RESUMEN

In two autopsy prospective studies, at the Prosectures of the Departmental Hospital and of the Pediatric Hospital in Cluj, we found an incidence of Meckel's diverticulum of 1.16%, 1.46% respectively. Besides, we have retrospectively followed the incidence of the ileal diverticulum during the laparatomies performed at the Pediatric Hospital in Cluj between 1981 and 1990 on an acute abdomen of a supposed appendicular cause. Consulting the pathological bulletins which registered 8,385 laparatomies we found 200 Meckel's diverticula. Of the 200 diverticula, 64 exhibited a pathological process and 136 were trophic, and we suppose that they were found incidentally (incidence of 1.63%, corrected incidence of 1.38%). Up to the age of 16, 4.5% of the diverticula were excised. In the 64 symptomatic cases of ileal diverticulum, 50 presented inflammatory complications, 10 were ulcero-hemorrhagic, two were obstructive, one diverticulum presented an entero-umbilical fistula and one displayed a hemorrhagic infarction. Seventeen of the 200 diverticula presented heterotopic tissues; 12 of the 17 cases were symptomatic; in 64.7% of the cases the heterotopic tissue was the gastric mucous coat. Out of 8,385 laparatomies performed on an acute abdomen of a supposed appendicular cause, in 64 cases the symptomatology was generated by a meckelian pathological process (0.76%). An acute appendicitis was concomitantly present in 22 of the 64 cases.

The authors studied a number of 445 children with malabsorption in which a jejunal biopsy was done. Concerning the celiac disease, they emphasized the value of jejunal biopsy in diagnosis and monitoring the treatment.

RESUMEN

Studying a series of 30 children with chronic gastritis (diagnosed by optic fiber gastroscopy and by mucosal biopsy), authors have founded important changes of the superficial mucosa and gastric glands. The histologic changes could explain the well-known functional alterations in chronic gastritis. They discussed also a possible immuno-pathogenic mechanism for these mucosal ultrastructural changes.

RESUMEN

Neonatal cholestatic disturbances are frequent in premature neonates. An important role in the group of causes which may induce cholestasis is that of hypoplasia of the intrahepatic biliary pathways. Ten cases are described, of prematurely born infants with hypoplasia of the intrahepatic biliary pathways. The children had 980-1,900 g at birth. The diagnosis was made on the basis of the clinical picture, of conjugated hyperbilirubinemia, and of the necropsy findings in 7 of the cases. The incidence and the lethality were high in very small premature infants, in whom compensation responses of the hepatic tissue are rather a particular case. In all the cases the authors noted the presence of malabsorption syndromes, of atrophic enteritis, and a defect in the growth rate.

RESUMEN

The authors report on an infant of three weeks of age, suffering from mycotic septicaemia (Candida), in the course of which the syndrome of disseminated intravascular coagulation (DIC) occurred. The digestive expression of DIC was represented by numerous intestinal intramural haematomas, one of which was large enough to produce the clinical signs of mechanical ileus. The aetiology of the intestinal intramural haematoma is discussed and its relatively frequent occurrence in infants with DIC is stressed.

Kidney biopsies were obtained in 28 children with glomerular diseases and studied using indirect immunofluoreoscence and immunoperoxidase for the detection of IgG, IgA, IgM, Clq, C3c, C4, Fibrinogen and the neoantigens of the terminal C5b-9 complement complex. An affinity-purified rabbit IgG was used to recognize the neoantigens of the assembled terminal components of the complement system into the C5b-9 complex. The immunohistochemical studies were correlated to the clinical data and laboratory investigations. Fourteen of the 28 patients presented specific C5b-9 glomerular deposits; they were also present at the tubular sites in 8 patients and at the vascular sites in 12 patients. An unfavourable evolution was observed for patients presenting C5b-9 deposits in contrast to those without such deposits, even of the same histopathological pattern of glomerulonephritis. The presence of C5b-9 complex at the site of glomerular injury suggests a pathological involvement of the "in situ" complement activation and could be a marker in the prognosis of the glomerular diseases.

Kidney biopsies were obtained in 28 children with glomerular diseases and studied using indirect immunofluorescence and immunoperoxidase for IgG, IgA, IgM, C1q, C3c, C4, fibrinogen and the neoantigens of the terminal C5b-9 complement complex. C5b-9 deposits were detected in glomeruli of 14, in tubules of eight and in vessels of 12 cases. Patients with C5b-9 deposits fared worse than those without such deposits, even with the same histopathological type of glomerulonephritis. The C5b-9 complex suggests an in situ complement activation.

RESUMEN

The parallelism between hypoxic changes in the myocardium revealed by Lie's technique and by fluorescence microscopy and the ionic changes (Na+, K+, Ca2+) was studied in 51 autopsy cases. The Lie technique was positive in 52.6 per cent of the cases of cardiosclerosis with myocardial infarction and in 15 per cent of the cases of cardiosclerosis without infarction, while fluorescent areas were found in all cases of infarction and in 35 per cent of cardiosclerosis cases without infarction. Na+ and K+ were decreased and Ca2+ ions were increased in both ventricles and in both sexes, both in infarctions and in cardiosclerosis cases without infarction. Ionic changes are more probably related to the intensity of cardiosclerosis rather than to age or sex.

RESUMEN

Seven out of 36 gastric carcinomas showed neuroendocrine cells, as revealed by histochemical methods (Grimelius' staining). Histologically, two of these tumors were adenocarcinomas, while the other five were scirrhous type carcinomas. PAS and mucicarmin stainings revealed the presence of intracytoplasmic mucins in all the tumors with a positive argyrophilic reaction. The problems related to histogenesis and clinical behaviour of tumors with a neuroendocrine component are briefly discussed.

RESUMEN

About 50 cases of histiocytic necrotizing lymphadenitis without granulocytic infiltration were published till now. The authors describe the case of a 19-year-old patient with the right ear and the right ring finger bitten by squirrel about 40-50 days before the appearance of an axillary homolateral painful lymphadenomegaly and fever. Histopathologic examination of excised lymph nodes showed areas of necrosis, necrobiosis and nodular histiocytosis. Histiocytes were dystrophic or showed evidence of macrophagy. Problems referring to morphogenesis and etiology are discussed.

RESUMEN

Records of 36,775 autopsies performed between 1870 and 1979 in the Department of Morphopathology, Cluj-Napoca, were re-studied. 10.82 per cent of deaths were due to cancer, whose incidence increased during the last 50 years. Cancer is more often seen in males and its highest incidence was noted between 50 and 70 years of age. Gastric and lung cancers are more frequent in males, while uterine, intestine, stomach and ovarian cancers are the most frequent localizations in females.
