RESUMEN

OBJECTIVE: To evaluate the efficacy of stereotactic soft-channel and hard-channel aspiration in the treatment of primary brainstem hemorrhage. METHODS: The clinical data of 146 patients with primary brainstem hemorrhage at the First Hospital of Hebei Medical University from August 2014 to August 2023 were analyzed. At admission, 146 patients were divided into soft-channel group or hard-channel group using a random number table method. Sixty-five patients were treated with stereotactic soft-channel aspiration, and 81 patients were treated with stereotactic hard-channel aspiration. The amount of residual hematoma was evaluated by head CT immediately after the operation, and the length of the indwelling drainage tube was recorded. Survival status at 30d after the operation and treatment outcome (mRS score and GOS score) at 90d after the operation were also recorded. The amount of residual hematoma immediately after the operation, the length of the indwelling drainage tube after the operation, the mortality rate and the treatment outcome were compared between the two groups. RESULTS: There was significant difference in the mortality rate 30d after the operation (41.5% vs. 14.8%, χ2 = 20.192, P = 0.035) between stereotactic soft-channel aspiration and stereotactic hard-channel aspiration for brainstem hemorrhage. The hard-channel group had a higher survival rate. There was significant difference in the treatment outcome at 90 days after treatment (18.5% vs. 32.1%, χ2 = 4.783, P = 0.047). The hard-channel group showed better treatment outcomes 90 days after surgery. CONCLUSION: Stereotactic hard-channel aspiration for primary brainstem hemorrhage has greater clearance efficiency than soft-channel aspiration and can significantly reduce patient mortality and improve treatment outcomes. Stereotactic hard-channel aspiration is a safe and effective method for treating primary brainstem hemorrhage.

RESUMEN

Objective To study the expression of thyroid hormone receptor binding protein 4(TRIP4)and DNA damage inducing transcription factor 4(DDIT4)in glioma tissue and their relationship with clinical pathological characteristics and prognosis.Methods 94 glioma patients admitted to the First Hospital of Hebei Medical University from February 2018 to February 2019 were selected as the research subjects.The expression of TRIP4,DDIT4 proteins in tissues were detected by immunohistochemistry.The relationship between the expression of TRIP4,DDIT4 proteins in glioma tissues and clinical pathological characteristics were compared.The differences in survival prognosis of glioma patients with different levels of TRIP4,DDIT4 protein expression were analyzed by Kaplan-Meier survival curve.Univariate and multivariate COX regression analysis was conducted to analyze the factors affecting the survival prognosis of glioma patients.Results The positive rates of TRIP4(68.09%),DDIT4(65.96%)proteins in glioma tissues were higher than those in adjacent tissues(13.83%,10.64%),with statistically significant differences(χ2=57.212,60.866,all P<0.05).There was a significant positive correlation between TRIP4 and DDIT4 protein expression in glioma tissues(r=0.722,P<0.05).The positive rates of TRIP4(83.64%vs 46.15%,80.00%vs 51.28%)and DDIT4(80.00%vs 46.15%,76.36%vs 51.28%)proteins in glioma tissues with tumor diameter≥3cm,WHO grade Ⅲ were significantly higher than those in tissues with tumor diameter<3cm,WHO grade Ⅰ~Ⅱ(χ2=6.393~14.754,P<0.05).The 3-year overall survival rates of the TRIP4 positive and negative expression groups were 37.50%(24/64)and 66.67%(20/30),respectively.The 3-year cumulative survival of the TRIP4 positive expression group was significantly lower than that in the TRIP4 negative expression group(Log-rank χ2=5.949,P=0.015).The 3-year overall survival rate of DDIT4 positive and negative expression group was 37.10%(23/62)and 70.00%(21/30),respectively.The 3-year cumulative survival of the DDIT4 positive expression group was significantly lower than that in the DDIT4 negative expression group(Log-rank χ2=7.642,P=0.006).Tumor diameter≥3cm(HR=1.614,P=0.000),WHO grade Ⅲ(HR=1.790,P=0.000),positive TRIP4(HR=1.665,P=0.000)and positive DDIT4(HR=1.476,P=0.000)were independent risk factors affecting the survival prognosis of glioma patients.Conclusion The expression of TRIP4 and DDIT4 protein in glioma tissue was increased.Both of them were related to tumor diameter and WHO grade,and are potential tumor markers for survival prognosis of glioma.

RESUMEN

OBJECTIVE To evaluate the palatability and chewability of chewable tablets, and provide reference for the quality evaluation of various types of chewable tablets. METHODS Using self-made Glucosamine hydrochloride chewable tablets as the model drug, the quality test was conducted. The in vitro simulation system for chewable tablets was established by using a texture analyzer and rheometer, and an oral simulation experiment was conducted on chewable tablets. The texture analyzer was used to measure the force required for chewing and simulate the static disintegration process of chewable tablets; the rheometer was adopted to measure the viscoelasticity, thixotropy, and deformability of chewable tablets during the chewing process. RESULTS The disintegration time limit, principal component content, and dissolution of self-made Glucosamine hydrochloride chewable tablets all met the limit requirements. The in vitro simulation results of the texture analyzer showed that self-made chewable tablets were easy to chew in both axial and radial directions, and the force required for chewing was within the range of the chewing force of the teeth; chewable tablets could disintegrate at an appropriate time without being chewed and only taken in the oral cavity. The in vitro simulation results of the rheometer showed that the chewable tablets in the oral cavity exhibited a behavior of elasticity as the main factor and viscosity as the secondary factor through the continuous stirring of the tongue, and the viscosity of the chewable tablets gradually decreased with tongue stirring or tooth chewing; when chewing with teeth, the internal force of the chewing tablets decreased, causing plastic deformation and crushing. After being crushed, the shape couldn’t be restored, making it easy to chew and swallow. CONCLUSIONS The combination of texture analyzer and rheometer can be used to simulate the oral chewing process and evaluate the palatability and chewability of self-made Glucosamine hydrochloride chewable tablets. This model can provide reference for the evaluation of various chewable tablets.

@#Objective To explore the relationship between serum tenascin C(TN-C),irisin(Irisin) and the severity and prognosis of patients with acute ischemic stroke(AIS).Methods 203 AIS patients admitted to our hospital(January 2017 to December 2019) were selected as the AIS group.According to the National Institutes of Health Stroke Scale(NIHSS) and Modified Rankin Scale(mRS),they were divided into severe group(n=52),moderate group(n=87),mild group(n=64) good prognosis group(n=125) and poor prognosis group(n=78),another 74 healthy people at the same period were selected as the control group,and the serum TN-C and Irisin level were compared between the groups,Pearson correlation analysis was used to analyze the correlation between serum TN-C and Irisin levels and NIHSS,mRS scores,ROC curve analysis was used to analyze the prognostic value of serum TN-C and Irisin levels in AIS.Results The serum TN-C levels in the AIS group were significantly higher than those in the control group,and the Irisin levels were significantly lower than those in the control group(P<0.05);the serum TN-C levels in the severe group were significantly higher than those in the moderate group,and the Irisin levels were significantly lower than those in the moderate group(P<0.05);the serum TN-C levels in the moderate group were significantly higher than those in the mild group,and the Irisin levels were significantly lower than those in the mild group(P<0.05);the serum TN-C levels in the poor prognosis group were significantly higher than those in the good prognosis group,Irisin levels were significantly lower than those in the good prognosis group(P<0.05);pearson correlation analysis showed that the serum TN-C level in the AIS group was positively correlated with NIHSS and mRS scores (P<0.05);the serum Irisin level was negatively correlated with NIHSS and mRS scores(P<0.05);ROC curve showed that the AUC,sensitivity and specificity of TN-C+Irisin predicting the prognosis of AIS were higher than those predicted by TN-C and Irisin alone.Conclusion Elevated serum TN-C level and decreased irisin level can reflect the severity of AIS and adverse prognosis,the combined determination of the two can improve the predictive value of the prognosis of AIS patients,and can be used as an indicator to evaluate the prognosis.

RESUMEN

A 66-year-old man was admitted for a 7-day history of painful blisters and erosions in the upper lip.Real-time PCR with herpes simplex virus (HSV) type-specific primers showed that the blister fluid and crusts were positive for HSV-1,but negative for HSV-2.Observation of the blister wall with transmission electron microscopy (TEM) revealed 3 types of nucleocapsid in the karyoplast of epithelia cells,including the electron-translucent core,granular core and electron-dense core.Numerous matured viral particles with envelope were found in the cytoplasm,which were identified as HSV.The diagnosis was made as herpes simplex in the upper lip based on the above findings.PCR based molecular typing combined with observation of HSV particles via TEM may be an effective approach to the definite diagnosis of primary herpes simplex.

RESUMEN

OBJECTIVE:To establish a HPLC method for simultaneous determination of paracetamol and dioxopromethazine hydrochloride in compound paracetamol and zinc gluconat tablets. METHODS: The determination was performed on Sinochrom ODS-BP column with the mobile phase consisting of 0.04 mol?L-1 KH2PO4 (pH=2.60,0.40% triethylamine)-acetonitril (75∶25) at a flow rate of 1.0 mL?min-1;the detection wavelength was set at 264 nm;the column was maintained at room temperature and the sample size was 20 ?L. RESULTS: The calibration curve was linear over the range of 2.818? 10-3~3.605?10-1 mmol?L-1 for paracetamol and 1.118?10-4~1.431?10-2 mmol?L-1 for dioxopromethazine hydrochloride. The recovery rates of both were greater than 99.2%. The intra-day RSDs of both were less than 1.82% and the between-day RSDs of both were less than 1.90%. CONCLUSION: The method is rapid,simple and accurate,and it is applicable for the quality control of the tablets.
