Intracellular levels of NAD+ regulate metabolism, among other ways, through enzymes that use NAD+ as a substrate, capable of inducing catabolic processes, such as lipid oxidation, glucose uptake, and mitochondrial activity. In several model organisms, administering precursor compounds for NAD+ synthesis increases its levels, improves lipid and glucose homeostasis, and reduces weight gain. However, evidence of the effects of these precursors on human patients needs to be better evaluated. Therefore, we carried out a systematic review and meta-analysis of randomized clinical trials that assessed the effects of NAD+ precursors on Metabolic Syndrome parameters in humans. We based our methods on PRISMA 2020. Our search retrieved 429 articles, and 19 randomized controlled trials were included in the meta-analysis. We assessed the risk of bias with the Rob 2 algorithm and summarized the quality of evidence with the GRADE algorithm. Supplementation with NAD precursors reduced plasma levels of total cholesterol and triglycerides in volunteers, but the intervention did not significantly affect the other outcomes analyzed. Three of the included articles presented a high risk of bias. The quality of evidence varied between very low and low due to the risk of bias, imprecision, and indirectness. The number of participants in outcomes other than lipidemia is still generally tiny; therefore, more clinical trials evaluating these parameters will increase the quality of the evidence. On the other hand, quality randomized studies are essential to assess better the effects of NAD+ precursors on lipidemia.

Brazil is one of the countries which has been most affected by dengue epidemics. This scenario became more challenging with the emergence of Zika virus after 2014. The cocirculation of dengue and Zika viruses makes their diagnosis and treatment a challenge for health professionals, especially due to their similar clinical outcomes. From 2015 to 2019, we followed a cohort of 2017 participants in Goiania, Goias, Central Brazil. Febrile cases were monitored weekly, and after identification of fever, the physician performed a home visit for clinical evaluation and collection of blood/urine for diagnosis of acute dengue/Zika infection in suspected cases. Dengue acute infection was investigated by NS1 antigen and real time RT-PCR and seroconversion of anti-dengue IgM. ZIKV infection was confirmed by real time RT-PCR. Six cases of Zika/dengue coinfection among participants were reported. The clinical outcomes were suggestive for both DENV and ZIKV infection. No coinfected patient had neurological clinical manifestation, warning signs or need for hospitalization. A continuous specific laboratory confirmation for both dengue and Zika viruses should be enforced as part of the surveillance systems even in the presence of very suggestive cases of dengue fever, minimizing the risk of a late detection of ZIKV circulation.

Despite promising preliminary results in treating fibromyalgia (FM) pain, no neuromodulation technique has been adopted in clinical practice because of limited efficacy, low response rate, or poor tolerability. This phase II open-label trial aims to define a methodology for a clinically effective treatment of pain in FM by establishing treatment protocols and screening procedures to maximize efficacy and response rate. High-definition transcranial direct current stimulation (HD-tDCS) provides targeted subthreshold brain stimulation, combining tolerability with specificity. We aimed to establish the number of HD-tDCS sessions required to achieve a 50% FM pain reduction, and to characterize the biometrics of the response, including brain network activation pain scores of contact heat-evoked potentials. We report a clinically significant benefit of a 50% pain reduction in half (n = 7) of the patients (N = 14), with responders and nonresponders alike benefiting from a cumulative effect of treatment, reflected in significant pain reduction (P = .035) as well as improved quality of life (P = .001) over time. We also report an aggregate 6-week response rate of 50% of patients and estimate 15 as the median number of HD-tDCS sessions to reach clinically meaningful outcomes. The methodology for a pivotal FM neuromodulation clinical trial with individualized treatment is thus supported. PERSPECTIVE: In this article, an optimized protocol for the treatment of fibromyalgia pain with targeted subthreshold brain stimulation using high-definition transcranial direct current stimulation is outlined.

Transcranial pulsed current stimulation (tPCS) is a non-invasive brain stimulation technique that employs weak, pulsed current at different frequency ranges, inducing electrical currents that reach cortical and subcortical structures. Very little is known about its effects on brain oscillations and functional connectivity and whether these effects are dependent on the frequency of stimulation. Our aim was to evaluate the effects of tPCS with different frequency ranges in cortical oscillations indexed by high-resolution qEEG changes for power and interhemispheric coherence. Thirty-eight healthy subjects were enrolled and received a single 20-min session of either sham or active stimulation with 1 Hz, 100 Hz or random frequency (1-5 Hz). We conducted an exploratory analysis to detect changes in mean power for theta, alpha and beta, and interhemispheric coherence for alpha and theta and four different sub-bands cognitive and non-specific adverse effects were recorded. We found that active stimulation with a random frequency ranging between 1 and 5 Hz is able to significantly increase functional connectivity for the theta and low-alpha band as compared to sham and active stimulation with either 1 or 100 Hz. Based on these findings, we discuss the possible effects of tPCS on resting functional connectivity for low-frequency bands in fronto-temporal areas. Future studies should be conducted to investigate the potential benefit of these induced changes in pathologic states.

Os escorpiões representam um grave risco à saúde pública brasileira, especialmente nas ßreas urbanas, devido à alta incidência e potencial gravidade de casos ocorridos por acidentes com esses animais peçonhentos. Do ponto de vista médico-sanitario, o gênero Tityus, com destaque para a espécie Tityus serrulatus é, atualmente, o reponsßvel pelo maior número de casos de picadas de escorpião no país. O objetivo do presente estudo foi relatar e comparar dois casos clínicos de escorpionismo ocorridos no mês de março do ano de 2012, na região metropolitana de Belo Horizonte, Minas Gerais, Brasil. Ambos os acidentes foram classificados como graves. Entre- tanto, as diferenças na abordagem e no manejo entre os dois casos clínicos foram consideradas fatores determinantes nas suas evoluções e prognósticos. Desta forma, fazer o diagnóstico e iniciar a administração de soroterapia antiveneno específica adequada o mais precocemente possível são essenciais nos casos de envenenamento por picada de escorpião, sendo os Centros de Informações e Assistência Toxicológi- cas (CIATs) ferramentas fundamentais...


Scorpions represent a serious risk to public health in Brazil, especial/y in urban areas due to lhe high incidence and potential severity of cases occurred in accidents with these poisonous animais. From the perspective of health care, the Tityus gender, especially the Tityus serrulatus species, is currently responsible for the largest number of cases of scorpion stings in the country. The purpose of the present work was to report and compare two clinical cases of scorpion envenomation occurred in March of 20/2, in the metropolitan region of Belo Horizonte, Minas Gerais, Brazil. Both accidents were classified as severe cases. However, the differences in approach and management between the two clinical cases were considered determinant in their evolution and prognosis. Therefore, make the diagnosis and initiate the administration of properly specific antivenom serotherapy as early as possible are core in cases of poisoning by scorpions sting, and the Centers for Toxicological Information and Assistance (CIATs) are fundamental tools...

RESUMEN

A territorialização, preconizada pelo Sistema Único de Saúde, é um dos pressupostos básicos da organização do processo de trabalho do Programa de Saúde da Família. O reconhecimento do território da área adscrita à uma Unidade Básica de Saúde é essencial para a caracterização da população e de seus problemas de saúde. Esse conhecimento, que vai além de uma simples descrição de sua população e serviços de saúde delimitados por famílias, é uma importante ferramenta de gestão no processo de cuidado e construção de saúde coletiva. O objetivo deste trabalho foi propor um plano de ação para reorganizar as equipes de estratégia saúde da família da Unidade de Saúde da Família Maria da Conceição através da reestruturação do processo de territorialização das áreas de cobertura da Unidade. Foi proposto um plano de ação elaborado através da estimativa rápida. Foi proposta também a observação de campo e entrevistas direcionadas a funcionários e usuários da Unidade (atores sociais). Foi realizada revisão bibliográfica de trabalhos sobre o tema para sustentação teórica. Conclui-se que conhecer as condições de saúde e risco de sua comunidade são essenciais para posteriormente planejar e programar ações em saúde e, consequentemente, melhorar o processo de trabalho das suas equipes de saúde da família
