RESUMEN
Background: Several indexes have been developed to define the risk attributable to lipid metabolism with a single value. The total cholesterol/high-density lipoprotein (TC/HDL-C) and low-density lipoprotein/high-density lipoprotein (LDL-C/HDL-C) ratios are the most used. The higher the value of these ratios, the greater the probability of cardiovascular events. Objective: To identify whether the TC/HDL-C and LDL-C/HDL-C ratios are early prognostic markers of mortality and major cardiovascular events in patients with ST-elevation acute coronary syndrome. Material and methods: 265 patients with ST-segment elevation acute coronary ischemic syndrome were included, divided into 4 groups according to the values of the atherogenic indices. Mortality and major cardiovascular events at 30-day follow-up were analyzed. Comparison of the groups was performed using the chi-squared test or ANOVA, depending on the case (p < 0.05). Results: The cut-off point for the TC/HDL-C index was 6.9 and for the LDL-C/HDL-C it was 2.7. The comparative analysis of groups showed that cardiovascular death and arrhythmia were higher in group 3 (p = 0.006 and p = 0.003, respectively). Conclusions: TC/HDL-C and LDL-C/HDL-C indexes can be used as prognostic markers of cardiovascular mortality in the first 30 days of follow-up.

Background: Tracheotomy is a common technique; however, microbiological contamination of the surgical site can increase morbimortality. Up to 90% of patients present a positive culture of the airway. Among the most important related factors is the lack of tracheal cannula replacement and lower airway infections. It is convenient to identify microbiological contamination of surgical site in tracheal secretions samples and the specific microorganism associated. Objective: To determine the factors related to microbiological contamination of surgical site. Material and methods: A prospective cohort study which included patients undergoing tracheotomy was carried out. Tracheal secretion was sampled by direct swabbing for culture during surgery and from the surgical site 5 days after. Results: The initial report showed contamination of samples in 58.3%, and 5 days after in 80.6%, with an incidence of contamination of 22.3%. Initially the main agents identified were Pseudomonas aeruginosa in 13.9% of the cultures, Klebsiella pneumoniae in 11.1% and Enterobacter spp. in 11%. On day 5, the most common agents were Klebsiella pneumoniae in 25% of the cases, Acinetobacter baumannii in 11.1% and Pseudomonas aeruginosa in 11.1. Conclusions: The frequency of microbiological contamination is high. The main agents were Staphylococcus aureus, Klebsiella pneumoniae and Pseudomonas aeruginosa. No risk factors for the presence of post-tracheotomy contamination were identified.

Background: In the emergency room, the shock index was applied to determine the prognosis in various pathologies, such as acute infarction. The shock index is the re-sult of dividing heart rate by the systolic blood pressure. Objective: To determine the relationship between the systolic shock index and the di-astolic shock index as prognostic factors for mortality in acute myocardial infarction with ST segment elevation prior to admission to the Hemodynamics Room. Material and methods: A prolective analytical cross-sectional study was carried out in patients who were admitted to the Hemodynamics Room for cardiac catheterization within a period of 5 months in 2022. The systolic shock index and diastolic shock index were included as independent variables and mortality was the dependent variable. SPSS, version 25, was used and Pearson's chi-square test was used as statistical test, with a p value < 0.05 being significant. Results: 162 patients were analyzed and showed a diastolic shock index > 1.2143 with a sensitivity of 62.5% and a specificity of 77.4% with a p < 0.05. Their systolic shock in-dex was > 0.8908 with a sensitivity of 45.8% and specificity of 91.2%, with a p < 0.05. Conclusion: It was determined that both shock indices are related as mortality prognos-tic factors in acute myocardial infarction with ST elevation prior to admission to the He-modynamic Room.

Introduction: Depression is a mental health disorder characterized by the presence of sadness or loss of the ability to feel pleasure, with a high incidence in patients with COVID 19. The investigations have focused on patient care and little on the care of health personnel, these being the ones with the highest mortality rate, so the objective of the study was to investigate the prevalence of depression symptoms and suicide risk and understand the association of depressive disorder and suicide risk with levels of serum cholesterol and low levels of serum cortisol among internal medicine fellows in a specialist medical hospital in Leon, Guanajuato, Mexico, before and after COVID-19. Methods: In this longitudinal study, internal medicine residents were initially monitored for 2months before starting to care for patients with COVID-19. Participants were asked to fill out depression symptoms and suicide risk surveys. We measured the serum cholesterol and cortisol of each participant, and again after 11months of treating COVID-19 patients. Results: Depression symptoms and suicide risk were assessed; significant differences were found between the two time periods for depression (p < 0.01), and no difference was found for suicide risk (p = 0.182). We found a significant correlation between serum cholesterol levels and suicide risk (r = 0.366, p < 0.01); we also found differences in serum cortisol levels (p < 0.01) and cholesterol (p < 0.0001) before and after the pandemic. Conclusion: Caring for patients with COVID-19 in the hospital contributed to an increase in levels of depression symptoms and suicidal ideation, as well as differences in levels of cortisol and cholesterol in resident physicians of internal medicine; among the possible reasons for this change could be the conditions of personal protection while treating patients, the uncertainty in the first months of not knowing how the virus was transmitted and not having or knowing when vaccinations would be available, as well as the lack of a strategy of adequate mental health support from the institutions dedicated to their academic training.
Abstract Objective: To identify the differences (in men and women) and the relationship between emotions and coping strategies in university students when they fail an exam at the beginning of the school year. Method: The study was non-experimental and cross-sectional with multiple continuous variables, in which 1,774 Mexican university students participated (54 % women and 46 % men) of mean age 20.72 years (SD = 2.27). Two tools were used. The first one was the "Emotions scale when you fail": displeasure, guilt, shame, sadness, fear, frustration, anger, anguish, and indifference (α = 0.94), and the second tool was "Coping strategies when you fail": yelling, blaming someone else, reflecting, failing the exam, seeking advice, avoiding talking about it, avoiding thinking about it, pretending to be fine, using social media, listening to music, hanging out with friends, going out alone, talking to family (α = 0.88). Results: In the tool "Emotions scale when you fail," significant differences were found between men and women in every factor and its intensity. The most commonly used coping strategies for women (M = 2.96, SD = 0.66) and men (M = 2.69, SD = 0.67) were assertive and self-constructive, and the least commonly used were aggressive-defensive. As for the evasion strategies, although they did not show differences between men (M = 2.00, SD = 0.78) and women (M = 2.11; SD = 0.84) in the general sample correlations, those strategies were the ones that had the highest scores with emotions (r = 0.53 **), followed by the search for solutions (r = 0.38 **) and assertive and self-critical (r = 0.42 **). When performing the contrast analysis among women (r = 50 **) and men (r = 0.56 **), evasion strategies were the ones that showed the highest correlation with emotional factors (r = 0.50 **). Conclusions: Although statistically significant differences were found between men and women in emotion intensity, women being the ones that showed greater intensity, no differences were found in aggressive-defensive coping strategies, evasion, and support seeking. Requesting help is an indicator that women are (1) able to recognize that they have a problem that is currently beyond their abilities to solve independently and (2) able to ask for help in order to solve problems. Knowing which emotions university students experience and the coping strategies they use when they fail, gives the support staff in charge a chance to implement actions that help strengthen them and move forward with their academic journey. It is important to support students, because the coping strategies that showed the highest correlation with emotions were the evasion strategies, which can have further negative consequences such as going from failing an exam to failing the subject.
RESUMEN
OBJECTIVE: This study aimed to evaluate the psychological impacts of COVID-19 prevention measures, such as social isolation, on a Mexican sample. METHODS: We conducted an online sociodemographic and Impact of Event Scale-Revised (IES-R) survey during the second phase of the COVID-19 outbreak in Mexico to evaluate the presence of psychological distress, signs of post-traumatic stress, and to identify the groups at highest risk in the sample. RESULTS: Prevalence of psychological distress at moderate or severe levels in the sample were as follows: 943 (22%) intrusive thoughts, 933 (22.3%) avoidance, and 515 (12.2%) hyperarousal. Furthermore, we found the symptoms of clinically significant post-traumatic stress in 1160 (27.7%) of the participants. The variables positively correlated with higher psychological distress were as follows: age (younger), sex (female), employment (employed), relationship status (single), in social isolation, number of days in isolation, the number of people in the household (3-5), and a perception of a high risk of contracting COVID-19, change in routine, engaging in less activity, and loss of income. CONCLUSION: During phase 2 of the COVID-19 outbreak in Mexico, we observed the presence of psychological distress and post-traumatic stress symptoms in over a quarter of the population. This investigation may guide mental health interventions and policies towards the groups that are most vulnerable to the impacts of the social and lifestyle changes taking place in Mexico due to COVID-19.
RESUMEN
Sensory Processing Sensitivity (SPS) is a genetically-determined trait that allows people to notice external and internal subtleties in sensory stimuli easily. This feature provides certain advantages to those who possess it, such as, greater empathy, artistic tastes or skills, also, this people can get greater benefit in circumstances with little exposure to sensory input. However, an unfavorable feature of the trait is an increased vulnerability to development psychopathology. In Mexico there is not an instrument to assess the presence of the SPS, thus, the purpose of this research was to analyze the psychometric properties of the Highly Sensitive Person Scale (HSP Scale) in Mexican population. The results showed that the adaptation of HSP Scale has adequate psychometric properties, so the essential contribution of this study is to provide a reliable and valid instrument that allows to discriminate the presence of the SPS.
RESUMEN
OBJECTIVES: This work sees to a) adapt and validate the "Stress profile" for the Mexican population, b) determine the proportion of variance that the instrument is able to explain based on the original data of a factorial model, c) group the variables into factors where they saturate most clearly, d) evaluate the validity of the content of the instrument, and e) evaluate the internal consistency of the instrument. METHODS: The validation sample consisted of 883 participants, 58.3% women and 41.7% men, aged between 15 and 76 years (M=21.40, SD=10.02). To verify the psychometric validity of the scale, the procedure proposed by Reyes-Lagunes and García-Barragán (2008) was used. It consists of the following steps: a) frequency, including bias for the selection of reagents; b) discrimination of reagents for extreme groups, having in mind that those that do not comply with the requirement will be eliminated; c) internal reliability; and d) validity. RESULTS: The "Stress Profile" has adequate psychometric characteristics for the Mexican population (Cronbach's alpha of 0.65 to 0.93). CONCLUSION: The "Stress Profile" was adapted and validated for the Mexican population. This is a useful tool for studies that aim to evaluate the factors associated with stress in general, including health habits, coping styles and psychological well-being.
OBJETIVOS: a.

OBJECTIVE: We report an association between depression and glycemic control in patients with type 2 diabetes mellitus (DM-2). MATERIAL AND METHODS: Sixty-five diabetic patients (26 men and 39 women) aged 40-60 years were studied within 5 years of the diagnosis. The patients were assessed using the depression scale validated in Spanish, and serum glucose and glycated hemoglobin (HbA1c) levels were measured. Pearson's correlation was used to identify associations between depression and DM-2 and glycemic control; p < 0.05 was accepted as significant. RESULTS: Sex, age, anthropometric measures, and time since the diagnosis of DM-2 did not differ between patients with and without depression. CONCLUSIONS: Patients with depression had higher fasting glucose and HbA1c levels, and these levels correlated significantly with the depression score.

INTRODUCTION: Blood pressure levels in childhood influence these levels in adulthood, and breastfeeding has been considered such as a cardioprotective. We evaluated the association between blood pressure levels and feeding type in a group of infants. MATERIAL AND METHODS: We conducted a comparative cross-sectional study in term infants with appropriate weight at birth, to compare blood pressure levels in those children with exclusively breastfeeding, mixed-feeding and formula feeding. The comparison of groups was performed using ANOVA and multiple regression analysis was used to identify variables associated with mean arterial blood pressure levels. A p value < 0.05 was considered significant. RESULTS: We included 20 men and 24 women per group. Infant Formula Feeding had higher current weight and weight gain compared with the other two groups (p < 0.05). Systolic, diastolic and mean blood pressure levels, as well as respiratory and heart rate were higher in the groups of exclusively formula feeding and mixed-feeding than in those with exclusively breastfeeding (p < 0.05). Multiple regression analysis identified that variables associated with mean blood pressure levels were current body mass index, weight gain and formula feeding. CONCLUSIONS: Infants in breastfeeding show lower blood pressure, BMI and weight gain.

AIM: To study the esophageal transit time (ETT) and compare its mean value among three anatomical inclinations of the body; and to analyze the correlation of ETT to body mass index (BMI). METHODS: A biomagnetic technique was implemented to perform this study: (1) The transit time of a magnetic marker (MM) through the esophagus was measured using two fluxgate sensors placed over the chest of 14 healthy subjects; (2) the ETT was assessed in three anatomical positions (at upright, fowler, and supine positions; 90 degrees , 45 degrees and 0 degrees , respectively). RESULTS: ANOVA and Tuckey post-hoc tests demonstrated significant differences between ETT mean of the different positions. The ETT means were 5.2 +/- 1.1 s, 6.1 +/- 1.5 s, and 23.6 +/- 9.2 s for 90 degrees , 45 degrees and 0 degrees , respectively. Pearson correlation results were r = -0.716 and P < 0.001 by subjects' anatomical position, and r = -0.024 and P > 0.05 according the subject's BMI. CONCLUSION: We demonstrated that using this biomagnetic technique, it is possible to measure the ETT and the effects of the anatomical position on the ETT.

BACKGROUND/AIMS: evidence of gender-related differences in gastric emptying have been reported in the literature. Usually, those researches have focusing only with solid or liquid meal and invasive techniques. The objective of this study was to know the differences in the half time of gastric emptying and frequency of peristaltic contractions measured with magnetogastrography (MGG) on healthy subjects, using a semi-solid test meal. METHODS: the study was carried out in 16 healthy subjects without gastrointestinal disease history they were divided in two groups, 8 male and 8 female. A test meal composed by 250 ml of yogurt and 3 grs of magnetite (Fe3O4), was employed. RESULTS: the gastric emptying half times were found to be 32.3 +/- 10.8 and 36.0 +/- 6.7 minutes, for men and women, respectively. Magnetogastrography modality presented in this study is a useful technique to measure the gastric emptying and the peristaltic contractions frequency. The studies were performed in healthy subjects without side effects. Using this technique a significant statistical difference (p<0.05) on gastric emptying from healthy volunteers was obtained between men and women. CONCLUSIONS: a contribution of this study was to demonstrate the feasibility of magnetogastrography to analyze gender differences in the gastric emptying half time using a semi-solid test meal.
