One hundred and sixty-one non-catheterized consecutive acute geriatric admissions were screened for bacteriuria on the day following admission, both in the morning and in the afternoon and seven days later--morning and afternoon. The prevalence of bacteriuria was 29% on admission. A correlation between bacteriuria and leucocyturia was shown. The patients were admitted with a variety of medical illnesses but in no case was a diagnosis of urinary-tract infection made prior to admission. There was a significant relationship between incontinence and bacteriuria on admission; 12% of abacteriuric patients became bacteriuric between day 1 and day 7 following admission. Escherichia coli accounted for 51% of the isolates on admission to hospital. Bacteriuria was associated with increased mortality within 1 year following admission to hospital.

The effects of sulphinpyrazone 800 mg daily on renal excretory function were studied in a double-blind placebo-controlled randomised trial of incremental and full doses of the drug in 28 patients with plasma urea concentration less than 10 mmol/l in the period 2-28 days following uncomplicated acute myocardial infarction. Sulphinpyrazone in both dosage regimens increased uric acid excretion and lowered plasma urate concentration. There was no evidence that the drug reduced glomerular filtration rate or damaged the renal tubules. These results suggest that sulphinpyrazone in the doses used in this study is not contraindicated in patients early after acute myocardial infarction even though they may have a moderate rise in the blood urea.

Ffty cases of malignant pleural mesothelioma (47 men and 3 women) have been diagnosed during the period 1966-76 in Barrow-in-Furness, all of them histologically proved and accepted by the Pneumoconiosis Panel. At the time of writing only three survive. There was a history of industrial exposure to asbestos in all 47 men, who had worked in various trades in the shipyard, as had one of the women. One of the women was married to a shipyard plumber, who may have brought home asbestos dust on his clothes. Only one patient, a housewife, aged 28 at diagnosis, had no known asbestos contact. The asbestos content of the lungs has been measured in the last 20 cases, in 18 of whom it was found to be substantially greater than the accepted levels for the general population. In a small number studied by electron microscopy, the predominant fibre was crocidolite. A positive histological diagnosis was achieved in 39 subjects during life in the expectation of securing industrial compensation, but, in spite of this, less than half of the dependants are currently receiving payments. Metastases, though never clinically apparent, were frequent at necropsy.
