic nursing practice. FINDINGS: Of 5,634 e-mails sent, 1,194 valid responses were returned (response rate 22.7%). This is consistent with results of the Orthopaedic Nurses Certification Board's 2007 RDS (23.3% response rate) and is considered acceptable for an RDS. CONCLUSION: Survey results were analyzed with assistance of psychometric staff at Applied Measurement Professionals, Inc., and used to review and revise examination specifications for the 3 certification programs. New specifications were implemented with March 2012 testing.

RESUMEN

Sorafenib is a chemotherapeutic agent primarily used to treat metastatic renal cell carcinoma. It is a multikinase inhibitor that blocks cell proliferation and angiogenesis. Numerous cutaneous side effects have been reported in association with this medication, including acral erythema, inflammation of actinic keratoses, erythema multiforme, vasculitis, and keratoacanthomas. Up to 40 percent of patients on this medication develop dermatologic manifestations. We describe chloracne-like eruptions in two different patients with no exposure to aromatic hydrocarbons but who were recently started on sorafenib for treatment of metastatic renal carcinoma. The primary reason for discontinuation of sorafenib is secondary to its adverse side effect profile. Recognizing these effects early and administering appropriate treatment will likely increase medication compliance and minimize both dose reductions and discontinuation of the medication resulting in optimal treatment outcomes.

To determine the impact of the new Joint Academy on volume and length of stay (LOS), the authors undertook a retrospective record review for January-March of 2006 and 2007. A total of 413 primary and revision total joint replacements performed by two surgeons were reviewed, with 10.7% more procedures in 2007 than in 2006. The weighted average LOS in 2006 was 3.44 days; in 2007 it was 3.36 days. Significantly, the authors also compared volumes and LOS by weekday of admission and found no statistical variance in average length of stay (ALOS) by weekday of admission. Revisions negatively affected ALOS but added volume. It was concluded that the Joint Academy had a positive impact on volume and LOS, with most patients successfully discharged on postoperative day 3 due to the program's standardized order sets and appropriate resource allocation.
