RESUMEN

BACKGROUND: Penile squamous cell carcinoma (PSCC) is a rare malignancy with higher incidence in developing countries. Treatment options include surgery, radiation therapy, and systemic chemotherapy. However, effective treatments for advanced disease are lacking. To understand the biology underlying PSCC may help the development of new therapeutic strategies. The objective of this study was to evaluate immunohistochemical expression of programmed death-ligand 1 (PD-L1) and p16 in PSCC and its association with clinicopathologic features and outcomes. PATIENTS AND METHODS: A cohort of 40 patients with PSCC from an academic institution in Brazil was analyzed. Clinicopathologic features and outcomes were retrospectively collected. PD-L1 and p16 immunohistochemical expression were performed in formalin-fixed paraffin-embedded specimens. PD-L1 was positive with any staining in more than 1% of tumor, and p16 was positive in more than 10%. Associations were performed using the Mann-Whitney and Fisher exact test. Kaplan-Meier curves were used to estimate survival rates with log-rank. RESULTS: Of 35 patients, 5 were excluded, 4 owing to a lack of data and 1 owing to no tumor available; 18 (51.4%) patients were PD-L1-positive (PD-L1+). PD-L1+ was associated with larger tumors (P = .027). There was an association between PD-L1+ and p16 expression (P = .002). PD-L1+ was more frequent in grade II and III disease than grade I (77.8% vs. 22.2%) and was expressed in all patients with grade III disease. Lymph node involvement was associated with PD-L1 expression (69.2% PD-L1+ vs. 30.8% PD-L1-negative). The 5-year mortality was 37.1%. CONCLUSION: PD-L1 expression appears to be associated with p16 expression, larger tumors, and worse clinical outcomes in PSCC and may provide clinical data for new studies to evaluate anti-PD-L1 immune therapies.

OBJECTIVE: Identification of risk factors for cardiac surgery can improve surgical results. Our aim is to identify factors related to increased hospital mortality for patients who underwent mechanical cardiac prosthesis implant. METHODS: Prospective study with retrospective data acquirement study including 335 consecutive patients who underwent at least one implant of St. Jude Medical mechanical prosthesis between December 1994 and September 2005 at the Cardiology Institute of RS. Valve implants were 158 (47.1%) in aortic position, 146 (43.6%) in mitral and 31 (9.3%) in aortic and mitral. The following characteristics were analyzed in relation to hospital death: gender, age, body mass index, NYHA functional class, ejection fraction, type of valve lesion, hypertension, diabetes mellitus, serum creatinine, preoperative arrhythmias, prior heart surgery, CABG surgery, concomitant tricuspid valve surgery and operative priority (elective, urgent or emergent). Logistical regression was used to analyze data and odds-ratio was calculated for individual factors. RESULTS: During the follow-up there were 13 (3.88%) deaths. In-hospital mortality risk was associated with serum creatinine (P<0.05), ejection fraction < 30% (P<0.001), mitral valve lesion (P<0.05), concomitant CABG surgery (P<0.01), prior cardiac surgery (P<0.01) and reoperation (P<0.01). Increased odd-ratio were related to previous cardiac surgery (5.36; IC95% 0.94-30.56), combined revascularization (5.28; IC95% 1.51-18.36), valvar reoperation (4.69; IC95% 0.93-23.57) and concomitant tricuspid annuloplasty (3.72; IC95% 0.75-18.30). CONCLUSION: The mortality rate is within the parameters found in the literature, identifying recognized factors which neutralization by changes in surgical indication and medical management may enable risk reduction.

INTRODUÇÃO: A identificação dos fatores de risco pré-operatórios na cirurgia valvar visa à melhoria do resultado cirúrgico por meio da neutralização de fatores relacionados à mortalidade aumentada. Este estudo tem por objetivo identificar fatores de risco para mortalidade hospitalar em pacientes submetidos a implante de prótese valvar mecânica. MÉTODOS: Estudo prospectivo com aquisição retrospectiva de dados com 335 pacientes consecutivamente submetidos ao implante de prótese mecânica St Jude Medical, entre dezembro de 1994 e setembro de 2005, no Instituto de Cardiologia do RS, sendo 158 aórticos, 146 mitrais e 31 mitro-aórticos. Foi analisada a relação da mortalidade hospitalar com características demográficas e operatórias dos pacientes: sexo, idade, índice de massa corporal, classe funcional (NYHA), fração de ejeção, lesão valvar, hipertensão arterial sistêmica, diabete melito, creatinina, arritmia cardíaca, cirurgia cardíaca prévia, revascularização miocárdica, plastia tricúspide concomitante e caráter da cirurgia (eletivo, de urgência ou de emergência). Utilizada regressão logística para identificar os fatores de risco e quantificada sua influência pelo cálculo de odds-ratio. RESULTADOS: Ocorreram 13 (3,88 por cento) óbitos hospitalares. Características relacionadas à mortalidade aumentada foram creatinina sérica (P<0,05), fração de ejeção < 30 por cento (P<0,001), lesão valvar mitral (P<0,05), revascularização miocárdica (P<0,01), cirurgia cardíaca prévia (P<0,01) e reoperação (P<0,01). Odds ratio aumentado ocorreu para cirurgia cardíaca prévia (5,36; IC 95 por cento 0,94-30,56), revascularização combinada (5,28; IC 95 por cento 1,51-18,36), reoperação valvar (4,69; IC 95 por cento 0,93-23,57) e anuloplastia tricúspide associada (3,72; IC 95 por cento 0,75-18,30). BACKGROUND: Identification of preoperative heart valve surgery risk factors aim to improve surgical outcomes with the possibility to offset conditions related to increased morbidity and mortality. OBJECTIVE: Intent of this study is to identify hospital risk factors in patients undergoing bovine pericardial bioprosthesis implantation. METHODS: Retrospective study including 703 consecutive patients who underwent implantation of at least one St. Jude Medical-Biocor bovine pericardial bioprosthesis between September 1991 and December 2005 at the Rio Grande do Sul Cardiology Institute; 392 were aortic, 250 were mitral and 61 were mitroaortic. Characteristics analyzed were gender, age, body mass index, NYHA (New York Heart Association) functional class, ejection fraction, valve lesions, systemic hypertension, diabetes mellitus, kidney function, arrhythmias, prior heart surgery, coronary artery bypass graft, tricuspid valve surgery and elective, urgent or emergency surgery. Main outcome was in-hospital mortality. Relationship between risk factors and in-hospital mortality was analyzed using logistic regression. RESULTS: Were 101 (14.3%) in-hospital deaths. Characteristics with significant relationship to increased mortality were female gender (p<0.001), age over 70 years (p=0.004), atrial fibrillation (p=0.006), diabetes mellitus (p=0.043), creatinine > 2.4 mg/dl (p=0.004), functional class IV (p<0.001), mitral valve lesion (p<0.001), previous heart surgery (p=0.005), tricuspid valve surgery (p<0.001) and emergency surgery (p<0.001). CONCLUSION: Mortality rate observed is accepted by literature and is justifiable due to the high prevalence of risk factors, showing an increased significance level for female gender, age above 70, functional class IV, tricuspid valve repairs and emergency surgery. Offsetting these factors could contribute to reduced in-hospital mortality for valve surgery.

FUNDAMENTO: Identificação de fatores de risco pré-operatórios na cirurgia cardíaca valvar visa melhor resultado cirúrgico pela possível neutralização de condições relacionadas com morbi-mortalidade aumentada. OBJETIVO: Este estudo objetiva identificar fatores de risco hospitalar em pacientes submetidos a implante de bioprótese de pericárdio bovino. MÉTODOS: Estudo retrospectivo incluindo 703 pacientes consecutivos submetidos a implante de pelo menos uma bioprótese de pericárdio bovino St. Jude Medical-Biocor® de setembro de 1991 a dezembro de 2005 no Instituto de Cardiologia do RS, sendo 392 aórticos, 250 mitrais e 61 mitro-aórticos. Analisadas as características sexo, idade, índice de massa corporal, classe funcional (New York Heart Association - NYHA), fração de ejeção, lesão valvar, hipertensão arterial sistêmica, diabete melito, função renal, arritmias cardíacas, cirurgia cardíaca prévia, revascularização miocárdica, plastia tricúspide e caráter eletivo, de urgência ou de emergência da cirurgia. Desfecho primordial foi mortalidade hospitalar. Utilizou-se regressão logística para examinar relação entre fatores de risco e mortalidade hospitalar. RESULTADOS: Ocorreram 101 (14,3 por cento) óbitos hospitalares. Características significativamente relacionadas à mortalidade aumentada foram sexo feminino (p<0,001), idade superior a 70 anos (p=0,004), fibrilação atrial (p=0,006), diabete melito (p=0,043), creatinina > 2,4mg/dl (p=0,004), classe funcional IV (p<0,001), lesão valvar mitral (p<0,001), cirurgia cardíaca prévia (p=0,005), plastia tricúspide (p<0,001) e caráter cirúrgico emergencial (p<0,001). CONCLUSÃO: Observada mortalidade aceita pela literatura, justificável pela prevalência de alguns fatores de risco, tendo elevado nível de significância sexo feminino, idade superior a 70 anos, classe funcional IV, plastia tricúspide e caráter emergencial.
