RESUMEN

This article presents a clinical case of urgent, life-saving surgical intervention in a 69-year-old woman with left atrial myxoma with rapid morphological and clinical progression and a history of COVID-19 and breast cancer in remission. However, the concurrent (perhaps secondary) thrombophilic condition facilitated the complication development in the form of superior vena caval orifice thrombosis in the early postoperative period. For this complication, repeated surgery in the volume of thrombectomy was performed, which resulted in stabilization of the patient's condition.

RESUMEN

The paper presents a detailed clinical, hematological, morphological, ultrastructural and immunological characterisation of T-cell lymphosarcoma with prolymphocytic-lymphocytic leukemic transformation (PLLT). In PLLT the proportion of T-cell immunological subvariant of lymphosarcoma came to 15% being detected only in 8 out of 52 examinees. The patients (6 males and 2 females) varied in age from 24 to 76 years (median 49 years) and had the following histological forms of primary tumor tissue: lymphoblastic lymphosarcoma (n = 3), pleiomorphic small cell lymphosarcoma (n = 1), large-cell anaplastic lymphosarcoma (n = 1), prolymphocytic lymphosarcoma. Immunological characteristics of these 8 cases were heterogeneous: in lymphoblastic variant there was immature T-immunological phenotype. In pleomorphic small-cell lymphosarcoma there were also signs of T-cell activation. In large-cell anaplastic lymphosarcoma an immunological phenotype of lymphoid cells from the primary tumor tissue and bone marrow differed in more advanced immunological differentiation of bone marrow tumor cells. In prolymphocytic variant tumor cells had features of T-helpers or T-suppressors. Most of the patients received polychemotherapy according to the schemes for high-grade lymphosarcomas despite PLLT though the latter is not a universal indicator of late tumor progression, more aggressive course of the disease and poor prognosis.

This is the first presentation of information on the circulation of human herpes virus type 6 (HHV-6) in the population of the mid-European part of this country. The results of the study indicate that HHV-6 is widely spread both among apparently normal people and among patients with different pathologies. The studies using immunofluorescence test showed that the percentage of HHV-6-infected subjects was practically similar among blood donors, patients with nasopharyngeal cancer, rheumatoid arthritis, hemophilia, and patients with transplanted kidney, varying from 34% to 49%. A significantly higher per cent (66%) and higher antibody titres to HHV-6 were detected in sera from patients with hemoblastoses. A high level of HHV-6 infection among patients with hemoblastoses was also confirmed by the results of immunoblotting showing a wider spectrum and higher intensity of the detectable proteins. It should be noted that the amounts of virus-specific proteins detectable by some sera showed individual variations indicating different levels of expression of viral proteins in different individuals.

RESUMEN

Therapeutic-prophylactic measures in 60 patients were directed to remove the main pathogenetic mechanisms of development of lung emphysema (control of the obstructive syndrome, normalization of the sputum rheology, reduction of the protease level, restoration of the surfactant system activity). The efficacy of complex treatment (broncholytic agents, mucus regulators, protease inhibitors, etc.) depended largely on the necessity of active prophylactic medical care of patients with dust-induced bronchitis at early stages of the disease.

RESUMEN

The study of surfactant effects and their biochemical composition in the expiration condensate covered 102 patients with dust bronchitis (DB). Decrease of surfactant effects and violation of the proportion of surfactant lipid fractions (increase in the percentage of free fat acids, triacylglyceride decrease) were noted along with the growth of DB severity and clearly defined pulmonary emphysema.
