RESUMEN

This study explores indoor air pollutant (PM1, PM2.5 and NO2) concentrations over a 15-week period during the COVID-19 pandemic in a typical suburban household in Oxford, UK. A multi-room intensive monitoring study was conducted in a single dwelling using 10 air quality sensors measuring real-time pollutant concentrations at 10 second intervals to assess temporal and spatial variability in PM1, PM2.5 and NO2 concentrations, identify pollution-prone areas, and investigate the impact of residents' activities on indoor air quality. Significant spatial variations in PM concentrations were observed within the study dwelling, with highest hourly concentrations (769.0 & 300.9 µg m-3 for PM2.5, and PM1, respectively) observed in the upstairs study room, which had poor ventilation. Cooking activities were identified as a major contributor to indoor particulate pollution, with peak concentrations aligning with cooking events. Indoor NO2 levels were typically higher than outdoor levels, particularly in the kitchen where a gas-cooking appliance was used. There was no significant association observed between outdoor and indoor PM concentrations; however, a clear correlation was evident between kitchen PM emissions and indoor levels. Similarly, outdoor NO2 had a limited influence on indoor air quality compared to kitchen activities. Indoor sources were found to dominate for both PM and NO2, with higher Indoor/Outdoor (I/O) ratios observed in the upstairs bedroom and the kitchen. Overall, our findings highlight the contribution of indoor air pollutant sources and domestic activities to indoor air pollution exposure, notably during the COVID-19 pandemic when people were typically spending more time in domestic settings. Our novel findings, which suggest high levels of pollutant concentrations in upstairs (first floor) rooms, underscore the necessity for targeted interventions. These interventions include the implementation of source control measures, effective ventilation strategies and occupant education for behaviour change, all aimed at improving indoor air quality and promoting healthier living environments.

BACKGROUND: Ablation or surgical excision is the typical treatment of anal high-grade squamous intraepithelial lesions (HSIL). Recurrences are common due to the persistence of underlying human papillomavirus (HPV) infection. Additional well-tolerated and effective non-surgical options for HPV-associated anal disease are needed. METHODS: This 3+3 dose escalation Phase I clinical trial evaluated the safety and tolerability of artesunate suppositories in the treatment of patients with biopsy-proven HSIL. RESULTS: The maximal tolerated dose was 400 mg, administered in 3 cycles. All adverse events associated with the use 200- and 400-mg artesunate suppositories were Grade 1. At the 600-mg dose, patients experienced clinically significant nausea. CONCLUSION: Artesunate suppositories are a safe treatment option for anal HSIL.

BACKGROUND: Readmission after ileostomy creation in patients undergoing colorectal surgery creates a significant burden on health care cost and patient quality of care, with a 30-day readmission rate of 40%. OBJECTIVE: This study aimed to evaluate the implementation of our perioperative quality improvement program, Decreasing Readmissions After Ileostomy Creation. DESIGN: Perioperative interventions were administered to patients who underwent ileostomy creation. SETTINGS: A single tertiary care academic center. PATIENTS: Eighty patients participated in this program from February 2020 to January 2021. MAIN OUTCOME MEASURES: The primary outcomes measured were 30-day readmission rates and causes of readmission, which were compared to a historical national database. Descriptive statistics were used to evaluate the effectiveness of this quality improvement program. RESULTS: Eighty patients were enrolled in this prospective quality improvement program. The mean age was 52 (±15.06) years. The most common indication for patients undergoing creation of an ileostomy was colorectal cancer (40%; n = 32). The overall 30-day readmission rate was 8.75% (n = 7) throughout the study period, which was significantly lower than historical cohort data (20.10%; p = 0.01). Among the 7 readmitted patients, 3 (3.75%) were readmitted due to dehydration. The most significant associated risk factor for all-cause readmission was urgent/emergent operative status, which was associated with an increased risk of readmission ( p = 0.01). The 3 readmitted patients with dehydration had a mean Dehydration Readmission After Ileostomy Prediction risk score of 11.71 points, compared to 9.59 points in nondehydrated patients, who did not require readmission ( p = 0.38). LIMITATIONS: This study is limited by its small sample size (N = 80). CONCLUSIONS: The Decreasing Readmissions After Ileostomy Creation program has been successful in reducing both the all-cause readmission rate and readmission due to dehydration both within an academic tertiary care referral center and in comparison with historical readmission rates.
