RESUMEN
The immediate surgical results of 163 consecutive carotid endarterectomies performed in a neurosurgical training center are presented. The mortality rate was 0.6% and the permanent neurological morbidity rate was 2.5%. Several factors in the surgical technique including the use of magnified vision, full heparinization, the liberal use of intimal tacking sutures, and the employment of an indwelling shunt are thought to be responsible in part for the improved surgical results. This report describes a case in which an intracranial histoplasmoma was successfully treated with surgical removal and amphotericin B. This is the third reported case of its kind. The authors discuss problems of preoperative diagnosis in a patient with depressed cell-mediated immunity, and no evidence of extracerebral dissemination.

Two children are presented with arteriovenous malformations of the upper brain stem, emphasizing the clinical presentation, diagnosis, and surgical therapy. The lesions were located on the pia mater, facilitating total surgical removal. The importance of detailed angiography with subtraction technique is discussed.
