RESUMEN

Background: Effective glycemic control is crucial for hospitalized patients, leading to benefits such as shorter hospital stays and reduced postoperative infection rates. While previous studies have emphasized the effectiveness of multidisciplinary collaborative stewardship for hospital-wide hyperglycemia management, patient perspectives and preferences have not been adequately considered. Objective: To identify factors influencing treatment preferences of Chinese hospitalized diabetes patients using discrete choice experiments (DCEs) and provide practical insights for the construction of a hospital-wide glycemic control programme. Methods: A face-to-face survey was conducted among diabetes patients admitted to nonendocrine departments in a tertiary hospital in Nanjing, China. The attributes and levels were determined based on DCE principles, and a conditional logit model was used to quantify patients' preferences. Results: A total of 157 respondents were analyzed. Antihyperglycemic effectiveness, healthcare providers, treatment regimen, monitoring frequency, and adverse reactions were the five attributes that significantly influenced patient preference (p < 0.05). Notably, an 80% glycemic control rate (ß = 2.009) and a multidisciplinary management team involving clinical pharmacists (ß = 1.346) had the greatest impact. Negative effects were observed for hypoglycemia (ß = -1.008), insulin pump use (ß = -0.746), and frequent glucose monitoring (ß = -0.523). Female patients exhibited higher concern for healthcare providers (ß = 1.172) compared to males. Younger and shorter-course patients prioritized antihyperglycemic effectiveness (ß = 3.330, ß = 1.510), while older patients preferred multidisciplinary management (ß = 1.186) and opposed increased monitoring frequency (ß = -0.703). Patients with higher educational backgrounds showed greater acceptance of continuous glucose monitoring (ß = 1.983), and those with higher annual income placed more emphasis on glycemic control rate. Conclusion: Treatment preferences of hospitalized diabetes patients are mainly influenced by antihyperglycemic effectiveness, adverse reactions, healthcare providers, and individual characteristics. Comprehensive consideration and an individualized therapy strategy should be given when constructing a hospital-wide glycemic control programme.

RESUMEN

Promoting the in situ reconstruction of transition metal (TM)-based precatalysts into low-crystalline and well-modified TM (oxy)hydroxides (TMOxHy) during the alkaline oxygen evolution reaction (OER) is crucial for enhancing their catalytic performances. In this study, we incorporated gadolinium (Gd) into a cobalt hydroxide precatalyst, achieving a deep reconstruction into cobalt oxyhydroxide (γ-CoOOH) while retaining the incorporated Gd during the activation process of the alkaline OER. The unconventional non-leaching Gd dopants endow γ-CoOOH with reduced crystallinity, increasing the exposure of electrolyte-accessible Co atoms and enhancing its bulk activity. Furthermore, the modulation of the electronic structure of γ-CoOOH substantially boosts the intrinsic activity of the active Co sites. As a result, when supported on nickel foam, the catalyst exhibits remarkable alkaline OER performance, achieving a current density of 100 mA cm-2 at a low overpotential of approximately 327 mV. Notably, an ultrahigh current density of 1000 mA cm-2 is robustly maintained for 5 days, highlighting its immense potential for practical applications in large-scale hydrogen production.

RESUMEN

BACKGROUND: During the ongoing global pandemic of COVID-19, the association between hyperglycemia and COVID-19 infection has emerged as a notable concern. Therefore, finding effective methods to manage hyperglycemia in patients with COVID-19 is crucial. OBJECTIVE: To introduce the clinical pharmacists participating in multidisciplinary collaborative whole hospital blood glucose management mode, and to explore its effect on blood glucose control in patients with coronavirus disease 2019 infection and complicated with hyperglycemia. METHODS: Patients with COVID-19 treated at Nanjing Drum Tower Hospital from December 2022 to January 2023 were assigned to routine diagnosis and treatment group and whole hospital blood glucose management group according to the blood glucose management plan received by patients. The groups were compared in regards to their adherence to management advice, blood glucose levels, fluctuation, inflammation-related indicators, medical service-related indicators, and incidence of hypoglycemia and adverse events. RESULTS: After 5 days of glucose management, both groups showed a decrease in fasting and postprandial blood glucose. Postprandial blood glucose in the whole hospital glucose management group was significantly lower than the routine group (P < 0.05). The whole hospital glucose management group showed a significant increase in compliance rate, improved inflammation-related indicators, and higher detection rates for hemoglobin and islet function (P < 0.05). Implementation rates for medical orders and treatment plans were also higher in the whole hospital group (P < 0.05). There was no significant difference in incidence of adverse events. CONCLUSIONS: Multidisciplinary blood glucose management is highly recommended for patients with COVID-19 who have hyperglycemia due to its effectiveness, standardization, safety, and improvement of inflammation indicators.

RESUMEN

Objective: To explore the incidence and influencing factors of postoperative hypoglycemia in diabetic patients during the perioperative period and to construct a risk prediction model for postoperative hypoglycemia. Methods: Patients with T2DM admitted to the nonendocrinology department of Nanjing Drum Tower Hospital from December 2019 to January 2022 were included as research subjects. Basic information, hospital blood glucose management methods, laboratory indicators, and surgery-related indicators were collected. A risk prediction model and scoring table for postoperative hypoglycemia in patients with perioperative diabetes mellitus were established. Results: A total of 440 patients were included, of which 109 had hypoglycemia, resulting in an incidence of postoperative hypoglycemia of 24.78%. The results show that preoperative C-peptide and operation duration were risk factors for postoperative hypoglycemia, while BMI and preoperative fasting blood glucose were protective factors. Conclusion: The model constructed in this study is a good method for evaluating the risk of postoperative hypoglycemia. The scoring table intuitively quantifies the risk of risk factors for outcome variables and has strong clinical practicability.

RESUMEN

The emission of hydrogen sulfide in the waste gas from slaughter plant, fishmeal feed processing and some other food industrial processing could cause serious air pollution to the surrounding environment. The purpose of this study was to screen heterotrophic bacterium strains for the removal of hydrogen sulfide odor. One heterotrophic bacterial mutant ZJNB-B3 was derived from the sulfide degrader Bacillus cereus XJ-2 and its sulfide removal efficiency was 97%. Based on the morphology studies, biochemical tests and 16S rRNA gene analysis, the strain was identified as Bacillus cereus ZJNB-B3. The NCBI GenBank accession number is MF679650. Batch tests showed that the strain tolerated up to 300 mg/L of toxic S²â» concentration. Response surface methodology was applied to optimize the conditions of degradation of sulfide. The optimal parameters were as follows: initial sulfide concentration 211.8 mg/L, initial pH 6.72, inoculum volume 5.04%, and incubation temperature 30 ℃. The accumulated sulfate concentration was 63.8 mg/L and the sulfide removal efficiency was 97.3% after 48 h incubation. No sulfuric acid was generated during sulfide oxidation by the strain. Sulfide could be removed effectively by this strain under mild pH conditions. The results suggested that the strain may have great industrial application potential. This study provides the fundamentals for the removal of hydrogen sulfide gas.

RESUMEN

Hydrogen sulfide is a common odor gas of volatile sulfur-containing compound. The emission of hydrogen sulfide in the waste gas from industrial processing and agricultural operations could cause air pollution to the surrounding environment. The aim of this study was screening and isolation of wild yeast strains from the sludge of sewage pool in the fishmeal processing plant to remove hydrogen sulfide odor. A yeast strain ZJY-7 was obtained. Its hydrogen sulfide removal efficiency was 97.1 %. The morphology studies were investigated using microscope and scanning electron microscope. The yeast isolate was then identified by biochemical tests using API 20 C AUX strip and sequencing 26S rDNA genes. Both biochemical tests analysis and the molecular identification indicated that the yeast isolate ZJY-7 was Candida tropicalis ZJY-7. The NCBI GenBank accession number is KX259479. Batch tests showed that the yeast strain tolerated up to 300 mg/l of dissolved S2- concentration. The yeast also tolerated a wide pH range (2.5-9.0). The optimal initial sulfide concentration of C. tropicalis ZJY-7 on sulfide oxidation and sulfate generation was 200 mg/l, and at initial pH value 6. The highest accumulated sulfate was 91.8 mg/l at 48 h. These results broadened the range of sulfide-oxidizing organism and new application of C. tropicalis on the control of hydrogen sulfide odor pollution. The yeast may have potential to be used in bioreactor for removal of hydrogen sulfide gas.

RESUMEN

Water-soluble carbon nanoparticles (CNPs) were fabricated by a facile, one step hydrothermal synthetic route using acid/alkali as additives. These CNPs emit bright photoluminescence (PL) covering the entire visible-near infrared (NIR) spectral range. PL measurements confirmed that the CNPs have up-conversion of PL properties, and that the NIR PL of the CNPs can also be observed by NIR excitation. Control experiments indicated that different additives can strongly affect the PL properties of the CNPs. With a combination of free dispersion in water and attractive PL properties, these CNPs hold promise for applications in nanotechnology.

RESUMEN

A series of high performance Ag@Ag salts photocatalysts were designed and fabricated. The photocatalytic abilities of this kind of catalysts can be tuned by altering the negative charged ions (Cl(-), Br(-), I(-), CrO(4)(2-), PO(4)(3-), PW(12)O(40)(3-), and SiW(12)O(40)(4-)): higher stability and higher charged anions lead to the stronger photocatalytic ability.
