RESUMEN

The in vitro effect of Mycobacterium leprae suspensions on the PMN ability to phagocytizing and killing Candida albicans and Candida pseudotropicalis was studied in forty-five patients of Hansen's disease and in fifteen healthy controls. Our results show no significative differences between the different studied groups, both for the phagocytosis and for the lysis of yeasts. There was no significant changes in the mean values of these functions after previous or simultaneously incubation with Mycobacterium leprae suspensions. Those observations confirmed that there are not alterations in the enzymatic battery of PMN in Hansen's disease patients and that the Mycobacterium leprae presence does not exert stimulating effect on this in vitro model.

RESUMEN

The lymphocyte transformation test was applied to the study of cell mediated immune (CMI) response in vitro to mycobacterial antigens in Bufo arenarum and Leptodactylus ocellatus previously inoculated with a suspension of Mycobacterium tuberculosis. The stimulated samples of blood both in B. arenarum and L. ocellatus showed a significant increase in the percent of blastic dedifferentiation (almost 100% with respect to controls) suggesting the presence of a positive CMI response to PPD. The differences were observed on the fifth day of incubation and lasted until the seventh day.

The cell mediated immunity (CMI) to protein purified derivates of Mycobacterium leprae, Mycobacterium tuberculosis, Mycobacterium avium and Mycobacterium marinum was studied. Leukocyte Migration Inhibition (LMI) and Delayed Hypersensitivity skin reactions to these antigens were examined in 44 hanseniasis patients (20 quiescent Virchowians, 13 reactional Virchowians and 11 tuberculoid patients) and 15 healthy subjects. An impairment in LMI and Delayed Hypersensitivity tests to M. leprae and M. marinum was observed in Virchowians patients both quiescent and reactional. The CMI response to all mycobacterial antigens was increased in tuberculoid patients and was observed a poor response to M. leprae and M. marinum in healthy controls. Our results show a high correlation between the CMI response to M. leprae and to M. marinum (r = + 0,8). This close relationship between both antigens may express cross-reactivity.

Se estudio la inmunidad mediada por celulas (IMC) frente a derivados proteicos purificados de Mycobacterium leprae, Mycobacterium tuberculosis, Mycobacterium avium y Mycobacterium marinum. Se efectuaron tests cutaneos de hipersensibilidad retardada e inhibicion de la migracion de leucocitos in vitro en 44 pacientes hansenianos (20 virchowianos quiescentes, 13 virchowianos reaccionales y 11 tuberculoides) y en 15 testigos sanos. Se observo una alteracion de la inhibicion de la migracion de leucocitos y de la hipersensibilidad cutanea frente a M.leprae y M. marinum en los pacientes virchowianos, quiescentes y reaccionales. La respuesta de la IMC frente a todos los antigenos micobacterianos se encontro aumentada en los pacientes tuberculoides, y se observo una respuesta debil frente a M. leprae y M.marinum en los controles sanos. Los resultados obtenidos muestran una estrecha correlacion entre la IMC frente a M. leprae y M. marinum en todos los casos, la cual podria expresar reactividad cruzada entre ambos antigenos

The phagocytosis and lysis of Candida albicans and Candida pseudotropicalis by human neutrophils were examined in 43 hanseniasis patients (19 quiescent virchowians, 12 reactional virchowians and 12 tuberculoid patients) and 15 healthy subjects. The purpose of this study was to determine whether neutrophils from hanseniasis patients were altered in their enzyme systems myeloperoxidase-dependent (tested by lysin of Candida albicans) and myeloperoxidase-independent (tested by lysin of Candida pseudotropicalis). The results indicate that polymorphonuclear neutrophils of hanseniasis patients had a similar candidacidal activity that human normal neutrophils (p greater than 0.2) in their myeloperoxidase-dependent and myeloperoxidase-independent systems. The enzymatic system activity is also similar in all clinical forms of hanseniasis (p greater than 0.3).

Se estudio la fagocitosis y lisis de Candida albicans y Candida pseudotropicalis por polimorfonucleares neutrofilos, en 43 pacientes hansenianos. (19 virchowianos quiescentes, 12 virchowianos reaccionales y 12 tuberculoides quiescentes) y 15 testigos sanos. El proposito de este estudio fue determinar si los polimorfonucleares neutrofilos de pacientes hansenianos presentaban alteraciones de sus sistemas enzimaticos mieloperoxidasa-dependientes (investigado mediante lisis de Candida albicans) y mieloperoxidasa-independientes (investigado mediante lisis de Candida pseudotropicalis). Los resultados indican que los neutrofilos de los pacientes hansenianos tienen una actividad candidicida similar a la de los neutrofilos de testigos sanos, (p > 0.2) en sus sistemas mieloperoxidasa dependiente e independiente.La actividad de estos sistemas enzimaticos es tambien similar en todas las formas clinicas de hanseniasis (p > 0.3)

RESUMEN

The LDH isoenzymes levels in fresh serum of seven patients affected by different forms of hanseniasis from Argentina, were measured by acrylamide electrophoresis. In all cases the LDH-4 fraction was found elevated; on the contrary, LDH-2 was reduced. The LDH-5 fraction was detected in only three serums, all of them significantly increased when compared with controls.

The liver function was studied in 100 hanseniasis patients. They were classified in the following 10 groups: quiescent LL (20 patients), quiescent BL (13 patients), quiescent BB (7 patients), quiescent BT (9 patients), quiescent TT (12 patients), reactional LL (12 patients), reactional BL (10 patients), reactional BB (7 patients), reactional BT (5 patients), reactional TT (5 patients); a comparison was made with a control group (10 healthy individuals). It was found a hyperproteinemia with high levels of globulin and normal levels of albumin in the serum of all clinical forms, and a typical pattern of "light damage hepatic cells" with raised in the enzymatic activity of GOT, GPT and alkaline phosphatase with no changes in turbidity tests and bilirrubinemia, in all reactional patients. The physiopathology of this problem and the types III and IV hypersensitivity phenomena are discussed.

Cell migration inhibition of white blood cells in the presence of total protein lepromin (TPL) was studied in ten lepromatous patients, six tuberculoid patients, and ten normal controls; adding normal, tuberculoid, lepromatous, or no serum to the culture medium. Using normal or no serum, lepromatous patients and skin negative controls gave negative reactions, while tuberculoid patients and skin positive controls gave positive cell migration inhibitions. The addition of lepromatous serum gave a very significant overall increase of migration indices in all groups of subjects, turning to negative the positive reactions of lepromatous patients and positive controls. On the contrary, the addition of tuberculoid serum gave a decrease of migration index in all groups of subjects, turning to positive the reactions in lepromatous patients. The significance of these circulating factors, able to enhance or inhibit cell migration inhibition responses in patients and controls, is discussed.
