RESUMEN

314 alimentary tract cancers were discovered in 219 black males and 95 black females at the occasion of 18.000 endoscopies (high or low) performed during the 5 past years at the Principal Hospital (Dakar-Senegal). Mean age of the patients was 50, gastric cancer (170 cases) was the most frequent localization, then rectocolonic one (71 cases) and oesophageal one (61 cases). The more frequent utilization of endoscopy and may be a modification of diet in urban milieu might explain such a high frequency of discovery of alimentary tract cancers in Dakar.

RESUMEN

On the basis of 10 cases (8 cases of rectosigmoid cancer and 2 of sigmoid volvulus), the authors report about their experience in the restoration of GI tract continuity using colorectal intubation into the rectal lumen and a transproctoanal anastomosis. This technique helps avoiding subperitoneal sutures, which are a cause of fistulae and shrinkage. It is simple enough to be performed by younger surgeons. This technique was initially developed for anterior resections for rectosigmoid cancer, and it can be utilized as an emergency procedure with single-piece resection without untwisting the sphacelous sigmoid loops, so that neither a temporary left iliostomy nor a second operation are needed.

RESUMEN

314 bowel cancers were discovered in 219 men and 95 women of negroid race in the course of 18,000 upper and lower endoscopies taken over the past 5 years at Dakar's Main Hospital. The average age of the sick persons is 50 years. Gastric cancer (170 cases) is the most frequent, followed by recto-colic cancer (71 cases) and oesophagian cancer (61 cases). The use of endoscopy and perhaps dietetic changes in the urban area, explain the large number of bowel cancers discovered in Dakar.

RESUMEN

The authors report two cases of Peritoneum Gelatinous Sickness of appendicular origin, occurring in male adults aged 65 and 57. Laparoscopy practised on an extended abdomen showing non-homogeneous dullness confirmed the diagnosis in both cases. The laparotomy highlighted the gelatinous substance and led to appendicectomy aimed at the mucocele. They take this opportunity to discuss the unusualness of this affliction in Africa, and recall the clinical and etiological means and the diagnostical difficulties that this disease evokes.

RESUMEN

Obstetrical hysterorrhexis, accident that has been described for a long time, is still of special concern in rural villages where sufficient health care facilities are not available. This accident is always serious, leading to the death of the newborn baby and, too frequently, to the death of the mother. A number, however, could be avoided with a better training of the paramedical staff, so that neither inadequate actions nor uncontrolled use of oxytocics be done. The author advocates to perform a systematic examination of the uterus after every difficult or outlasting delivery or having required either manual or instrumental help. That remains the best way to save the uterus of the mother and sometimes her life itself.

RESUMEN

The influence of the parasitic diseases on pregnancy and the influence of pregnancy on the parasitic diseases are both well known concepts. The authors lay special interest on malaria, amebiasis and hookworm disease by giving a description of their effects and by mentioning the risks they may entail; then, they give practical directions intended for their detection and the treatment of their effects. It is, once again, emphasized on the utmost importance of performing antenatal examinations.

RESUMEN

Three symptoms define the renovascular syndromes during pregnancy: hypertension, edemas and proteinuria. It is essential to detect them as soon as possible during the antepartal examinations. The directions for treatment consist in simple acts which are easy to perform in overseas conditions. They intend to prevent very serious disorders such as abruptio placentae and eclampsia which should not be found any longer.

RESUMEN

Abruptio placentae is a rather frequent syndrome in overseas countries. Leading to foetus death most of the time, it often endangers prognosis for the mother's life. Serious complications may be found such as shock due to hypovolaemia, coagulopathy among which process of disseminated intravascular coagulation prevail, organic renal insufficiency that may become unrecoverable because of the cortical atrophy of the kidney. The treatment combines obstetrical means with medical ones. Spontaneous uterine voiding will be tried for, since caesarian operation, often unavoidable, is not free from complication. Rehydration will be carried out by transfusing fresh blood and unfrozen plasma, the amount of which often enormous has to be dealt out in accordance with the central venous pressure.
