RESUMEN

STATEMENT OF PROBLEM: Hard denture acrylic resins must maintain color stability to remain esthetic. PURPOSE: The purpose of this in vitro study was to measure the color stability of denture resins subjected to staining agents and cleaned with denture cleansers MATERIAL AND METHODS: Forty-five disks (10-mm diameter, 2-mm thick) fabricated from 3 denture resins (light polymerized Eclipse; heat polymerized Lucitone 199 Heat; autopolymerized Lucitone 199 Repair) were stored for 24 hours in 37°C water. After baseline colorimetry measurement were made, the specimens were soaked for 8 hours in a staining agent: coffee, cola, or grape juice. Subsequently, the specimens were soaked for 12 hours in a commercial denture cleanser (Polident, GlaxoSmithKline; Efferdent, Prestige Brands Inc) or water (control). This procedure was repeated every 24 hours. Staining agents and cleansers were replenished every 24 hours. Color change (ΔE) was determined after 2, 4, 6, and 8 weeks (n=5). Results were submitted to analysis of variance and Tukey honest significant difference test (α=.05). RESULTS: Significant interaction was found between cleansers and staining agents (P=.006), cleansers and resins (P=.034), and resins and staining agents (P<.001). For the staining agent of grape juice, water allowed more discoloration (ΔE=16.93) than Efferdent (ΔE=14.41) or Polident (ΔE=14.81); Efferdent and Polident effects were not significantly different (P=.761). The Lucitone 199 Repair resin cleaned with water discolored more (ΔE=15.24) than with Efferdent (ΔE=14.14) or Polident (mean ΔE=13.82); the difference between Polident and Efferdent was not significant (P=.566). For Eclipse, grape juice discolored more (ΔE=16.43) than coffee (ΔE=12.23; P<.001) or cola (ΔE=13.78; P=.001); coffee and cola staining were not significantly different (P=.071). CONCLUSIONS: Eclipse resin stained most, especially in grape juice. Compared with water, Polident and Efferdent reduced discoloration better in Lucitone 199 Repair and grape juice stains. Nevertheless, discolorations in all 3 resins were perceivable, regardless of cleanser.

OBJECTIVE: To evaluate the effectiveness and tolerability of an experimental 9.5% hydrogen peroxide whitening strip relative to a placebo control over a three-week period. METHODS: In this parallel-design, double-blind clinical trial, 54 adult volunteers were randomized to an experimental 9.5% hydrogen peroxide whitening strip or placebo strip balancing for age and baseline tooth color, and received treatment. Strips were worn on the maxillary arch 30 minutes daily for 20 days. Efficacy was measured objectively as L*a*b* color change from digital images at Days 4, 7, 15, and 21. RESULTS: As early as Day 4 and at all subsequent visits, the 9.5% strip group experienced significant (p < 0.004) color improvement relative to placebo for b* and L* color parameters. The amount of color improvement increased with continuing peroxide strip use. Mean ± SE between-group differences in Ab* were -0.6 ± 0.16, -0.8 ± 0.15, -1.6 ± 0.19, and -1.9 ± 0.20 at Days 4, 7, 15, and 21, respectively. Similar results were noted for AL*. Minor tooth sensitivity was the most common adverse event, as reported by 12% of subjects in the 9.5% strip group and 11% of subjects in the placebo group. No subjects discontinued treatment due to an adverse event. CONCLUSION: This placebo-controlled clinical trial demonstrated that an experimental 9.5% hydrogen peroxide strip yielded significant tooth whitening relative to a placebo strip as early as after three days of product use.

BACKGROUND: The introduction of small-diameter implants has provided dentists the means of providing edentulous and partially edentulous patients with immediate functioning transitional prostheses while definitive restorations are being fabricated. The successful use of these small-diameter implants for temporary stabilization of prostheses has led many clinicians to explore the option of using them as a definitive alternative, especially as the technique requires minimal time and also is economical for the patients. To date, there has been no study with multiple patients looking at both the subjective and objective outcomes of these small-diameter implants. MATERIALS AND METHODS: Twenty-seven edentulous patients were enrolled in this study, seven of them were smokers. One-hundred and eight small-diameter (2.0 mm, MDL) implants were surgically placed in 24 edentulous mandibles. All implants were immediately loaded. The patients filled out a screening questionnaire and four subsequent questionnaires to test their satisfaction with the altered prosthesis at 6, 12, 18 and 24 months. The survival of the implants was also noted. RESULTS: Smokers had an implant survival of 79%. Non-smokers had an implant survival of 100%. The results of the questionnaire indicated an overall satisfaction with the implant-supported prosthesis.
