RESUMEN

We present a synoptic analysis of the ground motions from the 11 March 1933 Mw 6.4 Long Beach, California, earthquake, the largest known earthquake within the central Los Angeles Basin region. Our inferred shaking intensity pattern supports the association of the earthquake with the Newport-Inglewood fault; it further illuminates the concentration of severe damage in the town of Compton, where accounts suggest vertical ground motions exceeding 1 g. We use a broadband simulation approach to develop a rupture scenario for this earthquake, informed by the damage distribution. The predicted shaking for a 25-km-long fault matches the intensity distribution, with an indication that non-linear site response on soft sediments in some near-field regions was stronger than predicted using a simple model to account for non-linearity. Our results suggest that the concentration of damage near Compton can be explained by a combination of local site amplification, source-controlled directivity, and three-dimensional basin effects whereby energy was channeled towards the deepest part of the Los Angeles Basin.

Concerns regarding pain constitute a large component of dental anxiety, and patients with high dental anxiety are likely to have exaggerated memory and prediction of dental pain. It remains to be investigated, however, if memory of anxiety is exaggerated in a manner similar to that of pain, and if anxiety and pain assimilate in memory over time. A sample of 79 patients presenting for emergency extraction rated their anxiety and pain before, during, and two weeks after the procedure. Measures of trait dental anxiety and fear of pain also were collected. All patients exaggerated their recall of procedure pain, but only those high in trait dental anxiety exaggerated their recall of anxiety. Highly anxious patients reported more pain prior to the procedure and expected more pain; ratings of anxiety and pain for all participants assimilated over time.

RESUMEN

Tramadol hydrochloride is a novel, centrally acting analgesic with two complementary mechanisms of action: opioid and aminergic. Relative to codeine, tramadol has similar analgesic properties but may have fewer constipating, euphoric, and respiratory depressant effects. A two-center randomized double-blind controlled clinical trial was performed to assess the analgesic efficacy and reported side effects of tramadol 100 mg, tramadol 50 mg, codeine 60 mg, aspirin (ASA) 650 mg with codeine 60 mg, and placebo. Using a third molar extraction pain model, 200 healthy subjects were enrolled in a 6-hour evaluation after a single dose of drug. Of the 200 patients enrolled, seven provided incomplete efficacy data or discontinued prematurely and one was lost to follow-up. Using standard measures of analgesia, including total pain relief score (TOTPAR), maximum pain relief score (MaxPAR), sum of pain intensity difference scores (SPID), peak pain intensity difference (Peak PID), remedication, and global evaluations, all active treatments were found to be numerically superior to placebo. ASA/codeine was found to be statistically superior to placebo for all measures of efficacy. Tramadol 100 mg was statistically superior to placebo for TOTPAR, SPID, and time of remedication, whereas tramadol 50 mg was statistically superior to placebo onlyfor remedication time. Codeine was not found to be statistically superior to placebo for any efficacy measure. A greater TOTPAR response compared with all other active measures was seen for ASA/codeine during the first 3 hours of study. The 6-hour TOTPAR scores for the tramadol groups and ASA/ codeine group were not significantly different. Gastrointestinal side effects (nausea, dysphagia, vomiting) were reported more frequently with tramadol 100 mg, ASA/ codeine, and codeine 60 mg than with placebo.

STUDY OBJECTIVE: To describe the type of medical care that disaster medical assistance teams (DMATs) provided to a community struck by a major hurricane. STUDY DESIGN: A prospective study describing the use of DMAT field clinics by a population affected by a major hurricane. Data regarding the type of medical care provided to disaster victims and the acuity of each patient's medical condition were abstracted from medical charts at each field clinic. SETTING: Three DMAT field clinics that provided medical care to residents of Kauai, Hawaii, after Hurricane Iniki struck the island on September 11, 1992. RESULTS: From September 16 to 19, 1992, three DMATs provided medical care to 614 people. The patients' average age was 34 years, and 60% were male. The largest treatment categories were injury (40.4%), illness (38.6%), and preventive services (9.0%). Most illnesses and injuries were minor, and 99% of the patients were ambulatory. Only 33 patients (5.4%) were referred to another medical provider. Referrals were generally for procedures not available in DMAT field clinics rather than for life-threatening conditions. CONCLUSION: DMATs sent to assist with the medical needs of a US community struck by a major hurricane should be prepared to deliver basic medical services and primary health care. The need for these medical services will continue beyond the impact phase of a hurricane disaster.

RESUMEN

Bilateral bifid mandibular condyles are a rare anomaly that had previously been described only by Hrdlicka in material from skull collections. We present here three cases of bilateral bifid mandibular condyles in patients who sought treatment for other reasons. The radiographic appearance of this anomaly and its embryology are discussed.

Previous studies have suggested a role for the isotope bone scan in the evaluation of patients with temporomandibular joint (TMJ) related symptoms. The studies have not evaluated the frequency or significance of increased TMJ uptake in a random population. One hundred fourteen patients receiving routine bone scans were asked to complete questionnaires regarding TMJ symptoms. The data show that marked TMJ uptake is relatively rare (4%) and correlated with appropriately lateralizing symptoms in all cases. Lesser degrees of increased TMJ uptake correlated poorly (50% symptomatic). The authors suggest that increased TMJ uptake is a significant finding on routine isotope bone scans. The role of the isotope bone scan in the symptomatic patient remains unclear.

RESUMEN

We have described two cases involving unusual rectal foreign bodies and their subsequent removal. Suggestions are included to aid in the removal of colorectal foreign bodies, as well as references to provide for the appropriate management of these cases in the emergency department. Primary care providers need to be aware of the issue of colorectal foreign bodies as a clinical entity and the fact that their occurrence is expected to continue to rise dramatically. Patient education of the dangers inherent in the insertion of objects into the colorectal area should be more prevalent. At the very least, all physicians need to be aware of anal eroticism and the necessity to take a careful medical history.
