RESUMEN

The authors of the study demonstrate that Helicobacter pylori (HP) strains with the cytoxin-associated protein Cag A are more frequent in patients suffering from duodenal ulcer (DU) or lymphocyte gastritis. DU is more severe and often displays a recurrent character in cases with persistent Cag A-positive HP strains. The authors studied the sensitivity of HP strains with the cytoxin-associated protein Cag A to various antibacterial preparations. The study shows that resistance of HP to metronidazole is found mostly among Cag A strains (58.5% of cases).

RESUMEN

The aim of the study was to determine serum insulin level in patients with metabolic syndrome (MS), pronounced manifestation of cholelithiasis (CL), and abdominal obesity. The measurements were compared with hyperlipidemia and glycemia levels in these patients. The subjects were 55 women with MS, divided into two groups according to 1991 Kh. Kh. Mansurov's classification of CL stages. Insulin level was measured by immune-enzyme assay; carbohydrate and lipid metabolism (lipid levels, lipoprotein and fatty acid spectrums, malonic dialdehyde and catalase levels)--by conventional methods. The study revealed a correlation between high insulin level in patients with hyperglycemia, disorder of tolerance to glucose, and type II diabetes, on the one hand, and lipid peroxidation level and dislipidemia, on the other. The study also found high insulin level at various stages of CL, especially during the period of concrement formation. Insulin is a CL risk factor, i.e. it elevates the risk of nucleation.

Daily urine levels of PGE2 and PGF2 alpha as well as water-electrolyte exchange were measured in 72 patients with chronic active hepatitis (CAH) and hepatic cirrhosis (HC) in comparison with healthy controls. Furosemide treatment was performed in 26 HC patients. It was established that in CAH and compensated HC daily excretion of PGE2 and PGF2 alpha as well as 22Na elimination were within the range registered in the control. In association of HC with nonrefractory ascites the above excretion was elevated, with refractory one daily excretion of PGE2 was low, the urine ratio PGF2 alpha/PGE2 rose. Daily urine excretion of prostaglandins and PGF2 alpha/PGE2 value may be of prognostic significance in evaluating efficacy of diuretic therapy in HC patients with ascites.

The relationship between adrenocorticotropic hormone as well as renin and potassium activity and blood aldosterone secretion was examined in normal subjects and patients with chronic hepatic diseases. It is demonstrated that aldosterone stimulation is controlled by simultaneous renin-angiotensin (RA) and hypothalamo-adenopituitary effects in normal subjects and patients with chronic active hepatitis (CAH), the RA effects prevailing in normal subjects, and hypothalamo-adenopituitary ones, in CAH patients. Cirrhosis of the liver was associated with the greatest deviations in the stimulant-aldosterone relationship. Viral cirrhosis with ascites featured a considerable RA increase, affecting the adrenals, while the contribution of ACTH was reduced considerably. ACTH level was the highest in patients with alcoholic cirrhosis.

The activity of plasma renin, concentration of serum aldosterone and plasma corticotropin were determined by a radioimmunoassay in 78 patients with diffuse toxic goiter with thyrotoxicosis of various degrees of gravity, im 21 patients with primary hypothyroidism and 25 controls in euthyroid condition. In the patients with thyrotoxicosis such investigations were conducted before and after a course of drug therapy. In thyrotoxicosis the activity of the renin-angiotensin-aldosterone system was raised, in hypothyroidism it was lowered, a degree of expression of appropriate changes being associated with the gravity of disease. In the patients with marked thyrotoxicosis after a course of drug therapy the indices of plasma renin activity and serum aldosterone concentration did not return to normal. Hypophyseal corticotropic function was raised in marked thyrotoxicosis, mild hypothyroidism and corresponded to the normal level in the patients with hypothyroidism of average gravity. Plasma ACTH concentration in the patients with marked thyrotoxicosis returned to normal after a course of drug therapy.
