RESUMEN
Between June 1996 and September 2000, nine angioplasties of the infra-renal aorta were performed in two Surgical Vascular Centers (Hospital Saint-Joseph, Paris, Polyclinic La Baule), in 6 men and three women age ranged from 36 to 72 years (middle age 48 years and 2 months). Eight of these patients presented important stenoses of the middle infra-renal aorta, or in the aortic bifurcation, one of them presented an occlusion, leading to severe intermittent claudication, and rest pain. After endoluminal kissing balloon angioplasty, five aortic stents (five patients), and six iliac primitive arteries stents (in three patients), extended beyond the aorta, were placed. One isolated angioplasty was also performed. All the procedures were performed in the operative room, with good results, except for one patient requiring emergency aorto-bi-femoral by-pass, because of acute leg ischemia, immediately following the procedure. Follow-up was clinical, with Us-Doppler scan, and angiogram in all cases. After 36 months follow-up (4-52), all patients are asymptomatic, with permeable stents. This technique is well-known since 1980, without excessive morbidity. Aortic angioplasty represents the right technique, with good long-term results, for middle or terminal aorta atheromatous stenosis, according to classical aorto-bi-femoral by-passes. Final result depends on associated iliac arteries stenoses.

One case of isolate stenosis of the popliteal artery in a young woman is reported in this article. Since acid Methysergide maleate had been taken over a longer period, this lesion was placed into the nosological context of toxic arterial fibrodysplasia. Although such lesions usually recede at the end of treatment, the severity and abruptness of ischemia in this case led to operating the patient. This allowed an anatomopathological examination, which showed the diffusion of fibrosis into all layers of the arterial wall.

Melanesian population amounts to 61,270 out of 145,368 inhabitants in New Caledonia according to the last census. Melanesians of phototype VI have no predisposition in regard to malignant melanoma. From 1977 to 1987, 7 cases have been reported. Localisations are always in some zones not exposed to sunlight: 3 acral-lentiginous melanoma of the sole, 2 tumorous nodules of the anus margin and the vulva, 2 choroidal localisations. So, incidence of cutaneous malignant melanoma is very low (0.7/100,000 per annum) and in accordance with classical data regarding black people. But one has to keep in mind its being in some characteristic localisations. All the above referred cases have been detected at an advanced local extension.

Strategic therapeutic principles in 5 cases of injury to supra-hepaticocaval veins were based on vascular exclusion of liver prior to any approach to the lesion. An approach to the intrapericardial inferior vena cava by sternotomy is proposed together with the use of an endocaval shunt introduced through the atrium.

As 92 emergency operations of the colon have been carried out, a record is given of their indications, the most prevailing being, in Africa, intussusceptions and volvulus of the pelvic colon. Out of 64 exercises and colic derivations, 42 ideal colectomies are used as a ground for the analysis of the requirements and limits of this emergency operation. As out of the 7 fistulas they have observed, 6 of them have been outcomes of exeresis of necrosed colic segments, the authors in accordance with the newest bibliographic data, emphasize the importance of mastering the anastomosis technique.

The absence of obstetrical supervision and medical attention in dispensaries and health centers, the frequency of the hysterotomies are the reasons of numerous hysterorrhexis in Rwanda. This publication analyses the reasons and effects of this dreaded complications and proposes some prophylactic guidelines for this area of Africa, south of Sahara.
