RESUMEN

BACKGROUND: To describe the correlation between standardized A-scan echography and histopathology in a ciliary body melanocytoma. CASE PRESENTATION: We present a case of a large ciliary body melanocytoma with significant growth, vision loss, and elevated intraocular pressure that was diagnosed clinically as a melanoma, but the standardized A-scan findings correlated to the histopathological description of a melanocytoma with multiple pseudocysts. CONCLUSIONS: The reflectivity of this melanocytoma by standardized A-scan was consistent with multiple pseudocysts on pathological evaluation. This echographic pattern guided the differential diagnosis. Standardized A-scan is an important diagnostic tool in the differentiation of ciliary body melanocytomas from melanomas.

RESUMEN

PURPOSE: To report a case of post-traumatic endophthalmitis secondary to Bacillus pumilus/safensis. METHODS: Observational case report of a single patient. RESULTS: A 62-year-old man presented with a traumatic corneal laceration due to baling wire while working on a sheep farm. Appropriate corneal laceration repair with injection of intravitreal antibiotics (ceftazidime, clindamycin, and vancomycin) was performed. A single organism, identified as either Bacillus pumilus or Bacillus safensis, was isolated from the vitreous aspirate. A subsequent pars plana vitrectomy, pars plana lensectomy, anterior capsulotomy, and fluid air exchange was required due to severe inflammatory reaction from retained lens material, retinal edema, and vitreous opacities. Vision improved from hand motion to 20/60 at the three-month follow-up visit. CONCLUSION: We describe a case of acute post-traumatic endophthalmitis secondary to Bacillus pumilus/safensis.Précis: The authors describe a case of acute post-traumatic endophthalmitis secondary to Bacillus pumilus/safensis, which was successfully managed with intravitreal antibiotics, pars plana vitrectomy, pars plana lensectomy, anterior capsulotomy, and fluid air exchange.

RESUMEN

BACKGROUND: To describe the superiority of ocular ultrasound in the diagnostic management of extrascleral extension in choroidal melanoma. CASE PRESENTATION: We present a case of a 94-year-old male with choroidal melanoma of the right eye imaged with MRI and ocular ultrasound to aid in the detection of extrascleral extension. CONCLUSIONS: With advancement in technology and new imaging modalities emerging, it can become difficult to determine the best diagnostic approach for patients. We believe that ocular ultrasound remains the superior imaging modality in detection of extrascleral extension in choroidal melanoma.

RESUMEN

A 53-year-old man with a 2-month history of left periorbital swelling was found to have a large solid intraconal mass on CT scan. Orbital ultrasound showed that the lesion had a cavernous pattern of internal reflectivity. Histopathology revealed hyaline-vascular type Castleman disease (CD). This article represents the first reported orbital ultrasound findings in CD. The findings of CT scan and ultrasound may be useful in the preoperative evaluation of orbital hyaline-vascular type CD.
