RESUMEN

OBJECTIVE: Congenital nasolacrimal duct obstruction (CNLDO) is a pediatric disorder with a wide range of pathology. If untreated, the condition may end up with serious complications. Multiple treatment options for CNLDO exist throughout the literature, and there is an ongoing debate on the best intervention for each disease subgroup and the best timing of such interventions. This study compares the success and failure rates of silicone tube intubation (STI) against probing and balloon dilation (BD). METHODS: The authors searched the literature for relevant articles using PubMed, Scopus, web of Science, and Cochrane Library until January 2024. Using RevMan 5.4, the authors compared STI's success and failure rates to probing and BD using risk ratios (RRs) and a random-effect model. In addition, the complication rate of monocanalicular intubation (MCI) versus bicanalicular intubation (BCI) was investigated. The authors used the leave-one-out method to check for influential studies and to resolve heterogeneity. RESULTS: The screening process resulted in 23 eligible articles for inclusion in the authors' review. Silicone tube intubation had a higher chance of resolving the symptoms of CNLDO than probing (RR = 1.11; 95% CI: 1.04, 1.20; P = 0.004) while having less risk of surgical failure (RR = 0.48; 95% CI: 0.30, 0.76; P = 0.002]. Monocanalicular intubation showed no statistically significant difference when compared with BCI in terms of surgical success and failure; however, MCI had a lower risk of complications (RR = 0.68; 95% CI: 0.48, 0.97; P = 0.04). In addition, STI did not demonstrate any significant difference from BD. CONCLUSION: There was no significant difference in success/failure between MCI and BCI; monocanalicular had fewer complications. Silicone tube intubation did better in terms of surgical success than probing, especially in children over 12 months, suggesting that it is the preferred intervention for older patients with CNLDO.

Background Diabetes mellitus is a disease whose dramatic increase in prevalence worldwide poses a global health crisis. Saudi Arabia has the seventh highest rate of diabetes in the world and the second highest rate in the Middle East. The incidence of diabetes has risen due to insufficient knowledge, attitude, and practice surrounding the disease. Saudis' quality of life has decreased due to the recent rise in diabetes-related mortality and illnesses. Hence, leading healthy lives requires patients to have positive attitudes and self-awareness, which will eventually contribute to reducing diabetes mellites-related complications. Methods A cross-sectional study among patients with type 2 diabetes mellites (T2DM) in the Jazan region of Saudi Arabia was conducted to assess Saudi adults' knowledge, attitude, and practice (KAP) regarding the disease. The patients participated in the study by filling in a questionnaire. Data analysis was carried out using R software, version 4.0.5 (R Studio: Integrated Development for R, Boston, MA) and the analysis included general sample demographics. KAP scores were the dependent variables. Following the sample description, regression analysis was performed to examine the adjusted relationships between KAP factors and independent variables. The impact of all independent variables on KAP variables was examined using multiple linear regression analysis. A p-value of less than 0.05 was considered significant, and the beta coefficient was applied to estimate the associations between the independent variables and the KAP variables.  Results A total of 424 participants were involved in this study, with 194 male participants and 230 female participants. The mean duration of having T2DM was 7.63 ± 7.19 years. More than half of the study participants had a degree (58.49%), and nearly half were employed (42.45%). Married participants had higher knowledge and attitude scores than single and divorced or widowed participants (9.19 ± 3.38, 80.14 ± 14.72, respectively). In comparison, divorced or widowed participants had higher practice scores than single and married participants (16.35 ± 7.42). Participants with higher education attainment had higher knowledge and attitude scores than those with no degree (9.54 ± 3.39, 80.58 ± 17.57, respectively), whereas participants with no degree demonstrated higher practice scores (16.85 ± 8.3) than those with degrees. In examining the study participants' KAP scores, we found knowledge to be insufficient in 51.2% and sufficient in 48.8% of the participants. Of the participants, 7.8% were found to have negative attitude levels and 92.2% were found to have positive attitude levels. Finally, study participants' practice levels were found to be negative in 24.8% and positive in 75.2% of participants. Conclusions This study revealed that patients have gaps in their knowledge, attitude, and practice in regard to T2DM. In this study, associations and correlations were established between KAP scores and the sociodemographic characteristics of the patients. The findings of this study could be helpful to policymakers, decision-makers, health care professionals, and patient advocacy groups in developing interventions to improve the health outcomes in T2DM patients.

BACKGROUND: Various graft materials that are classified as autografts, xenografts, and allografts based on their origin have been used to endoscopically repair skull base defects. Tutoplast® (Tutogen Medical GmbH), an allogeneic natural collagen matrix, is processed through chemical sterilization that preserves tissue biocompatibility and structural integrity. OBJECTIVE: To study the safety and efficacy of Tutoplast Fascia Lata and Tutoplast Temporalis Fascia® as primary graft materials in the endoscopic reconstruction of skull base defects of different sizes and etiologies and to compare the outcomes with those of other traditional graft materials based on our experience. METHODS: This is a multi-center retrospective chart review of patients who underwent cerebrospinal fluid leak (CSF) endoscopic reconstruction with Tutoplast Fascia Lata or Tutoplast Temporalis Fascia as either a stand-alone reconstruction material or a part of a multilayer reconstruction depending on the defect at Prince Sultan Military Medical City and King Faisal Specialist Hospital and Research Center in Riyadh, Saudi Arabia, between 2017 and 2020. Our inclusion criteria were CSF leak repair with Tutoplast and a transnasal endoscopic approach. We reviewed demographic data, intraoperative and postoperative complications, repair materials, repair failure, defect size and location. RESULTS: Tutoplast® was used as the primary graft material in 33 cases. Our main outcome was repair success with lack of post operative CSF leak, observed in 30 cases (90.9%). There was no significant association between postoperative CSF leaks and factors including different defect sizes, defect sites, demographic data, hospitalization duration, or postoperative radiation in oncological cases. CONCLUSION: Tutoplast alone or in combination with other materials can be used safely and effectively for skull base defects repair.

PURPOSE: To determine the prevalence of refractive errors among Saudi adults in Riyadh, Saudi Arabia. METHODS: A cross-sectional study was conducted targeting healthy Saudi adults (20-40 years old) at two major gatherings in Riyadh, Saudi Arabia from December 2018 to January 2019. Refractive errors were measured, and data pertaining to age, sex, region of origin, and spectacle use was collected. Clinically significant myopia was defined as SE ≥ -0.50 D, hyperopia as SE ≥ 0.50D, and astigmatism as cylindrical error ≥ 0.50D. Refractive error measurements were assessed using Topcon's Auto-Kerato-Refractometer, KR-800. RESULTS: A total of 660 adult individuals (1,319 eyes) were included in this study, of which 321 participants (48.7%) were found to be myopes, 167 subjects (25.2%) were hyperopes, and 438 individuals (66.3%) had astigmatism. With regards to correlations, myopia was highly correlated with being male (P-value = 0.036), belonging to age group 20-25 years (P-value = 0.033), originating from the northern regions of Saudi Arabia (P-value <0.001). Similarly, hyperopia was significantly correlated with being male (P-value = 0.048), age groups 20-25 years (P-value = 0.04), and 31-35 years (P-value = 0.011) and was higher in people from northern region (P-value = 0.011). In contrast, astigmatism was only found to be correlated with age group 36-40 years (P-value = 0.002). Additionally, 71.7% of myopic participants and 76.1% of astigmatic individuals opted not to wear spectacles (P-value <0.001). CONCLUSION: In this study, the refractive error with the highest prevalence among Saudis was astigmatism, followed by myopia and hyperopia. Gender, region of origin, and spectacle wear were all observed to be highly correlated with higher rates of refractive error.
