RESUMEN

Clear cell chondrosarcoma is a rare histological subtype of chondrosarcoma, usually with a relatively non-aggressive clinical course. However, infrequently they may relapse and metastasize. We describe a case of a male patient, 53 years old, with rib cage metastases of a clear cell chondrosarcoma 11 years after the first surgical intervention, and review the literature.

RESUMEN

The increasing production of oil and gas in the marine environment and the growing participation of companies of different nationalities and sizes require the use of tools to support the sustainable management of offshore oil and gas production units. This paper presents the results of the application of a sustainability indicator system, developed from the identification of the economic, environmental, social, and operational impacts of the activities of these production units. The sustainability performances of 3 oil and gas production units operating in the Brazilian marine environment were compared to the performance of one considered ideal, through the application of the PROMETHEE II and ordinal COPELAND multicriteria methods. The indicator system applied favored the analysis of the sustainability management of offshore oil and gas production activity in a multidimensional approach, considering the points of view of experts from various areas of knowledge, and proved to be a reliable tool to support the sustainable management of these offshore production units. RESUMEN

Leiomyosarcomas of the lower extremity are extremely rare disorders and account for 10-15% of limb soft tissue sarcomas. These tumours have poor prognosis and even in early stages, patients persist at high risk for local and distant relapse; consequently, the treatment of advanced leiomyosarcoma of the lower extremity embodies a substantial defy. We present the case of a 73-year-old man diagnosed with metastatic lower extremity leiomyosarcoma of the hallux soft tissue, and with bone, lung and lymph node metastasis. After core needle biopsy confirmation of high-grade fusocellular sarcoma, the patient underwent surgery of the primary tumour and received anthracycline-based chemotherapy. However, after a 7-month progression-free survival period, a CT revealed lung disease progression. Sequentially, the patient was treated with trabectedin (Yondelis®) at a dose of 1.5 mg/m2 resulting in complete remission of the lung metastasis and stable disease of the remaining lesions after 26 months of treatment. Afterwards, the patient started on maintenance therapy with trabectedin, resulting in long-lasting stable disease, as he was able to receive 94 cycles with very acceptable quality of life. Finally, in March 2019, the patient died of community-acquired pneumonia without objective progression disease. This clinical case reports the first patient ever treated with 94 cycles of trabectedin. Our results additionally confirm that trabectedin wields relevant oncostatic benefits with a manageable safety profile and without cumulative toxicities. Trabectedin properties enable a maintenance long-term therapy (until disease progression or unbearable toxicity), with a high impact on survival and with a preserved quality of life.

It is well known that the salivary flow is reduced by aging but ionic composition changes associated to aging have been less evaluated. To measure salivary and plasmatic [Na(+)], [K(+)] and [Cl(-)] and to correlate with age in healthy, non-medicated subjects of any gender, 165 healthy participating subjects (over 15 years old) were asked to give sample of 5 mL mix basal saliva in a plastic vial without any stimulation technique, additionally, 5 mL of venous blood was collected. Samples [Na(+)] and [K(+)] were measured by flame photometry (Corning™ M-405) and [Cl(-)] by voltametric chlorometry (Corning™ M-920). Ionic concentrations were expressed as (X±DE; meq.L⁻¹). All three ionic concentrations progressively increased with age, with the lineal regression equation being: [Na(+)] mEq=17.76 + 0.26(Age); r=+0.42; F=31.5; P=0.00001; [K(+)] mEq=13.2+0.15(Age); r=+0.32; F=16.5; P=0.00001; [Cl(-)] mEq=9.05+0.18(Age); r=+0.35; F=7.8; P=0.0071. Age induced changes in salivary ionic concentrations were not associated to blood ionic changes. However, saliva and blood [Na(+)] and [K(+)] were correlated (r=+0.25; F=4.49; P=0.04 and r=+0.30; F=6.98; P=0.01, respectively). Significant association was found among salivary ions: [Na(+)] mEq=9.14+0.99[K(+)] (r=+0.79; F=95.2; P=0.000001); [Cl(-)] mEq=0.95+0.56[Na(+)] (r=0.79; F=106.6; P=0.000001) and [Cl(-)] mEq=3.45+0.69[K(+)] (r=0.73; F=72.5; P=0.000001). These results confirm and measure the impact of aging over the mixed and resting salivary secretion process and suggest that local changes are not related to blood ionic composition.

The use of alpha-chymotrypsin to cleave covalently bound N-acetyl- l-tryptophan (Ac-Trp-OH) from the surfaces of aminopropylated controlled pore glass (CPG) and the polymer PEGA 1,900 was investigated. Oligoglycine spacer chains were used to present the covalently attached Ac-Trp-OH substrate to the aqueous enzyme. In the absence of the oligoglycine spacer chain, the rate of release was relatively slow, especially from the PEGA 1,900. These slow rates reflect the position of the amino group to which Ac-Trp-OH is covalently attached. On the glass there was a clear optimum with a chain of four glycine residues. For PEGA 1,900 there is no real apparent change beyond two glycine residues. The decline in rate beyond these optima are a possible result of changes in oligoglycine structure. Comparing different surface loadings of bound substrate the rate of release of Ac-Trp-OH from CPG with a pore diameter of 1,200 A was optimal when using 83% of the maximum that can be coupled, then fell again at higher loading. The rate of Ac-Trp-OH release from CPG was the same for surface coverages of 0.4 and 1.0. The introduction of permanent surface charges on CPG 1,200 exhibits a distinct influence on enzymatic cleavage with an increase in the rate of biocatalysis at the surface. Optimal presentation of covalently immobilized substrate on different supports by use of appropriate linkers leads to favorable biocatalysis from the support.
