RESUMEN

Abstract The prevalence of pregnancy in adolescent women is high in Mexico and represents a public health problem. The pregnant teenager with heart disease has a high probability of complications during pregnancy and the delivery, which carries a risk of death of both the mother and the product. In many cases the pregnancy should have been avoided, planned or interrupted, however the majority at this age is vulnerable and although certain cases must be interrupted by their high risk of maternal-fetal death, prevention and legal aspects should be considered. In some cases the woman wants a pregnancy although her health condition does not allow it, but there are options of adoption or recourse to a surrogate belly. In response to this growing social problem, the National Cardiology Institute Ignacio Chávez and National Institute of Perinatology, with the coordination of Ministry of Health in Mexico, started a pregnancy prevention module within a clinic of follow-up of cardiopathy and pregnancy. This review raises the global problem in our country that occupies the first place in pregnancies in adolescents, with more than 400,000 pregnancies a year and the form of immediate response in a multidisciplinary way.


Resumen La prevalencia de embarazo en mujeres adolescentes es muy alta en México, y representa un problema de salud pública. La adolescente embarazada con cardiopatía tiene altas posibilidades de complicaciones durante el embarazo y su resolución, lo que pone en riesgo la vida tanto de la madre como del producto. En muchos casos el embarazo debió ser evitado, planeado o interrumpido, sin embargo la mayoría a esta edad es vulnerable y si bien ciertos casos deben ser interrumpidos por su alto riesgo de muerte materno-fetal, es fundamental considerar la prevención y los aspectos legales. En algunos casos la mujer desea un embarazo aunque su condición de salud no se lo permite, pero existen opciones de adopción o recurrir a un vientre subrogado. Atendiendo este problema social cada vez más creciente, el Instituto Nacional de Cardiología Ignacio Chávez, en coordinación con la Comisión Coordinadora de la Secretaría de Salud y el Instituto Nacional de Perinatología, echaron a andar un módulo de prevención de embarazo dentro de una clínica de seguimiento de cardiopatía y embarazo. Esta revisión plantea el problema global en nuestro país, que ocupa el primer lugar en embarazos en adolescentes, con más de 400 mil embarazos al año y la forma de dar respuesta inmediata de manera multidisciplinaria.

The aim of this study was to compare the post-thaw distribution of motile sperm subpopulations, following simple or colloid centrifugation. A new analysis was used to evaluate the available number of sperm from each subpopulation after each centrifugation protocol. Frozen/thawed semen samples were divided into the following after-thawing treatments: uncentrifuged control (UDC), sperm washing (SW) and two colloid centrifugation procedures (Equipure, SLC-E, and Androcoll, SLC-A). Percentage of total and progressive motility (TM and PM), as well as sperm motility kinematics, distribution of motile sperm subpopulations, and recovery rates, were statistically compared among treatments. The SLC treatments showed higher (P < 0.001) TM and PM than UDC and SW. Following each SLC procedure, different percentages of the subpopulation with the most vigorous and progressive sperm (sP4) were obtained. SLC-A recovered a larger number of sperm belonging to sP4 than SLC-E, but not significantly higher than SW. From a practical point of view, sperm washing, the standard centrifugation procedure for equine semen processing, recovered the same amount of fast and progressive sperm as colloid centrifugation, apparently the best treatment according to traditional analysis. In conclusion, samples processed by SLC have higher motility percentages than SW and UDC but, after combining the available number of sperm, SLC and SW techniques are equally efficient in recovering sperm from the most vigorous, fast and progressive motile subpopulation (sP4).

RESUMEN

The scientific research goals developed and applied beyond their own frontiers, by the National Institutes of Health and High Specialty Hospitals, are presented regarding the specialized medical care of excellence on the field of global health, in benefice of the Mexican population. Maternal mortality as well as malnutrition, and the lack of micronutrients in the pregnant mother and child, put them in disadvantage to face future physical and mental diseases. Low weight at birth, prematurity, and overweight during childhood are factors that can trigger the the development of the metabolic syndrome, with all its consequences on physical and mental health. These challenges are faced with a preventive vision by means of pilot programs like "Towards a new generation of Mexicans" and "Chronic kidney disease stratification in high risk patients", besides the development of new strategies for the prevention and treatment of other chronic diseases such as cancer, hypertension, and diabetes among others by decreasing the number of overweight and obesity people in the Mexican population.

RESUMEN

Introducción. La sobrepoblación de los servicios médicos ha incrementado la necesidad de referir pacientes a otras unidades médicas; en esas situaciones la nota de envío representa frecuentemente la vía de comunicación entre las unidades médicas. Objetivo: evaluar con qué frecuencia las notas de envío incluyen la información requerida así como la asociación entre integridad de éstas y el tipo de formato estructurado para elaborarlas. Material y métodos. Se colectaron y evaluaron las notas de envío de pacientes referidos al Hospital Infantil de México Federico Gómez de acuerdo a las recomendaciones propuestas por la Secretaría de Salud. A cada nota se le evaluó un total de 12 apartados. Se correlacionó la información incluida de cada categoría con la presencia de un espacio especialmente reservado para esa categoría de información en el formato utilizado.Resultados. Se evaluaron 100 notas de envío de pacientes atendidos desde el 15 de noviembre de 1997 al 15 de julio de 1998. En la mayoría de las notas evaluadas no se incluían ni la información ni los apartados para cada uno los puntos relevantes. Ninguna nota incluía toda la información requerida. Sólo en 4 de los 12 puntos evaluados se pudo demostrar asociación entre la inclusión de un espacio en el formato con la inclusión de dicha información.Conclusión. Las notas de envío frecuentemente no incluyen información referente a cada uno de los reactivos que se consideran necesarios. El uso de un formato que incluya apartados para cada tipo de información pudiera mejorar la integridad de las notas de envío.
