RESUMEN

The global population is ageing at a time when climate variability is increasing. It is argued that if we are to minimize the negative impact of environmental change on older people, interdisciplinary study of the convergence of the two trends, "climate gerontology", is needed. Climate gerontology can examine the unique challenges and needs of older people in coping with extreme weather events. It can contribute to our understanding of the everyday challenges of growing old in a changing climate and how we can achieve the "longevity dividend". In particular, it can lead the way to the development of effective policies to reduce the carbon footprint of people during their life course, protect older people from climate-related threats and mobilize their wealth of knowledge and experience to address these.

RESUMEN

During a 3-year period 11 neonates underwent general anesthesia for primary repair of tracheo-esophageal fistula (TEF). The age ranged from 1-10 days. Out of these patients, 8 (72.7%) had atresia of the esophagus with a blind upper pouch and lower segment communicating with a trachea. A total of 7 patients (63.6%) had aspiration pneumonitis pre-operatively. Intubation was difficult in 3 (27.3%). There was no intraoperative mortality. However, the incidence of post-operative mortality was 27.3% (3 cases). The cause of death in all these cases was severe non-resolving pneumonia.

RESUMEN

Diagnostic accuracy of 5 tests viz., endoscopy, rapid urease, 24 hours urease, culture and histology, were evaluated in the detection of Helicobacter pylori (H. pylori) infection in 50 patients undergoing upper G.I. endoscopy. Endoscopic evidence of gastritis to predict H. pylori infection was 50% specific and 46% sensitive. Rapid and 24 hours urease test and culture were 100% specific when compared with histology and their sensitivity was 71%, 62% and 21% respectively. Of the three 100% specific tests, rapid urease test yields results within 15 minutes; therefore this test being easy, rapid and sensitive should be used for screening of H. pylori infection; followed by histology for further confirmation.

RESUMEN

Twenty patients were divided into 2 groups. Group - A had 10 ASA Grade I while Group - B consisted of 10 asymptomatic patients with ECG evidence of ischaemia. One week of coronary dilator treatment was given to the patients in group-B before surgery. Intra operative cardioscopic monitoring was done in both the groups. In group-B, 5 patients showed ECG evidence of intraoperative myocardial ischaemia, and/or ventricular ectopics, which were in most cases attributable to anaesthetic/surgical stimuli but were not always associated with increased Rate pressure product. In group-A only one patient showed ECG evidence of ischaemia intraoperatively. It is therefore concluded that patients with optimal therapy for IHD continue to be at risk of intraoperative cardiovascular abnormalities.

RESUMEN

Thirty-six adult female patients undergoing abdominal hysterectomy under general anaesthesia were evaluated for the haemodynamic effects of IV verapamil in bolus dose of either 50 micrograms/kg (group A), 60 micrograms/kg (group B) or 75 micrograms/kg (group C). The result showed a dose dependent fall in the mean arterial pressure and increase in the P-R interval. The fall in heart rate was marginally more in group A and B as compared to group C.

RESUMEN

The quality of extubation and circulatory changes during extubation of the trachea with or without prior xylocaine (1 mg/kg iv) were observed in 30 patients having CAD or 2 or more cardiac risk factors. 73.33 per cent of the patients receiving xylocaine had smooth extubation (grade I) in contrast to only 46.66 per cent in the control group. Also, in patients in the xylocaine group, the circulatory changes were statistically insignificant as compared to highly significant rise in rate pressure product in the control group.

RESUMEN

The Mantoux test was done on 181 tuberculous patients, of which 124 were sputum-positive. In 102 (93%) sputum positive cases the tuberculin reaction was between 10 and 19 mm. and in only 2 cases was the reaction less than 6 mm. In the sputum negative cases 7 (12%) were Mantoux negative and 5 (9%) had reactions of less than 10 mm. In some of the sputum-negative cases the diagnosis of tuberculosis may have been incorrect.
