RESUMEN

AIM: The objective of the present study was to evaluate the influence of pollen of Mediterranean Cypress (Cupressus sempervirens) on the effectiveness of the spa and health resort-based treatment (SHRT) of the patients with broncho-pulmonary pathology at the southern coast of the Crimea (SCC). MATERIAL AND METHODS: This article presents the results of the analysis of the data on 122 patients presenting with broncho-pulmonary pathology who received SHRT under the conditions of the Southern Coast of the Crimea. Fifty one (41.8%) of these patients suffered from chronic obstructive pulmonary disease and 71 (58.2%) ones had bronchial asthma. The whole group was comprised of 44 men (36.1%) and 78 women (63.9%). The average (median) age of the patients was 55.8 years. All the participants of the study underwent the comprehensive examination including the general medical and physical examination, complete blood count, the study of sputum cytology and immunological properties of blood including the standard characteristics, IgE and lysozyme levels), evaluation of the respiratory and locomotor (physical) functions with the use of the 6-minute walking test. RESULTS: The aeropollenological study of the aerial environment of the southern coast of the Crimea in the region of Yalta showed that the «bloom¼ of the Mediterranean Cypress occurs during the period from February till April inclusive. The highest concentrations of cypress pollen are observed in March and early April. The overall effectiveness of SHRT for the patients with broncho-pulmonary pathology arriving for the treatment at the southern coast of the Crimea from other Crimean localities does not depend on the period of «flowering¼ (the presence of pollen in the air) of the Mediterranean Cypress. In these patients, the termination of the spa and health resort-based treatment during the «flowering¼ of the Mediterranean Cypress resulted in nothing more than a strained adaptive response manifested as the altered blood leukocyte count. Such reaction is considered to be a natural consequence of intake of antigenic substances into the human body.

RESUMEN

One of the primary objectives of military otorhinolaryngology is to provide specialized otorhinolaryngological aid to the sick,wounded, and injured servicemen during local armed conflicts and in times of peace. The military forces of the Russian Federation must have up-to-date, mobile, readily available, and many-level otorhinolaryngological service. The concept of further development of this service has been elaborated to ensure its balanced and sustainable functioning. The concept specifies a system of measures for the enhancement of the efficacy of military otorhinolaringological services facilitating the fulfillment of the assigned tasks, improvement and maintenance at the high level of preparedness of its assets and capabilities for the proper actions under conditions of the armed conflicts and other emergency situations, promotion of the availability and enhancement of the quality of the provided medical aid.

RESUMEN

The objective of the present study was to carry out clinical examination of the patients presenting with peripheral total cochleovestibular syndrome combined with vertebrobasiliar insufficiency before (in the phase of exacerbation), during, and after the conservative treatment that consisted of a course of traditional drug therapy. A relatively new diagnostic method of computed videooculography was applied. The 316 patients with the aforementioned combined pathology who received conservative pharmacotherapy were available for the dynamic follow-up. It was shown that computed videooculography provides a highly sensitive diagnostic tool detecting even the minimal changes in the responsiveness of the vestibular analyzer. The introduction of the computed videooculographic technique in the program of medical examination makes it possible to considerably increase the accuracy of diagnostics, optimize differential diagnostics and management of the patients suffering from vestibular disorders.

RESUMEN

The objective of the present study was to estimate the efficacy of the tools and methods for the optimization of the activity of the central nervous system (CNS) and analyzers involved in the maintenance of the statokinetic (SK) stability in man. To this effect, we evaluated the outcome of bemitil treatment during 10 days with and without A.I. Yarotsky test and the influence of these procedures on the pathophysiological characteristics of selected elements of the work of operators of aerospace systems. Based on the data obtained in the study, the tools and methods have been developed that allow the efficacy and quality of certain aspects of the operators' activity to be improved, viz. general working capacity under conditions requiring enhanced statokinetic stability, self-confidence, emotional and somatic comfort.

The objective of the present work was to study the role of GST genes in pathogenesis of various neurodegenerative conditions with special reference to sensorineural impairment of hearing. The DNA samples obtained from 200 subjects were available for the investigation; 109 of them were the patients hospitalized in the otorhinolaryngologcal clinic of S.M. Kirov Military Medical Academy for the treatment of uni- or bilateral impairment of hearing. The polymerase chain reaction with subsequent specific restriction endonuclease analysis was used to study genetic mutations. The analysis was focused on the 35delG mutation in the GJB2 gene (DFNB1). In addition, the MTRNR1 (12S rRNA) and MTTS1 (serine tRNA) genes in the patients with acquired sensorineural impairment of hearing were subjected to analysis. Polymorphism of the second detoxication phase gene (GSTM1) was investigated. It was shown that mitochondrial genes MTRNR1 and MTTS1 contribute to the development of acquired sensorineural impairment of hearing. The frequency of heterozygous 35delG mutations of the GJB2 (DFNB1) gene among the subjects with impaired hearing does not significantly differ from its prevalence in the general population. The allelic mutation of the GSTM1 gene is believed to be a predisposing factor for the development of sensorineural impairment of hearing under the action of intoxication and/or infections.

RESUMEN

This work was designed to develop a mathematical model based on the results of computed stabilometry in the subjects differing in the degree of resistance to motion sickness. The model is designed for the purpose of instant diagnostics in the course of occupational selection. Stabilometric characteristics of 61 men aged from18 to 24 years were compared with those obtained by the method of continuous cumulation of Coriolis accelerations. The accuracy of diagnostics with the use of stabilometry was estimated at 93.2%. It is concluded that computed stabilometry provides an efficacious tool for the assessment of sensitivity to motion sickness for the purpose of professional selection.

RESUMEN

The present work was designed to estimate changes in the quality of the equilibrium function (QEF) and dynamic stabilization indices (DSI) of the vertical position of the body in comparison with the traditional stabilometric characteristics (the length and the area of statokinesigram, mean radius of body deviation, average linear centre of foot pressure displacement velocity). The computed stabilometric technique was used to study the equilibrium function in the patients (n=133) presenting with otorhinolaryngological problems and requiring the surgical treatment. It was shown that selected stabilometric characteristics (such as QEF and DSI) may be useful for control of the efficacy of the treatment of surgical patients; moreover, they can be applied to the evaluation of dynamics of the patients' health status during the postoperative period.

In the system of staged treatments the medical company is the first medical division where the qualified medical aid is delivered. The last one consists of elimination of conditions menacing for life, preventive maintenance of their complications at traumas and wounds ENT-organs, preparation of the wounded man sick to evacuation, treatment of sharp pathology of ENT-organs. There is an expert-otolaryngologist in medical company of a brigade. He renders the out-patient-polyclinic and urgent help at ENT-diseases and traumas. The organization and carrying out of prophylactic medical examination for preservation of health of military men are very important. It allows to reveal in due time ENT-disease is important and to arrange necessary medical aid.

The terminology of the battle mechanical damages has a direct connection with determination of frequency and structure of such damages and value of sanitary loss. The entitled figures are necessary for troop basis during the medical treatment. The present article is written because of appearance in large academic and research works of military-medical directionality inadequate and incorrect data of frequency of damages by the similar wounds. It is caused by the false methodological approach to the usage of this terminology and requires the valid correction of results of such works.

RESUMEN

One of the most important goals in military otolaryngology is a prevention ENT-diseases of armed forces personnel. Prophylactic medical examination of their staff is the major instrument in it. The article presents detailed project of its realization in accordance with present-day classification of ENT-diseases (International Classification of Diseases by World Health Organization), new structure of medical service and its resources and conditions. The diseases of the nose, ear and throat were considered. Algorithm of patients' observation and volume of diagnostic examinations were given.

RESUMEN

There was effectuated a research of killability of traumatic weapon in laboratory as well as during analyzing of clinical observations. There were considered the results of treatment of 8 persons, suffered from traumatic weapon. It was occurred a clinical recovery by 7 wounded persons from 8. There was 1 case of lethal outcome for first hours, caused by trauma of temporal bone and cerebrum (suicide). Thus, influence of traumatic weapon on ENT-organs can be a reason of serious penetrating wounds, facial bones fracture, hematomas and contusion, formation of foreign bodies and middle ear barotraumas.

RESUMEN

The effect of hypoxic-hypercapnic trainings (HHT), conducted before the induction of inflammation or started at next day after foreign body bringing, was shown in experiment on rats with chronic inflammation process in bronchopulmonary tissue. HHT conducted before the induction of inflammation in general had an favourable effect. HHT conducted later had a positive effect together with expressed negative changes, in particular worsening of inflammation process in lung.
