RESUMEN

This work aims to develop and test a vendor-independent computer-aided diagnosis (CAD) system that uses conventional B-mode ultrasound images to distinguish between benign and malignant breast tumors. Three morphological features were extracted from 323 breast tumor lesions including the perimeter, regularity variance, and circularity range ratio. Lesions were segmented using the active contour method via semi- andfully-automated algorithms. Then, the support vector machine classifier was used to identify breast lesions. Results of the CAD system exhibited accuracies of 95.98% and 95.67%using the semi- and fully-automated segmentation, respectively. Based on the preliminary results, this CAD system with such unique combination of geometrical features shall improve the diagnostic decisions and may reduce the need of unnecessary needle biopsies.

Young patients with breast cancer (BC) are often concerned about treatment-induced infertility and express maternity desire. Conception after BC does not seem to affect outcome, but information in estrogen-receptor positive (ER+) disease is not definitive. From September 2012-March 2013, 212 evaluable patients with ER+ early BC, <37 years at diagnosis, from 5 regions (Europe/US/Canada/Middle-East/Australia) answered a survey about fertility concerns, maternity desire and interest in a study of endocrine therapy (ET) interruption to allow pregnancy. Overall, 37% of respondents were interested in the study; younger patients (≤30 years) reported higher interest (57%). Motivation in younger patients treated >30 months was higher (83%) than in older women (14%), interest was independent of age in patients treated for ≤30 months. A prospective study in this patient population seems relevant and feasible. The International-Breast-Cancer-Study-Group (IBCSG), within the Breast-International-Group (BIG) - North-American-Breast-Cancer-Groups (NABCG) collaboration, is launching a study (POSITIVE) addressing ET interruption to allow pregnancy.

RESUMEN

Breast cancer is one of the most frequently diagnosed malignancy among adolescent and young adult (AYA) women, accounting for approximately 14% of all AYA cancer diagnoses and 7% of all breast cancer. Breast cancer in AYA women is believed to represent a more biologically aggressive disease, but aside from commonly known hereditary predispositions, little is still known about the underlying molecular genetic causes. This review examines the current trends of breast cancer in AYA women as they relate to clinical, social, genetic, and molecular pathologic characteristics. We highlight existing trends, treatment and imaging approaches, and health burdens as they relate to breast cancer in AYA women and provide a discussion on ways to help improve the overall management of this breast cancer cohort.
