BACKGROUND: Studies focusing on Patient-Therapist sexual relations have been carried out mainly in the U.S. This study comes to broadly explore this phenomenon in Israel. METHOD: An indirect questionnaire was sent to all 1,817 psychiatrists, psychologists and social workers who are members of the Israeli Psychiatric Association, Psychologists Association and Psychotherapy Association, respectively. RESULTS: 29% of the responding therapists reported that at least one of their patients had experienced sexual relations with the most recent, former therapist. LIMITATION: The design of our survey made it impossible for us to conduct a follow-up after termination of therapy and to differentiate intercourse from nonintercourse sexual exploitation. CONCLUSIONS: Similar numbers of the gender of the exploiting therapist and exploited patients were obtained for both the indirect current Israeli study and previous U.S. studies. This may suggest that such parameters are influenced by common norms regarding the relationships between men and women of both societies, and from the psychotherapeutic dyadic situation.

For years the subject of mental health has been neglected in Israel, and reform of mental health services is now of paramount importance. Psychiatric medicine has altered considerably over the years, and emphasis is shifting from treatment in mental health institutions to treatment at the community level. This transition is the result of the awakening of groups in our society advocating civil rights for the mentally ill and their integration into the community. This process is also bolstered by the advent of new anti-psychotic drugs. However, the social and medical infrastructure set up to deal with these issues has been found lacking. Over the past few years the Minister of Health has appointed a number of committees to address this issue, and they have all recommended extensive reform of mental health services in Israel. The recommendations handed down by the committees are for: (1) Restructure of mental health services, with emphasis on community services and gradual reduction of psychiatric beds; (2) Allocation of additional funding specifically ear-marked for the mentally challenged, enabling transfer of stabilized patients out of the hospital setting and often lengthy and unnecessary hospitalization, into community rehabilitation centers; (3) Transfer of responsibility for health insurance for mentally ill people from the State to the Health Funds, enabling integration of psychiatric treatment into the general treatment framework. The reform has already been initiated. This body of work will review the stages, processes and the difficulties that preceded the reform.

UNLABELLED: In 1991 the authority of psychiatric committees responsible for judicial decisions concerning mentally ill individuals hospitalized compulsorily was greatly extended. The committees' work procedures have gradually been established despite lack of binding rules and regulations, often evolving from the dynamics of the field. METHODS: The study population included 92 hospitalized patients. The examination was performed by administering a questionnaire, reading a protocol of the latest committee hearing and perusing the patient record. RESULTS: The committee tends to decide gradually about discharge from hospitalization and sometimes to include it as part of a rehabilitation process. The reasoning leading to discharge is generally from the field of law. CONCLUSIONS: Tension between medical and legal approaches often underlies psychiatric committee work. Generally, the committee tends to accept the position of the treating physicians. This may indicate that the two approaches are drawing closer to one another and that they have learned to work together cooperatively and fruitfully.

In this study, the authors examined the language of 43 participants with chronic schizophrenia under the basic assumption that a paradigmatic shift is needed in the methodology used to investigate the language of schizophrenia. The pragmatic protocol (C. Prutting and D. Kirchner, 1987) was chosen as the method of analysis to attain a general profile of pragmatic abilities. The results showed that the participants with schizophrenia exhibited a high degree of inappropriate pragmatic abilities compared to participants with mixed anxiety-depression disorder and participants with hemispheric brain damage, as previously assessed by Prutting and Kirchner. Statistical methods for clustering analysis yielded 5 distinct parameter clusters: Topic, Speech Acts, Turn-Taking, Lexical, and Nonverbal. Group clustering analysis of the 43 participants with schizophrenia produced 3 distinct groups with different profiles: minimal impairment, lexical impairment, and interactional impairment. The results are discussed in terms of theoretical implications in the area of pragmatics, the diagnosis of schizophrenia, and other goals.

RESUMEN

The purpose of this article is to review the latest developments in the area of rehabilitation of disabled mentally ill patients. Rehabilitation is a comprehensive and multi-disciplinary treatment. It involves a wide variety of interventions. These interventions are designed to enable the client to integrate into the normal life of the community and to improve his quality of life. This review discusses the different approaches and their effectiveness. The approaches that are described include comprehensive psychiatric, psycho-social and psycho-educational interventions including family therapy, training in social skills, and vocational intervention in programs and services in the community. The approaches that have been shown to be the most effective in controlled studies include: follow-up psychiatric drug treatment, individual and family psycho-educational treatment; social skills training; community sheltered-housing facilities and vocational rehabilitation programs. It has been emphasized that the effectiveness of treatment is conditional not only on the client's resilience and a comprehensive and multidisciplinary treatment approach, but also on the community financial resources and the level of training of the professionals.

BACKGROUND: Patient satisfaction with medical treatment is important, and although in mental health the literature on the topic is scant, awareness of the issue is rapidly growing. AIM: Evaluation of patient satisfaction among hospitalized psychiatric patients. METHODS: 100 consenting patients (47 men and 53 women) were surveyed and completed a questionnaire regarding satisfaction with psychiatric care. SETTING: Fifty-nine percent of the patients were in a new modern building, 41% were in an older hospital building; 21% were in a psycho-geriatric ward. The various settings allowed comparison across different environments. RESULTS: Most participants expressed mild general satisfaction. The overall rating was 3.40 +/- 0.43 (range 1-5). The ratings were similar regarding the various components of treatment: satisfaction with the nursing staff, physical conditions on the ward and treatment per se. Positive correlation was found between satisfaction with nursing and overall ratings. CONCLUSION: Inpatients were mildly satisfied with treatment. However, no correlation was found between satisfaction and either the type of case manager or the physical conditions in the ward. Further research is needed to characterize specific variables that may influence patient satisfaction with psychiatric care. In addition, the question of whether self-report evaluation questionnaires are appropriate for the inpatient psychiatric population should be addressed.

This study compared the manifest dream content of 20 schizophrenic adolescent inpatients whose medications were stable for at least four weeks, 21 adolescent inpatients with other mental disorders (nonschizophrenic group) matched for age and gender, and 31 matched community controls. All participants were administered the standardized Formal Dream Content Rating Scale (FDCRS), which evaluates dream-related anxiety, cognitive disturbance, implausibility, involvement, primitivity, and recall, as well as two additional scales measuring emotional expression and duration of dream report. The Positive and Negative Symptoms Scale (PANSS) was administered to the two inpatient groups. The community controls demonstrated more involvement and emotional expression than the schizophrenic patients; furthermore, they demonstrated more implausibility and had a greater duration of dream report compared with the nonschizophrenic group. In the schizophrenic patients only, elevated scores on the negative subscale of the PANSS were significantly correlated with lower scores on involvement, emotional expression, and dream recall. No relationship was found between the positive subscale of the PANSS and any of the FDCRS subscales. These results suggest that psychopathology per se, rather than the specific psychiatric disturbance, may be associated with impoverishment of dream content, and that negative, rather than positive, schizophrenic symptomatology may be influential in the dream content of schizophrenic youngsters.
