The study aimed to analyze the economic impact of the adoption of optimized and nutritionally balanced diets to Brazilian families, considering the Brazilian dietary guidelines and the economic disparities of the population. Data from the Brazilian Household Budget Survey from 2008-2009 (550 strata; 55,970 households) were used. About 1,700 foods and beverages purchased by the Brazilians were classified into 4 groups according to NOVA system. Linear programming models estimated isoenergetic diets preserving the current diet as baseline and optimizing healthier diets gradually based on the "golden rule" of the Brazilian dietary guidelines, respecting nutritional restrictions for macronutrients and micronutrients (based on international recommendations) and food acceptance limits (10th and 90th percentiles of the per capita calorie distribution from the population). The diet cost was defined based on the sum of the average cost of each food group, both in the current and optimized diets (BRL per 2,000Kcal/person/day). The economic impact of the Brazilian dietary guidelines to Brazilian household budget was analyzed by comparison the cost of the optimized diets to the cost of the current diet, calculated for the total population and by income level. Three healthier diets were optimized. Current diet cost was BRL 3.37, differed among low- and high-income strata (BRL 2.62 and BRL 4.17, respectively). Regardless of income, diet cost decreased when approaching the guidelines. However, low-income strata compromised their household budget more than two times the high-income strata (20.2% and 7.96%, respectively). Thus, the adoption of healthier eating practices can be performed with the same or lower budget.

About one third of the most common cancers could be prevented by the reduction of modifiable behavioral risk factors. We aimed to identify behavioral patterns of risk and protective factors for cancer in Brazil, between 2014 and 2015. Data from Vigitel Survey (n = 95,027 adults aged ≥ 18 years) from all Brazilian capitals and Federal District were used. Thirteen risk (RBF) and protective behavioral factors (PBF) for cancer were investigated. RBF included the consumption of red meat, meat with high-fat content, soft drinks, sweets and abusive alcohol, replacement of lunch/dinner for snacks, television viewing, obesity, and smoking. PBF included the consumption of beans, fruits and vegetables, and physical activity practice. Patterns were identified by principal component analysis and linear regressions models assessed its association with sociodemographic characteristics. Four behavioral patterns for cancer were identified. The 'healthy behavior pattern' and the 'unhealthy food consumption pattern' were positively associated to females and schooling. The 'unhealthy behavior pattern' and the 'mixed behavior pattern' were both negatively associated to females, age and schooling. Our data revealed different vulnerable population groups for cancer. Actions for reduction of modifiable behavioral risk factors aiming at cancer prevention should consider distinct approaches by sex, age, and schooling.

El objetivo del estudio fue analizar el impacto económico de la adopción de dietas optimizadas y nutricionalmente equilibradas en familias brasileñas, considerando las guías alimentarias y las disparidades económicas de la población. Los datos se recopilaron de la Encuesta de Presupuestos por Hogares desde 2008-2009 (550 estratos; 55 970 hogares). Se clasificaron alrededor de 1.700 comidas y bebidas, adquiridas por brasileños en 4 grupos, según el sistema NOVA. Los modelos de programación lineal estimaron dietas isoenergéticas, preservando la dieta actual como base de referencia, y optimizando las dietas más sanas gradualmente, basadas en la "regla de oro" de las guías alimentarias, respecto a restricciones nutricionales en macronutrientes y micronutrientes (basadas en recomendaciones internacionales), y los límites de aceptación alimentarios (10o y 90o percentiles de la distribución calórica per cápita de la población). El coste de la dieta se definió basado en el total del coste medio de cada grupo de comida, ambos en las dietas actual y optimizada (BRL por 2.000Kcal/persona/día). El impacto económico de las guías alimentarias para el presupuesto por hogar se analizó mediante comparación del coste de dietas optimizadas con el coste de la dieta actual, calculado para el total de población y por nivel de ingresos. Se optimizaron las tres dietas más saludables. El coste de la dieta actual fue BRL 3,37, diferenciando los estratos entre bajo- y altos-ingresos (BRL 2,62 y BRL 4,17, respectivamente). A pesar del ingreso, el coste de la dieta decreció cuando se aproximaba a las recomendaciones de las guías. No obstante, los estratos de bajos ingresos comprometieron su presupuesto doméstico más de dos veces, respecto a los estratos de ingresos más altos (20,2% y 7,96%, respectivamente). Por ello, la adopción de prácticas alimentarias más sanas se puede conseguir con el mismo presupuesto o incluso más bajo.


O objetivo desse estudo foi analisar o impacto econômico da adoção de dietas otimizadas e nutricionalmente balanceadas para famílias brasileiras, considerando as diretrizes alimentares para a população brasileira e as disparidades econômicas da população. Foram usados dados da Pesquisa Nacional de Orçamentos Familiares de 2008-2009 (550 estratos; 55.970 domicílios). Cerca de 1,7 mil alimentos e bebidas adquiridos pelos brasileiros foram classificados em quatro grupos de acordo com o sistema NOVA. Modelos de programação linear estimaram dietas isoenergéticas preservando a dieta atual como linha de base e otimizando dietas mais saudáveis gradativamente com base na "regra de ouro" das diretrizes alimentares, respeitando restrições nutricionais para macronutrientes e micronutrientes (com base em recomendações internacionais) e limites de aceitação alimentar (10º e 90º percentis da distribuição de calorias per capita da população). O custo da dieta foi definido a partir da soma do custo médio de cada grupo de alimentos, tanto na dieta atual quanto na otimizada (R$ por 2.000Kcal/pessoa/dia). O impacto econômico das diretrizes alimentares para o orçamento familiar brasileiro foi analisado comparando-se o custo das dietas otimizadas com o custo da dieta atual, calculado para a população total e por nível de renda. Três dietas mais saudáveis foram otimizadas. O custo da dieta atual era de R$ 3,37, diferindo entre os estratos de baixa e alta renda (R$ 2,62 vs. R$ 4,17). Independentemente da renda, o custo da dieta diminuiu com a abordagem das diretrizes. No entanto, os estratos de baixa renda comprometeram seu orçamento familiar mais de duas vezes que os estratos de alta renda (20,20% vs. 7,96%). Assim, a adoção de práticas alimentares mais saudáveis pode ser realizada com orçamento igual ou inferior.

OBJECTIVE: To determine the agreement between body self-image (based on the Stunkard figure rating scale) and nutritional status and to evaluate body satisfaction among the Khisêdjê indigenous people of Parque Indígena do Xingu (Xingu Indigenous Park). METHODS: A cross-sectional study involving 131 natives aged 20 and older. Data on body image, body mass index and waist circumference were collected. Kappa statistics, χ2 (p < 0.05), crude and adjusted prevalence ratios and Student's t-test were used for data analysis. RESULTS: The prevalence of overweight and obesity was respectively 42 and 5.3%. The percentage of satisfaction with body profile was 61.8% with no difference between the sexes. There was good agreement between actual and ideal self-image (p < 0.001), but poor agreement between actual and ideal self-image with nutritional status for both sexes. A higher prevalence of body dissatisfaction due to overweight was detected in individuals with central obesity and overweight. CONCLUSION: The results suggest that body self-image evaluated by the Stunkard silhouette scale has little applicability as an indicator of nutritional status among the indigenous Khisêdjê of Xingu Indigenous Park.

RESUMO: Objetivos: Verificar a concordância entre autoimagem corporal (escala de silhuetas de Stunkard et al.) e estado nutricional e avaliar a satisfação corporal em indígenas khisêdjê do Parque Indígena do Xingu. Métodos: Estudo transversal que incluiu 131 indígenas khisêdjê, com 20 anos ou mais. Coletaram-se dados sobre imagem corporal, índice de massa corporal e perímetro da cintura. Foram utilizados a estatística kappa, o teste χ2 (p < 0,05), as razões de prevalências brutas e ajustadas e o teste t de Student. Resultados: As prevalências de sobrepeso e obesidade foram, respectivamente, 42 e 5,3%. A porcentagem de satisfação com o perfil corporal foi de 61,8%, sem diferença entre os sexos. Houve boa concordância entre autoimagem real e autoimagem ideal (p < 0,001), porém baixa concordância entre a autoimagem real e ideal e o estado nutricional para ambos os sexos. Maior prevalência de insatisfação corporal por excesso de peso foi detectada entre indivíduos com obesidade central e excesso de peso. Conclusão: Os resultados sugerem que a autoimagem corporal avaliada por meio da escala de silhuetas de Stunkard et al. tem pouca aplicabilidade como indicador do estado nutricional de indígenas khisêdjê do Parque Indígena do Xingu.

OBJECTIVE: To analyse trends in sweetened beverages consumption among adults in Brazil between 2007 and 2016. DESIGN: A time-series analysis, with data from the Surveillance System of Risk and Protection Factors for Chronic Diseases by Telephone Survey (VIGITEL). The prevalence of regular consumption (≥5 d/week), the average daily consumption (millilitres) and the prevalence of non-consumption of these beverages were analysed. The temporal variations of the indicators were calculated by linear regression. The analyses were performed for the complete set of the evaluated population and stratified by sociodemographic characteristics. SETTING: Brazilian capitals and Federal District. SUBJECTS: Brazilian adults aged ≥18 years (n 519 641). RESULTS: There was a reduction in both regular and average daily consumption of sugar- and artificially sweetened beverages (-1·28 percentage points (pp)/year, P=0·001 and -9·63 ml/year, P=0·001, respectively). The same result regarding regular consumption was found when only sugar-sweetened beverages were analysed (-1·11 pp/year, P=0·011). Similar trends were identified in the stratified analyses, with a greater magnitude of reduction among males, young adults, those with higher schooling and residents of more developed regions. Coincidentally, there was an increase in the prevalence of adults who did not consume sweetened beverages (1·54 pp/year, P=0·005). CONCLUSIONS: The consumption of sweetened beverages decreased during the period. However, a significant portion of the population still referred a daily consumption of these beverages.

OBJECTIVE: To describe trends in the prevalence of anaemia and Fe deficiency in children under 2 years of age living in a town in western Brazilian Amazonia. DESIGN: Temporal analysis of two cross-sectional population-based surveys. Information on socio-economic status, morbidity and breast-feeding was obtained using a structured questionnaire. Child weight and length were measured for anthropometric evaluation. Concentrations of blood Hb, plasma ferritin and soluble transferrin receptor were measured. SETTING: The town of Acrelândia, state of Acre, north-west Brazil. SUBJECTS: A total of 170 and 224 participants of the 2003 and 2007 surveys, respectively. RESULTS: Comparison between the 2003 and 2007 surveys revealed no statistically significant differences in the prevalence of anaemia (48 (95% CI 39, 56)% to 40 (95% CI 33, 47)%) or Fe-deficiency anaemia (39 (95% CI 30, 48)% to 37 (95% CI 30, 45)%), respectively. However, an increase in the overall prevalence of Fe deficiency from 62 (95% CI 51, 68)% to 81 (95% CI 75, 86)% was observed (χ2 test, P≤0.001). In age- and sex-adjusted analyses for risk of Fe deficiency, only early introduction of cow's milk (<90 d) was associated with Fe deficiency in 2003 (prevalence ratio (PR)=0.76; 95% CI 0.57, 1.01), while caesarean section (PR=1.18; 95% CI 1.03, 1.35) and birth weight <3500 g (PR=1.15; 95% CI 1.00, 1.34) were associated with Fe deficiency in 2007. CONCLUSIONS: No improvements were observed in the prevalence of anaemia, exposing a worrying scenario for public health, while a significant increase was found in the prevalence of Fe deficiency in the studied infants and toddlers.

Our objective was to investigate nutritional status and complementary feeding practices in children from 6 to 24 months of age living in the Western Brazilian Amazon. A cross-sectional study was conducted within an urban area of Acrelândia, Acre State. A total of 164 children were studied. Prevalence rates for stunting and anemia were 12% and 40%, respectively, and overall prevalence of iron deficiency was 85%. Vitamin A and B12 serum levels were below normal thresholds in 15% and 12% of children, respectively. Low intake was observed for the following nutrients (% of children): folic acid (33%), vitamin C (40%), vitamin A (42%), zinc (46%), and iron (71%). Iron bioavailability in the diet was approximately 8%. Very low dietary intakes of fruits, vegetables, and meats were observed, in contrast with excessive consumption of cow's milk and porridge.

Com objetivo de investigar o estado nutricional e alimentação complementar em crianças de 6 a 24 meses, residentes na Amazônia Ocidental Brasileira, um estudo transversal foi realizado na área urbana do Município de Acrelândia, Estado do Acre, com 164 crianças. As prevalências de déficit de estatura/idade e anemia foram de 12 por cento e 40 por cento, respectivamente, e de deficiência de ferro isolada, de 85 por cento. Os níveis séricos das vitaminas A e B12 estavam baixos em 15 por cento e 12 por cento das crianças, respectivamente. Houve baixo consumo alimentar dos seguintes nutrientes ( por cento de crianças abaixo das recomendações): ácido fólico (33 por cento), vitamina C (40 por cento), vitamina A (42 por cento), zinco (46 por cento) e ferro (71 por cento). A biodisponibilidade de ferro da dieta foi de 8 por cento. Observou-se baixo consumo de frutas, hortaliças e carnes, com consumo excessivo de leite de vaca e mingau.

Introdução: O panorama nutricional da infância brasileira nas últimas décadas caracteriza-se por tendência ao declínio da prevalência da desnutrição com manutenção da prevalência de anemia, principalmente a ferropriva. Contudo, desigualdades regionais ainda persistem especialmente na região norte do país. A substituição precoce e inadequada do aleitamento materno por outros alimentos tem sido considerada principal responsável pelas deficiências nutricionais entre menores de dois anos. Objetivo: Caracterizar a evolução das práticas de aleitamento materno, prevalências de anemia, deficiência de ferro e desnutrição em crianças menores de 2 anos em área urbana de Acrelândia, Estado do Acre. Métodos: Análise temporal de dois inquéritos transversais de base populacional realizados em 2003 (n igual a 170) e 2007 (n igual a 224). Informações sobre condições socioeconâmicas, morbidade e aleitamento materno foram obtidas por meio de questionário estruturado. Peso e comprimento das crianças foram medidos pela equipe de pesquisa, sendo considerada desnutrida a criança cujo indicador de altura para idade encontrava-se abaixo de -2 escores z, segundo padrão da Organização Mundial da Saúde (OMS). Avaliaram-se as concentrações de hemoglobina sanguínea (maiores de 6 meses de idade), ferritina e receptor de transferrina plasmáticos para diagnóstico de anemia e deficiência de ferro segundo critérios da OMS. Resultados: Na comparação entre os inquéritos 2003 e 2007, não houve diferenças estatisticamente significantes nas prevalências (intervalo com 95 por cento de confiança) de aleitamento materno total de 46 por cento (39 por cento-54 por cento) para 53 por cento (46 por cento-59 por cento), aleitamento materno exclusivo em menores de 6 meses de 23 por cento (10 por cento-41 por cento) para 16 por cento (6 por cento-34 por cento), desnutrição de 9 por cento (5 por cento-14 por cento) para 11 por cento (8 por cento-16 por cento), anemia de 48 por cento (39 por cento-56 por cento) para 40 por cento (33 por cento-47 por cento) e anemia ferropriva de 36 por cento (28 por cento-45 por cento) para 36 por cento (29 por cento-44 por cento), respectivamente. No entanto, houve aumento na prevalência de deficiência de ferro de 62 por cento (53 por cento-70 por cento) para 81 por cento (75 por cento-86 por cento) (teste do X2, p menor ou igual a 0,001).
