RESUMEN

Purpose of this paper is to report a case of foreign body obstructing the middle lobe bronchial origin, surgically removed through a longitudinal bronchotomy on the pars membranacea, reached by passing posteriorly to the lung. Anatomical considerations suggest that longitudinal bronchotomy on the pars membranacea of the main bronchial axis (stem bronchi, intermedius br (right), and lower lobar bronchus) gives direct visualization from inside also of foreign bodies sited into the lobar bronchi (sup, middle and lingula), allowing an easy bronchotomy repair, without lumen distortion or stenosis. Since the pulmonary artery is not in contact with this pars membranacea, broncho-arterial fistula in case of suture line dehiscence is quite improbable.

RESUMEN

The recent observation of a case of schwannoma in the second part of the duodenum lead the authors to a literature survey in order to state the present knowledge of this subject. Schwannoma is an ectodermal neoplasm arising from the nerve sheath that envelop axons. In the gastrointestinal tract these tumours can concern cells of the myentericus and submucosus plexus of the intestinal wall. A duodenal location is extremely rare. Gastroduodenal endoscopy, abdominal ultrasonography and CT scan were found as important investigatory tools, but only histological examination of the excised tissue suggests the definitive diagnosis. Therapy consists in the radical excision of the tumour; this is rarely obtained by endoscopic way because of the submucosal neoplastic growth. Then a surgical laparotomic approach is more often required and it allows to perform local excisions through a duodenotomy, partial duodenectomy or ampullectomy and more complex operations, like a cephalic duodenopancreatectomy, in front of extended papillar involvement. The latter was the suitable treatment for radical removing of the juxtapapillar schwannoma reported by the authors; the uneventful postoperative course, the patient's excellent general conditions and nutriture at 12 months confirm the validity of the therapeutic policy.

RESUMEN

We report on a clinical case where microsurgical techniques successfully supported traditional surgery in a wide reconstruction between the oropharynx and small bowel. Several years ago, the patient sustained a severe corrosive injury of the upper digestive tract with subsequent esophageal stricture and stiffening; at that time, an emergency gastrectomy was performed. In this case, the restoration of the defect could not rely on the classic colonic interposition. During the operation the ileo-colic flap, well-fitted for tension-free reconstruction, revealed the foreseen inadequacy of its vascularization based on the sole middle colic vascular pedicle. The blood supply to its proximal part was then increased by microanastomosis between the right internal mammary and ileo-colic vessels. The revascularization ensured the viability of the interposed tissue. Oral intake resumed after 3 weeks; nowadays the patient is able to maintain her ideal weight with adequate nutrition.

RESUMEN

The purpose of this paper is to illustrate the use of a device to quickly perform little holes in the costal lateral arch for synthesis of the standard posterolateral thoracotomy. The working principle of the instrument is similar to that of a hollow punch device. Preliminary laboratory experience allowed to identify the best diameter of the costal holes (2 mm), without fracture. Absorbable suture were passed through the holes and tied to approximate the facing ribs. The modality of costal plane synthesis, very easy and quick with this device, makes possible to prevent intercostal nerve compression and accidental intercostal vessel lacerations. The so frequent, severe and often long lasting painful sequelae of thoracotomy, on the other hand so rare in other surgical wound (i.e. laparotomy, sternotomy, etc.) justify in our opinion the routine use of this costal plane synthesis technique.

RESUMEN

The purpose of this report is to illustrate a bronchoplastic technique in upper right lobectomy which is equivalent to sleeve resection but allows to preserve the unaffected portion of the bronchial wall. The technique consists in the resection of the infiltrated bronchial wall, sparing the unaffected part The distal bronchial stump is then invaginated into the stem bronchus in such a way that the anastomosis line is located at the highest possible level, ideally where it would be carried out if standard sleeve resection would be performed. The anastomosis is accomplished by U-Shaped, 3-0 vicryl sutures. The major advantage of this procedure is probably due to the preservation of the systemic artery bronchial supply to the distal stump passing through the unaffected bronchial wall; the resulting more vital distal stump may reduce incidence of suture line healing problems.

RESUMEN

The authors critically review the literature in order to ascertain the current state of knowledge regarding the anatomopathological, clinical and therapeutic characteristics of choledochocele, or cystic dilation of the terminal portion of the biliary tract. In particular, the Authors examine the etiopathogenetic aspects to explain how the presence of a choledochocele may lead to the onset of chronic pancreatitis. A clinical case which was recently brought to their attention is examined in detail. It concerns a 27-year-old patient in whom the presence of a large choledochocele had led to the onset of severe cephalopancreatitis which resisted all forms of treatment. In the case in question, ERCP played a decisive role in the diagnosis of the choledochocele, whereas ultrasonography, CT and selective arteriography were useful above all in relation to pancreatitis. In the case described ultraradical surgery, namely duodenocephalopancreatectomy, allowed the severe pancreatopathy and its etiological cause (choledochocele) to be efficaciously treated, confirming that this type of surgery currently plays an important role in the treatment of chronic pancreatitis.

RESUMEN

The malignant fibrous histiocytoma is a tumor whose histological origin is still unclear. It occurs in the deep soft tissues and is only rarely found in the retroperitoneum. Because of scarse symptomatology during growth, the tumor is usually only diagnosed when it has reached considerable dimensions. Retroperitoneal origin is therefore difficult to diagnose by either ultrasonography or CT. Needle biopsy is at present the sole method which allows a precise diagnosis of the lesion to be made.

After examining the literature on the subject, all admissions to the Pavia University Department of Surgical Pathology from 1-1-1974 to 31-12-1985 belonging to the IIIrd age and suffering from non-neoplastic and neoplastic bile way pathology have been reviewed. The two groups were studied on the basis of their histories and laboratory examinations, the outcome of the operation and its complications. It was found that in patients affected by non-neoplastic pathology (lithiasis of the bile ways, alithiasic cholecystitis, etc.) the operation of choice was well tolerated, unlike emergency surgery which in most cases was dangerous. In the IInd group, on the other hand, general condition is very important because on this depends whether or not resective or derivative intervention is carried out. It should be noted here that in all IIIrd age patients associated diseases have to be studied closely for these can contribute to death. It is concluded that in the Ist group of patients, operation can take place in optimal general and local conditions so that the operation can be of choice and not emergency. In the IInd group, as soon as the general condition of the patient so permits, the operation must be as extensive as possible, as age is not an impediment to surgery.

The authors describe an observation of Hodgking's disease with primitive location in ileum, that they treated urgently for intestinal stoppage. After having explained the peculiar history of this disease, they analyze the diagnostic and therapeutic principles, emphasizing the elements that are useful for determining the primitivity of the form and the peculiarity of the treatment.
