RESUMEN

A 16-month-old Labrador-Poodle cross (case 1) and a 3-month-old German shorthaired pointer (case 2) were referred for patent ductus arteriosus (PDA) occlusion. Two-dimensional transthoracic and two- and three-dimensional transesophageal echocardiography revealed a window-like PDA characterized by a wide and short ductus. Due to the atypical PDA morphology with no ampulla in case 1, ductal occlusion was attempted with non-canine-specific Amplatzer occluder devices. However, these were too small and failed to remain stable. Amplatz Canine Duct Occluder (ACDO) devices were used with success in both cases. Due to the defects' morphology, the proximal ACDO disc protruded into the aorta but there were no signs of obstruction to aortic blood flow 16 months (case 1) and 1 month (case 2) post-occlusion. We describe two cases of a window-like type PDA that were successfully occluded with an ACDO.

RESUMEN

It's present an original method in the treatment of perforated duodenal ulcer (antro-duodenoplasty or antro duodenal aposition). To a group of 256 patients with gastro-duodenal ulcer operated in the period 1985-1995, it is fancy an apply these method to 56 patients (21%). These method applied is simple and performed in any surgical service and it't not necessary a special technical organization. The evolution of 56 patients there is no incidents. The originality consist in your simplicity, but and in the fact that these method it's not communicated in the surgical literature of Romania or anywhere in the latest ten years.

RESUMEN

The Greenfield filter is an effective and safe means of preventing postoperative pulmonary embolism in high-risk patients. However, it does not give absolute protection. We report a 69-year-old man who presented with deep vein thrombosis. Medical work-up revealed pancreatic tumor (Trousseau's sign). The Greenfield filter was placed in the inferior vena cava before operation. 11 days after total pancreatectomy clinical signs of pulmonary embolism appeared and the diagnosis was confirmed by ventilation-perfusion scan; anticoagulant therapy was instituted. The incidence of pulmonary embolism following interruption of the inferior vena cava by introducing the Greenfield filter is low (2.2%). Nevertheless, the diagnosis of pulmonary embolism should be considered in any patient with appropriate clinical signs, regardless of whether a filter was inserted.

RESUMEN

Fatal neutropenic enterocolitis was seen in a patient undergoing autologous bone marrow transplantation for non-Hodgkin's lymphoma. Excessive drug action due to a mildly diminished creatinine clearance could have contributed to the pathogenesis. Computed tomographic scanning and ultrasonography demonstrated pneumatosis of the gastrointestinal tract, but the disease had become extensive by then. Necrotizing enterocolitis should be suspected early in a granulocytopenic patient with abdominal pain and diarrhea or vomiting. Aggressive surgical or medical management may avoid a fatal outcome.

RESUMEN

Growth rate analysis of lung metastases of a cutaneous adenoid cystic carcinoma, which appeared 18 years after the resection of the primary tumor from the scalp is presented. The doubling times of the metastases were long compared with that of other lung metastases. They were 22 months for the metastasis in the right lung and 70 months for the metastasis in the left lung, with a shortening of the doubling time in the left side to 10.4 months in the last 4 months of observation. Backward extrapolation showed that the metastases to the lung were disseminated before the diagnosis and surgical resection of the primary tumor. To our knowledge, this is the third reported case of lung metastases from a cutaneous adenoid cystic carcinoma out of 25 documented cases. We present a review of the literature and discuss the clinical implications of our findings.
