Objective The COVID-19 pandemic has imposed a great burden on healthcare workers worldwide. The aim of the present study was to assess sleep quality, insomnia, and perceived stress in healthcare workers of a high complexity hospital located in Bogota, Colombia. Methods Cross-sectional study in which 1,155 healthcare workers at the Hospital Universitario San Ignacio in Bogotá, Colombia were included, between September and October 2020. Using an online-based survey, self-reported variables were assessed including demographics, Pittsburgh Sleep Quality Index (PSQI), Insomnia Severity Index (ISI), and 10 item Perceived Stress Scale (PSS-10). Associations between these variables were evaluated. Results Fifty percent of the respondents were between 31 and 45 years old, and 76 percent were women. Most of the surveyed were the nursing staff. Poor sleep quality, insomnia, and high perceived stress was found in 74.9, 12.4, and 13.2%, respectively. Poor sleep quality was predominantly found in females, in the 31 to 45 years old group and in married personnel. Also, poor sleep quality was found in relation to a moderate to high perceived risk of COVID-19 infection by the family of the workers surveyed. Discussion Poor sleep quality, moderate rates of insomnia, and perceived stress were found among healthcare workers committed to COVID-19 infected patients in Colombia. The identification of workers at greater risk and the implementation of targeted interventions are called upon as the results.

Periodontitis has been commonly linked to periodontopathogens categorized in Socransky's microbial complexes; however, there is a lack of knowledge regarding "other microorganisms" or "cryptic microorganisms", which are rarely thought of as significant oral pathogens and have been neither previously categorized nor connected to illnesses in the oral cavity. This study hypothesized that these cryptic microorganisms could contribute to the modulation of oral microbiota present in health or disease (periodontitis and/or obstructive sleep apnea (OSA) patients). For this purpose, the presence and correlation among these cultivable cryptic oral microorganisms were identified, and their possible role in both conditions was determined. Data from oral samples of individuals with or without periodontitis and with or without OSA were obtained from a previous study. Demographic data, clinical oral characteristics, and genera and species of cultivable cryptic oral microorganisms identified by MALDI-TOF were recorded. The data from 75 participants were analyzed to determine the relative frequencies of cultivable cryptic microorganisms' genera and species, and microbial clusters and correlations tests were performed. According to periodontal condition, dental-biofilm-induced gingivitis in reduced periodontium and stage III periodontitis were found to have the highest diversity of cryptic microorganism species. Based on the experimental condition, these findings showed that there are genera related to disease conditions and others related to healthy conditions, with species that could be related to different chronic diseases being highlighted as periodontitis and OSA comorbidities. The cryptic microorganisms within the oral microbiota of patients with periodontitis and OSA are present as potential pathogens, promoting the development of dysbiotic microbiota and the occurrence of chronic diseases, which have been previously proposed to be common risk factors for periodontitis and OSA. Understanding the function of possible pathogens in the oral microbiota will require more research.

Objective: The aim of this study was to analyze the cultivable oral microbiota of patients with obstructive sleep apnea (OSA) and its association with the periodontal condition. Methods: The epidemiology profile of patients and their clinical oral characteristics were determined. The microbiota was collected from saliva, subgingival plaque, and gingival sulcus of 93 patients classified into four groups according to the periodontal and clinical diagnosis: Group 1 (n = 25), healthy patients; Group 2 (n = 17), patients with periodontitis and without OSA; Group 3 (n = 19), patients with OSA and without periodontitis; and Group 4 (n = 32), patients with periodontitis and OSA. Microbiological samples were cultured, classified, characterized macroscopically and microscopically, and identified by MALDI-TOF-MS. The distribution of complexes and categories of microorganisms and correlations were established for inter- and intra-group of patients and statistically evaluated using the Spearman r test (p-value <0.5) and a multidimensional grouping analysis. Result: There was no evidence between the severity of OSA and periodontitis (p = 0.2813). However, there is a relationship between the stage of periodontitis and OSA (p = 0.0157), with stage III periodontitis being the one with the highest presence in patients with severe OSA (prevalence of 75%; p = 0.0157), with more cases in men. The greatest distribution of the complexes and categories was found in oral samples of patients with periodontitis and OSA (Group 4 P-OSA); even Candida spp. were more prevalent in these patients. Periodontitis and OSA are associated with comorbidities and oral conditions, and the microorganisms of the orange and red complexes participate in this association. The formation of the dysbiotic biofilm was mainly related to the presence of these complexes in association with Candida spp. Conclusion: Periodontopathogenic bacteria of the orange complex, such as Prevotella melaninogenica, and the yeast Candida albicans, altered the cultivable oral microbiota of patients with periodontitis and OSA in terms of diversity, possibly increasing the severity of periodontal disease. The link between yeasts and periodontopathogenic bacteria could help explain why people with severe OSA have such a high risk of stage III periodontitis. Antimicrobial approaches for treating periodontitis in individuals with OSA could be investigated in vitro using polymicrobial biofilms, according to our findings.

OBJECTIVE: To describe the sociodemographic and epidemiological characteristics of diagnosis and treatment of pediatric patients with sleep apnea, both central and obstructive, in Colombia between 2017 and 2021. METHODS: Observational, descriptive, cross-sectional, epidemiological study using the International Classification of Diseases and Related Health Problems as search terms for sleep apnea, based on SISPRO, the Colombian national health registry. Stratification by gender and age groups was performed. We also generated data of the amount of diagnostic and therapeutic procedures performed. A map of prevalence by place of residency was performed. RESULTS: National records report 15200 cases of SA between 2017 and 2021, for an estimated prevalence of 21.1 cases by 100000 inhabitants in 2019 the year with the most cases (4769), being more frequent and in the 6 to 11 age group and in males, with a male to female ratio of 1.54:1. The number of cases declined in 2020 and 2021. The map showed a concentration of cases in the more developed departments of the country. DISCUSSION: This is the first approximation to a nation-wide prevalence of sleep apnea in Colombia which is lower to what is found in the literature worldwide, including studies performed in Latin America and in Colombia, this could reflect sub diagnosis and sub report. The fact that the highest prevalence was found in males and in the 6-11 age group is consistent with reports in literature. The decrease in cases in 2020 and 2021 could be related to the COVID-19 pandemic impact in sleep medicine services.

OBJECTIVE: To determine the frequency of sleep problems in low-income, urban pediatric populations in cities at different altitudes in Colombia. METHODS: A descriptive, cross-sectional population-based observational study was conducted in children aged between 2 and 12 years in the low income, urban areas of three cities in Colombia (Santa Marta, Bucaramanga, and Bogotá) located at 15, 959, and 2640 m above sea level, respectively. Sociodemographic data were collected, and the Spanish version of the Pediatric Sleep Questionnaire was used. RESULTS: 1989 children were surveyed, distributed as follows: Santa Marta (32.0%), Bucaramanga (33.4%), and Bogotá (34.6%). The overall prevalence of sleep problems was 39.0%. Children from Santa Marta had the highest frequency of parasomnias (58.0%); those from Bucaramanga had the highest frequency of attention deficit symptoms (4.0%) and apneic pauses witnessed by parents or caregivers (5.7%). Finally, Bogotá, the only high-altitude location, had the highest frequency of sleep disordered breathing (17.2%). CONCLUSIONS: The study found a high frequency of sleep problems in the pediatric population, especially at higher altitudes when compared to lower altitude settings. Sleep disorders warrant early detection and timely therapeutic intervention.

AIM: The aim of this systematic review and meta-analysis was to evaluate the association between sense of coherence (SOC) and dental fear/dental anxiety. METHODS: Computerized searches were performed in six databases. Gray literature and manual searches were also conducted. Study selection, risk of bias assessment, and meta-analyses were performed. The results of meta-analyses were reported in odds ratio (OR), mean difference (MD), and confidence interval (CI). RESULTS: After removing duplicates, 131 references were retrieved and the full texts of 12 were evaluated. Nine references met the eligibility criteria and were included. The results of meta-analyses demonstrated that individuals with a weaker SOC had a score in the dental anxiety/dental fear scale 6.04 times higher than individuals with a stronger SOC (OR = 6.04, CI = 2.62-13.91). Individuals with no dental anxiety/dental fear had a mean score of the SOC scale higher than individuals with dental anxiety/dental fear (MD = 5.39, CI = 2.21-8.56). Sensitivity analysis demonstrated that the result of a higher mean score of the SOC scale among individuals with no dental anxiety/dental fear remained (MD = 7.38, CI = 3.79-10.98). Risk of bias ranged from low to high. The certainty of evidence was very low. CONCLUSION: Individuals with stronger SOC have lower levels of dental fear/dental anxiety.

OBJECTIVES: Polysomnogram is the gold standard for the diagnosis of sleep-disordered breathing (SDB); a sensitive and specific alternative strategy would be ideal, due to its low availability, and screening patients at high risk of OSA is very important. This study aimed to determine the operating characteristics of screening tests in patients with and without cardiovascular disease (CVD). MATERIAL AND METHODS: Epworth sleepiness scale (ESS), Berlin, STOP-bang and Pittsburgh sleep quality index (PSQI) were applied in adults with and without cardiovascular disease in three Colombian cities, as well as anthropometric measurements and a polysomnogram. Operating characteristics were calculated for each test and the best cut-off values in patients with and without CVD were obtained. RESULTS: 964 patients (median age: 58), 662 with and 302 without CVD were included. The prevalence for SDB (AHI =5) were 43.4 % (OSA), 16.2% (central apnea), and 12.4 % (other). In patients without CVD, the highest sensitivity for OSA and central apnea was for PSQI (80-85%). The highest specificity was for STOP-bang (68%) and Berlin (78.6%). In CVD the best sensitivity was for PSQI (81.9%) followed by Berlin (71.9%) and the best specificity for STOP-bang (82.1%). No isolated questionnaire showed good diagnostic performance (AUC=0.6) and the cut-off values had no variations except for ESS. CONCLUSION: Screening tests showed low operating characteristics for the diagnosis to SDB, but better performance in patients with CVD. They are not recommended as the only diagnostic test, but they can be useful to guide the initial diagnostic process.

Resumen El Síndrome de apnea - hipopnea obstructiva del sueño es una enfermedad con compromiso multisistémico, con especial repercusión en el sistema cardiovascular y por tanto con alto impacto en la morbimortalidad general. Se han demostrado múltiples mecanismos fisiopatológicos que explican y sugieren una asociación directa y proporcional al daño generado por esta entidad, en especial para las taquiarritmias, específicamente para fibrilación auricular y también para las bradiarrimias, con mejoría demostrada tras el tratamiento con presión positiva continua de la vía aérea. La asociación con aleteo auricular y arritmias ventriculares ha mostrado resultados contradictorios en algunos estudios, por lo que aún no es tan clara.


Abstract Obstructive sleep apnoea / hypopnoea syndrome is a disease of multisystemic involvement, with particular repercussions on the cardiovascular system, and thus a high impact on morbidity and mortality. Several pathophysiological mechanisms have been demonstrated that explain and suggest a direct and proportional relationship to the damaged caused by the condition, especially for tachyarrhythmias, specifically for atrial fibrillation, and also for bradyarrhythmias, with a demonstrated improvement with continuous positive airway pressure. As the association with atrial flutter and ventricular arrhythmias has shown contradictory results in some studies, the relationship is still not clear.

BACKGROUND: Several studies have reported an association between periodontal disease and obstructive sleep apnea (OSA). However, heterogeneity of results suggests that there is insufficient evidence to support this association. AIMS: The objective of this study was to identify the association between periodontal disease and OSA in adults with different comorbidities. SETTINGS AND DESIGN: One hundred and ninety-nine individuals (107 women and 92 men) underwent polysomnography with a mean age of 49.9 years were recruited. MATERIALS AND METHODS: The presence of OSA, comorbidities, and periodontal disease was evaluated in each individual. Student's t-tests or Chi-square and ANOVA tests were used to determine the differences between groups. RESULTS: The prevalence of periodontal disease was 62.3% and 34.1% for gingivitis. The results showed no statistically significant association between all groups of patients with OSA and non-OSA patients for gingivitis (P = 0.27) and for periodontitis (P = 0.312). However, statistically significant association was shown between periodontitis and mild OSA compared with the periodontitis and non-OSA referent (P = 0.041; odds ratio: 1.37 and 95% confidence interval 1.11-2.68). The analysis between OSA and comorbidities showed a statistically significant difference for patients with OSA and hypertension (P < 0.001) and for patients with OSA and hypertensive cardiomyopathy (P < 0.001) compared with healthy individuals. Periodontitis was more likely in men with severe OSA and with any of two comorbidities such as hypertension or hypertensive cardiomyopathy. Women with hypertension or hypertensive cardiomyopathy were more likely to have mild OSA, and these associations were statistically significant (P < 0.05). CONCLUSIONS: This study identified association between periodontitis and mild OSA and this association was more frequent in women with hypertension or hypertensive cardiomyopathy. Periodontitis was associated with severe OSA in men who showed any of two comorbidities such as hypertension or hypertensive cardiomyopathy.

Sleep studies conducted at an altitude that is different from the home altitude can yield misleading results regarding the severity of obstructive sleep apnea (OSA). The objective of the present study was to determine the sleep characteristics of a patient undergoing polysomnography (PSG) in three Colombian cities at different altitudes (Bogotá, at 2,640 m above sea level [ASL]; Bucaramanga, at 959 m ASL; and Santa Marta, at 15 m ASL). The patient was an obese man with diabetes and suspected OSA. All PSG recordings were scored and interpreted in accordance with American Academy of Sleep Medicine criteria. In Bogotá, PSG revealed moderate OSA (an apnea-hypopnea index [AHI] of 21 events/h); in Bucaramanga, PSG revealed increased upper airway resistance (an AHI of 2 events/h); in Santa Marta, PSG revealed mild OSA (an AHI of 7 events/h). The reduction in the AHI was predominantly a reduction in hypopneas and obstructive apneas. The respiratory events were shorter in duration in the city at an intermediate altitude. Given that the AHI varied widely across cities, we can assume that the patient is normal or has moderate OSA depending on the city where he is. Central apneas were found to have no influence on the AHI.

RESUMO Estudos do sono realizados a uma altitude diferente da altitude do local onde se vive podem produzir resultados enganosos quanto à gravidade da apneia obstrutiva do sono (AOS). O objetivo do presente estudo foi determinar as características do sono de um paciente submetido a polissonografia (PSG) em três cidades colombianas a diferentes altitudes [Bogotá, a 2.640 m acima do nível do mar (ANM); Bucaramanga, a 959 m ANM e Santa Marta, a 15 m ANM]. O paciente era um homem obeso com diabetes e suspeita de AOS. Os dados obtidos receberam pontuação e foram interpretados de acordo com os critérios da American Academy of Sleep Medicine. Em Bogotá, a PSG revelou AOS moderada [índice de apneias e hipopneias (IAH) = 21 eventos/h]; em Bucaramanga, a PSG revelou aumento da resistência das vias aéreas superiores (IAH = 2 eventos/h); em Santa Marta, a PSG revelou AOS leve (IAH = 7 eventos/h). A redução do IAH foi predominantemente uma redução de hipopneias e apneias obstrutivas. Os eventos respiratórios tiveram menor duração na cidade a uma altitude intermediária. Como houve grande diferença entre as cidades quanto ao IAH, pode-se concluir que o paciente é normal ou apresenta AOS moderada dependendo da cidade onde está. As apneias centrais não influenciaram o IAH.

OBJECTIVE: Poor sleep patterns are common in undergraduates and may turn them prone to mood disorders, substance abuse and impaired academic performance. The aim of this study was to assess sleep disturbances among medical students, and whether associations with academic performance, depressive symptoms or substance use were present. METHODS: Cross-sectional study in which 544 medical students of the Pontificia Universidad Javeriana in Bogota, Colombia were included. Using a computer-based survey, self-reported variables were assessed, including demographics, Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), Berlin Questionnaire, Diagnostic and Statistical Manual of Mental Disorders (DSM) - IV depression criteria, Grade Point Average (GPA), and substance use. Associations between these variables were obtained. RESULTS: Fifty-four percent of students were women. Poor sleep quality (PSQI>5) and daytime sleepiness (ESS>10) were found in 65% of the population. A higher GPA was more frequent in students with good sleep quality (OR= 2.6 [1.5-4.5]), lack of daytime sleepiness (OR= 2 [1.3-3.1]) and low risk of Obstructive Sleep Apnea Syndrome (OSAS) (OR= 3.1 [1.6-5.9]). DSM-IV depression criteria were fulfilled by 26% of the students and were associated with poor sleep patterns. Energy drinks use was associated with poor sleep quality. DISCUSSION: Poor sleep quality, daytime sleepiness and depressive symptoms are frequent among medical students and are associated with lower academic performance. The identification of students at risk and the implementation of targeted interventions are warranted. Fostering adequate sleep habits and training on sleep medicine may partly counteract these issues.

Antecedentes: La apnea obstructiva del sueño (AOS) es un trastorno del sueño altamente prevalente producido por una obstrucción anatómica o neuromuscular de la vía aérea superior. Propósito: Identificar la asociación entre los índices antropométricos: circunferencia de cuello (CC), perímetro abdominal e índice de masa corporal (IMC), con la presencia de AOS en adultos. Métodos: Se realizó un estudio observacional retrospectivo de casos y controles en 353 individuos entre 18 y 82 años de edad con diagnóstico polisomnográfico de AOS. Se correlacionó el índice apnea hipoapnea con los índices antropométricos y los datos demográficos. El grupo control estuvo conformado por 105 adultos sin AOS y el de estudio por 248 pacientes con diagnóstico de AOS. Para determinar la asociación entre las variables se utilizaron las pruebas de Chi cuadrado de Pearson y odds ratio. Resultados Se encontró una asociación estadísticamente significativa entre el IMC y la CC, que estaban aumentados con la presencia de AOS en adultos. La presencia de AOS fue mayor en mujeres y hombres entre 56 y 82 años. Conclusiones: Las medidas antropométricas IMC y CC son factores de riesgo de AOS en adultos.


Background: Obstructive sleep apnea (OSA) is a highly prevalent sleep disorder caused by anatomic or neuromuscular obstructions. Purpose: To identify the association between anthropometric indexes: neck circumference (NC), abdominal perimeter, and Body Mass Index (BMI), and the presence of OSA in adults. Methods: A retrospective observational case-control study was carried out in 353 individuals between the ages of 18 and 82 years with a polysomnographic diagnosis of OSA. The Apnea-Hypopnea Index was correlated with the anthropometric indexes and demographic data. The control group consisted of 105 adults without OSA and the case group were 248 patients diagnosed with OSA. Pearson Chi-square and Odds Ratio (OR) tests were used for statistical analysis. Results: Statistically significant associations between increased BMI and NC with the presence of OSA in adults were found. The presence of OSA was greater in 56-to-82-year-old women and men. Conclusions: Anthropometric measures BMI and NC are OSA risk factors in adults.

Resumen El síndrome de apnea-hipopnea obstructiva del sueño (SAHOS) es un trastorno frecuente asociado con secuelas cardiovasculares y neuropsicológicas severas que repercuten en altos costos socioeconómicos para la población. Con anterioridad se estimaba una prevalencia aproximada del 3% al 7% en hombres y del 2% al 5% en mujeres (40-60 años para ambos sexos); sin embargo, en las últimas dos décadas aumentó a 10% en hombres de 30 a 49 años, a 17% en hombres de 50 a 70, a 3% en mujeres de 30 a 49 y a 9% en mujeres de 50 a 70. Se calcula que el 20% de adultos de edad media tiene al menos SAHOS leve y el 80% de los casos permanecen sin diagnosticar, de allí la importancia de sospechar el diagnóstico de la enfermedad. En Colombia se realizó un estudio con la metodología ómnibus que mostró que la prevalencia global de alto riesgo de apnea del sueño, según el Cuestionario Berlín, fue del 19% en tres ciudades -Bogotá D.C., Bucaramanga y Santa Marta- (IC95%: 17.3; 20.8%). Con la escala STOP-Bang, la prevalencia global de alto riesgo de SAHOS fue de 26.9% (IC95%: 24.9; 29%).


Abstract Obstructive sleep apnea-hypopnea syndrome (OSAHS) is a frequent disorder associated with severe cardiovascular and neuropsychological sequelae, which has a high socioeconomic cost to the population. In the past, an average prevalence of 3% to 7% in men and 2% to 5% in women (40-60 years for both sexes) was estimated. However, in the last two decades, it increased to 10% in men aged 30 to 49 years, 17% in men aged 50 to 70, and 3% in women aged 30 to 49 and 9% in women aged 50 to 70. It is estimated that 20% of middle-aged adults have at least mild OSAHS, while 80% of cases remain undiagnosed, hence the importance of suspecting this disease. In Colombia, an omnibus survey was conducted showing that the global prevalence of high-risk sleep apnea, according to the Berlin questionnaire, was 19% in three cities -Bogotá D.C., Bucaramanga and Santa Marta- (CI95%: 17.3, 20.8%). The overall prevalence of high risk of OSAHS was 26.9% (95% CI: 24.9%, 29%) according to the STOP-Bang scale.

Resumen Las manifestaciones oftalmológicas que se relacionan con el síndrome de apnea-hipopnea obstructiva del sueño (SAHOS) incluyen síndrome de párpado flácido y cambios a nivel del nervio óptico asociados con glaucoma, así como neuropatía óptica isquémica anterior no arterítica y papiledema. La prevalencia del síndrome de párpado flácido en pacientes con SAHOS varía entre 2.3% y 32.6%, mientras que de la asociación entre glaucoma y SAHOS oscila entre 2% y 27%. En la población estudiada en Colombia se encuentra una frecuencia de 2.7% de asociación entre glaucoma de presión normal y SAHOS. El glaucoma presente en estos casos es el primario de ángulo abierto, que bien puede cursar con o sin un aumento de la presión intraocular. En cuanto a la neuropatía óptica isquémica y el papiledema, no se dispone de datos acerca de su prevalencia en pacientes con SAHOS. Se recomienda la valoración por oftalmología a los pacientes diagnosticados con este síndrome.


Abstract Ophthalmologic manifestations associated with obstructive sleep apnea-hypopnea syndrome (OSAHS) include floppy eyelid syndrome and changes in the optic nerve related to glaucoma, as well as nonarteritic anterior ischemic optic neuropathy and papilledema. The prevalence of floppy eyelid syndrome in patients with OSAHS ranges between 2.3% and 32.6%, while the association between glaucoma and OSAHS ranges from 2% to 27%. In the population studied in Colombia, an association frequency of 2.7% between normal pressure glaucoma and OSAHS has been found. The type of glaucoma observed in these cases is open-angle primary glaucoma, which may well occur with or without an increase of intraocular pressure. Regarding ischemic optic neuropathy and papilledema, data on their prevalence in patients with OSAHS are not available. An evaluation by ophthalmology is recommended to the patients diagnosed with this syndrome.

Resumen Los pacientes con diagnóstico de síndrome de apnea-hipopnea obstructiva del sueño (SAHOS) pueden ser llevados de forma segura a cualquier procedimiento quirúrgico, incluso de manera ambulatoria si tienen control adecuado de sus otras comorbilidades. El tratamiento con presión positiva continúa en vía aérea (CPAP) en pacientes con diagnóstico confirmado disminuye el riesgo de presentar complicaciones cardiovasculares a largo plazo. La sedación debe ser hecha por un anestesiólogo, quien, además, debe vigilar al paciente y disponer del equipo adecuado para atender complicaciones respiratorias emergentes. Se sugiere que, en estos pacientes, los procedimientos sean hechos en el ámbito hospitalario, donde se tenga disponibilidad en la unidad de cuidados post-anestésicos y personal para monitoreo por al menos una hora tras finalizar el procedimiento. El tratamiento con CPAP debe continuar según sea ordenado por el médico tratante. Es importante que desde la valoración preanestésica se identifique a los pacientes con riesgo de SAHOS para lograr trazar un plan anestésico que disminuya las complicaciones a nivel respiratorio y del manejo de la vía área.


Abstract Patients diagnosed with obstructive sleep apnea-hypopnea syndrome (OSAHS) can safely undergo a surgical procedure, even on an outpatient basis, if other comorbidities are adequately controlled. Continuous positive airway pressure (CPAP) treatment in patients with a confirmed diagnosis decreases the risk of long-term cardiovascular complications. Sedation should be done by an anesthesiologist, who must also monitor the patient and have the appropriate equipment to deal with emerging respiratory complications. Procedures undergone by these patients should be performed in a hospital setting, in which a post-anesthetic care unit and staff are available for follow-up, at least for an hour after the procedure ends. CPAP treatment should be continued as ordered by the treating physician. Identifying patients with OSAHS risk during the preanesthetic evaluation is important to propose an anesthetic plan that reduces respiratory complications and improves airway management.

Resumen Para el tratamiento de cualquier enfermedad es necesario un adecuado manejo multidisciplinar e involucrar al paciente en la mejor opción que se requiera a largo plazo; la falta de tratamiento de los pacientes implica una enorme carga para el sistema sanitario y la sociedad. En la actualidad, existen diferentes tratamientos para los pacientes con síndrome de apnea-hipopnea obstructiva del sueño (SAHOS); el de mayor elección, después de descartar anomalías anatómicas susceptibles de cirugía, es el PAP (presión positiva en vía aérea). Las indicaciones para terapia con PAP incluyen pacientes con índice de apnea-hipopnea (IAH) ≥15 eventos/hora o ≥5 y ≤14 eventos/hora y quejas de somnolencia diurna excesiva, deterioro cognitivo, trastorno del afecto o insomnio, hipertensión arterial documentada, enfermedad arterial coronaria o historia de evento cerebrovascular. Es importante recordar que el IAH debe basarse en sueño ≥2 horas de registro polisomnográfico. Hay diferentes modalidades de tratamiento con PAP y se debe intervenir para buscar mejorar la adherencia al dispositivo, principal limitante para alcanzar la eficacia en el tratamiento. El impacto del tratamiento del SAHOS ha sido investigado, pero la mayoría de los estudios reportados son de carácter observacional.


Abstract In order to treat any disease, an adequate multidisciplinary management and involving the patient in the long term are necessary, since not treating patients implies an enormous burden for the health system and the society. Currently, different treatments for patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) can be found, being PAP (positive airway pressure) the most frequently chosen after ruling out anatomical abnormalities susceptible to surgery. Indications for PAP therapy include patients with apnea-hypopnea index (AHI) ≥15 events/hour or ≥5 and ≤14 events/hour and complaints of excessive daytime sleepiness, cognitive impairment and affective disorder or insomnia, documented arterial hypertension, coronary artery disease, or cerebrovascular event history. It is important to remember that AHI should be based on polysomnographic recording of sleep ≥2 hours. There are different modalities of treatment with PAP and intervention should be sought to improve adherence to the device, which is the main limiting factor for achieving efficacy in the treatment. The impact of OSAHS treatment has been investigated, but most of the reported studies are observational.

Introducción: la apnea obstructiva del sueño (AOS) se considera un problema de salud pública que afecta en gran medida a hombres y mujeres entre 50 y 70 años de edad, donde en ocasiones son, además, pacientes edéntulos, lo cual dificulta su tratamiento. Objetivo: identificar los aparatos intraorales existentes para tratar la AOS leve y moderada en pacientes edéntulos. Metodología: se buscó literatura sobre el tema en bases de datos y de patentes. Resultados: se encontraron cinco artículos y dos patentes. El objetivo terapéutico de todos los dispositivos es aumentar la vía aérea superior entre la bsise de la lengua y la pared posterior de la faringe, mediante el avance mandibular. Dentro de las principales desventajas que presentan estos dispositivos están falta de retención, dificultad para mantener la dimensión vertical, problemas musculares y trastornos en la articulación temporomandibular. Conclusión: existen pocos dispositivos intraorales efectivos en la literatura para el tratamiento de la AOS en pacientes edéntulos. Esto hace necesario diseñar futuras investigaciones en esta área.


Introduction: The Obstructive Sleep Apnea (OSA) is considerad a problem of World Public Health that greatly affects men and women between ¿0 and 70 years oíd, where some of them are edentulous patients which treatment difficulty. Objective: lo identiíy the existing devices for mild and modérate OSA treatment ¿n edentulous patients. Method: A comprehensiva review was conducted in the literatura and patent databases. Results: 5 articles and 2 patents were round. The main therapy objective ¿n all devices is to increase the upper airway space between the tongue base and the posterior pharyngeal wall trough mandibular advancement. Several problems with use of these devices inelude lack retention, difficult for maintain the vertical dimensión, muscular discomfort and Temporomandibular Disorders. Conclusión: There are limited effective ¿ntraoral devices in literatura for OSA treatment in edentulous patients. It is necessary to design futura investigations in this area.

BACKGROUND: Sleep disorders are common but underdiagnosed conditions, which are associated with obesity. In Colombia, the distribution of sleep disorders remains unclear. We aimed to describe the distribution of sleep disorders, according to demographic, geographic and anthropometric characteristics, in adult Colombian populations. METHODS: A multicenter study was conducted with 5474 participants recruited from three Colombian cities at different altitudes. A two-stage cluster sampling method was applied. Participants' mean age was 40.2 years and 53.8% were female. Collected data included demographic information and anthropometric characteristics of adiposity such as body mass index, neck circumference and waist circumference, as well as participants' scores on five scales used to assess sleep disorders. Disorders included sleepiness, obstructive sleep apnea (OSA), insomnia, poor sleep quality and restless legs syndrome; the scales were the Epworth Sleepiness Scale, Berlin questionnaire, STOP-Bang questionnaire, Pittsburgh Sleep Quality Index and diagnostic criteria for the restless legs syndrome set out by the International Restless Legs Syndrome Study Group. RESULTS: Nearly two-thirds of the population reported at least one sleep disorder according to their results on the five scales (59.6% [95%CI 57.4; 61.81)]. This proportion was similar by sex. Prevalence of overweight was 34.8% and of obesity was 14.4%. Sleep disorders were more frequent among those aged 65 years or more (91.11 [95%CI 86.1; 94.43]), those who were obese (83.71% [95%CI 78.94; 87.56]) and those who resided in the cities at the lowest altitude (72.4% [95%CI 70.2; 74.5]). Waist circumference showed a stronger association with sleep disorders among women than among men. CONCLUSIONS: Sleep disorders are common in Colombia, irrespective of sex and geographical location. They are associated with obesity. Abdominal obesity could explain the high frequency of sleep disorders among women. We believe that this part of the study will substantially contribute to the understanding of sleep disorders. Further research is needed to identify key factors behind the high prevalence rates of sleep disorders and obesity in Colombia.

Objetivos: Establecer la prevalencia de positividad, la tasa de conversión de la tuberculina en trabajadores de nuestro hospital y describir las características demográficas y laborales asociadas a esto. Materiales y Métodos Estudio observacional descriptivo en una cohorte de trabajadores del hospital a los que se les realizó la prueba de tuberculina. Se definió la positividad de la prueba de tuberculina como un resultado mayor o igual a 10 mm y la conversión como un aumento de 6 o 10 mm con respecto a la prueba inicial. Resultados Se encontró una prevalencia de 23,7 % y una incidencia de conversión de 13,6 % para el punto de corte mayor de 10 mm y 23 % para el punto de corte de 6 mm. La edad de los sujetos estuvo relacionada a conversión, mientras que no se encontró relación con la ocupación. Conclusión La probabilidad de transmisión de la tuberculosis en trabajadores del hospital es mayor al de la población general. Deben ser implementadas medidas de promoción y prevención para disminuir la transmisión e incrementar el conocimiento de la tuberculosis asociada al cuidado de la salud en los trabajadores.(AU)


Objectives To establish the prevalence of positivity and conversion rate of the tuberculin skin test in workers of our hospital, and to describe the related demographic and occupational characteristics. Materials and Methods An observational, descriptive study was conducted in a cohort of hospital workers who underwent the tuberculin skin test. The positivity of the test was defined as a result of greater than or equal to 10 mm, and conversion was defined as an increase of 6 or 10 mm with respect to the initial test. Results Prevalence of 23.7 % and incidence of conversion of 13.6 % for the major cut-off point of 10 mm, and 23 % for the lesser cut-off point of 6 mm, was found. The age of the subjects was related to the conversion, but there was no relation with occupation. Conclusions The probability of tuberculosis transmission in health-care workers is higher than in the general population. Promotion and prevention measures must be implemented to decrease the transmission and to increase awareness of tuberculosis related to occupational activities.(AU)
