RESUMEN

OBJECTIVE: To test whether the cortisol awakening response (CAR) could be a biomarker for cognitive decline during electroconvulsive therapy (ECT). METHODS: We studied 50 older patients with depression who were treated with ECT from the MODECT cohort. We used linear regression analyses to examine the association between CAR and cognitive change, assessed by the change in Mini Mental State Examination scores between baseline and 1 week after ECT course. CAR was assessed by the area under the curve of cortisol levels, according to Pruessner's-formula. Associations were adjusted for putative confounders, based on previous literature and availability. RESULTS: We found no significant associations between the CAR and cognitive change during the ECT course in (un)adjusted models. CONCLUSION: Our results indicate that the CAR is not usable as a biomarker for ECT-induced cognitive decline during ECT course. Further research in cohorts with larger samples is needed.

RESUMEN

The traditional veterinary curriculum has little room for business courses. Yet the success of many veterinary practice owners depends on their knowledge and application of fundamental business skills and principles. This article reports on a veterinary business systems curriculum that has been implemented at Iowa State University to address the business and life skills needs of veterinarians.

RESUMEN

In the literature a connection is postulated between maternal condylomata and juvenile laryngeal papillomatosis in their children. We have attempted to shed light on the connection partly by seeking information about the children of 77 mothers with condylomata during delivery, and partly by seeking information about the mothers of nine children with juvenile laryngeal papillomatosis. Our research has not shown a clinical connection between the two diseases.

RESUMEN

In a randomized trial intramuscular ritodrine followed by oral ritodrine treatment and bed rest was compared with placebo and bed rest in the treatment of 99 cases of preterm labor. The ritodrine treatment did not have a statistically significant effect on birth weight, gestational age, or the incidence of low birth weight. However, it did inhibit preterm labor in the initial stage, resulting in a gain of a few days to a few weeks in length of gestation. This gain may be valuable. Where necessary, advantage can be taken of it to transfer the mother before delivery to a more specialized hospital with a neonatal intensive care unit or to administer steroid treatment to promote fetal lung maturation. No serious side effects were recorded. The intramuscular route is recommended because large fluid infusions are avoided and treatment can easily be started before the patient is transported from home to hospital.

RESUMEN

A case is presented of pregnancy complicated by hyperparathyroidism and acute pancreatitis in a 32-year-old gravida II, primipara. The diagnosis of pancreatitis was made during surgery; postoperatively the patient was diagnosed as suffering from hyperparathyroidism, and an adenomatous parathyroid gland was removed. The four previously reported cases of this trilogy are mentioned, and the methods of diagnosis discussed.

RESUMEN

A retrospective study was conducted into 146 hospitalised women with endometrial Arias-Stella phenomenon. Extra-uterine gravidity was recorded from only 24 cases (16 per cent). In 116 patients (79 per cent) the above phenomenon was observed in connection with spontaneous abortion or termination of pregnancy.

In a randomized trial, three alternative schemes of treatment of preterm labour with ritodrine (long infusion, short infusion, and intramuscular injection) were compared with standard treatment by bed rest and sedatives. None of the ritodrine treatments were found to be better in postponing delivery than the standard treatment. However, the Bishop scores of the patients in the standard treatment group tended to be lower than of the patients treated with ritodrine. Moreover, all cases of congenital malformation and polyhydramnios occurred in the patients treated with ritodrine. The incidence of respiratory insufficiency was significantly higher in infants of mothers treated with ritodrine. The mean birth weight was highest in the standard treatment group.
