RESUMEN
We report a case of intermittent chronic prostatitis for 13 years. Evaluation revealed a foreign body of the prostatic urethra as the etiology of the symptoms.

Blunt trauma to renal arteries often receives suboptimal management because of delayed diagnosis, incomplete preoperative evaluation, associated injuries, or unknown functional capacity of the contralateral kidney. We propose a rational treatment plan from an analysis of our three cases and a review of 118 cases. Two thirds of the patients were male, and left-sided injuries predominated over right 42.9% to 30.6%. In 20 cases the insult was bilateral. Associated injuries occurred in 72%, averaging 1.6 injuries per case, of which one half were intra-abdominal. Flank and/or abdominal pain was present in 73% of cases, tenderness in 65%, and gross or microscopic hematuria in 74%. Repairs were attempted on 38 renal arteries. "Successful" repairs were accomplished in 16 arteries, while repairs failed in 22. Subsequent hypertension developed in 32% of successes. Repair should be attempted in stable patients with unilateral injury of less than four hours' duration, bilateral injuries of less than 20 hours' duration, or injuries with nonocclusion on arteriogram.

Transitional cell carcinoma of the renal pelvis was encountered in a twenty-one-year-old white man. Review of the literature shows this tumor to be rare in individuals less than thirty years of age.

Retrograde loopography in 106 patients (212 renal units) with ileal conduit urinary diversion was assessed to define the accuracy of this study in identifying ureteroileal obstruction. In 171 renal units (81%), the radiographic findings demonstrated by retrograde loopography correlated appropriately with the findings on excretory urography. However, in 41 renal units (19%) with absent ileoureteral reflux, there was no evidence of ureteroileal obstruction by excretory urography. This potential disparity between excretory urography and retrograde loopography has limited our use of the latter study for selected patients in whom the excretory urogram demonstrates (1) evidence of progressive upper tract deterioration, (2) anastomotic obstruction, or (3) patients with chronic renal failure or iodide sensitivity in whom the intravenous administration of contrast material may be contraindicated.

Myelolipoma of the adrenal gland is a rare, benign tumor of which only 17 clinical cases have been reported previously in the literature. We document 1 additional case and review the pathogenesis of the tumor, with emphasis on the theory of the tissue necrosis as the major stimulus of the myelolipomatous changes in the adrenals.

The second reported case of bilateral renal cell carcinoma associated with von Hippel-Lindau disease and treated successfully with staged bilateral nephrectomy and hemodialysis is presented, accompanied by a brief review of the literature.
