RESUMEN

The work was aimed to estimate the interrelation and indexes which define the main alterations of hemostatic deviations in the patients with abdominal bleeding. For this reason the authors have analysed interrelations between 31 hemostasis indexes in 30 patients after operations. Fourteen groups of interrelated indications have been revealed. They are as follows: 1. Prothrombin index, activated partial thromboplastin index; 2. Prothrombin's inactive forms (prethrombin-1, decarboxylated prethrombin etc.), ecamulin index; 3. Velocity bend of distraction of fibrin clot, velocity bend of forming of fibrin clot, height of clot using turbidimetric method; 4. Half-lysis time, lysis time, time of fibrin clot using turbidimetric method; 5. Velocity, Intensity, remainder of platelet aggregation induced by ADP 10 mcg/ml, 2.5 mcg/ml, 1.9 mcg/ml; 6. Thrombin time; 7. Protein C; 8. Fibrinogen; 9. Platelet count; 10. Soluble fibrin; 11. Ancystron time; 12. X factor; 13. Antithrombin-III; 14. Fibrin(ogen) degradation products. It was shown how different groups affect hemostasis. The authors have suggested to use the data of mathematical analysis and laboratory tests for the estimation of hemostatic deviations.

RESUMEN

The three groups of patients who had morphine hydrochloride, phentanyl and dipidolor been used for intravenous autoanalgesia were examined. In each group, the state of the central hemodynamics, pulmonary ventilation and gas composition of the blood were studied. The effectiveness of analgesia was assessed, side effects were considered. The effectiveness of the use for postoperative intravenous autoanalgesia of morphine and phentanyl has been proved.

It has been established that in conditions of intraoperative blood and plasma loss base deficiency is determined not only by hypocarbonatemia, but also by hypoproteinemia, hypophosphatemia and HCO3 metabolism disturbances caused by anemia. Correction of metabolic acidosis in such patients should include infusions of NaHCO3, protein preparations, blood, phosphates. Mellemgaard and Astrup's technique presupposes correction of the deficiency of all buffer bases only with NaHCO3, which dramatically increases its dosage. Thus, it is evident that the technique should be revised. The comparison of the results of metabolic acidosis correction using a conventional and adapted techniques (hydrocarbonate dose in mmol or ml of a 8.4% solution is 24-SB.body weight.0.2%) in statistically homogeneous groups has shown that differentiated "polybuffer" correction of metabolic acidosis with adapted NaHCO3 dose 1.7 times more frequently normalized acid-base balance parameters, reducing the risk of the onset of post-correction metabolic alkalosis to minimum.

RESUMEN

The indications and regimens of spontaneous respiration with positive pressure at the end of expiration (SR with PPEE) in patients with morbid obesity are not established. The individual selection of resistance at expiration is necessary. In 53 patients, the selected resistance at expiration has proved to be the optimal, contributing to significant improvement of parameters of pulmonary ventilation without deterioration of hemodynamic indices. An inverse dependence of the values of optimal resistance at expiration and excess in ideal body mass was established. By multiplying the values mentioned, the K coefficient is obtained equal to 530.3 + 7.66. In patients over 60, K was 400.3 +/- 11.52. The use of a nomogram (in numerical and graphic versions) to dose resistance at expiration in patients with concomitant morbid obesity contributed to significant improvement of the indices of pulmonary ventilation and gas exchange without disorders in hemodynamics after seances of SR with PPEE.

RESUMEN

In 30 patients with IIA-IIB stage hypertension operated on for cholelithiasis, gastric and duodenal ulcer disease and other diseases, clophelinum was used as a component of total intravenous anesthesia. The different methods of its administration were assessed. The optimal variant of hemodynamic stabilization was noted in programmed clophelinum administration by means of infusator "Vita-beta" at a dose of 0.75 g/(kg.hr).

RESUMEN

Gel electrophoresis was used to study the blood serum of 13 patients with insulinomas who showed marked dysproteinemia in the preoperative period. Enucleation of the tumour or resection of the pancreas were performed depending on the location of the tumours. In the immediate postoperative period dysproteinemia became more pronounced, then a tendency to normalization of the protein spectrum occurred. Dysproteinemia was most pronounced at the zone of the starting peak where insulin-binding proteins are located.

RESUMEN

Enteral administration of pancreatic juice in 17 patients after pancreatoduodenal resection caused a decrease in the activity of trypsin, amylase and lipase, pancreatic lipase and trypsin of the pancreatic juice in the blood. The increase in the level of insulin in the blood was noted. The mentioned method can be used for prevention of the development of postoperative pancreatogenic complications.

RESUMEN

Direct thermometry of the pancreas in 44 patients after operations for carcinoma of the pancreato-duodenal zone or chronic pancreatitis has shown a relationship between temperature of the pancreas stump and the fermentemia degree. Characteristic changes of temperature regimens are observed 5-17 hours earlier than manifestations of clinical symptoms of postoperative pancreatitis make their appearance.
