RESUMEN

Pulmonary embolism occurring during pregnancy is a rare accident but that still brings about a high mother mortality; it seems to be five to six times more frequent during the pregnancy and the post-partum than for non-parturient women who don't take any estro-progestogens, pulmonary embolism would involve complications for 0.5/1000 pregnancies before delivery. As it presents a lot of diagnostic problems, it is under-estimated. The vascular radiological examinations expose the foetus or embryo to considerable radiation and to a risk of foetal hypothyroidism leading to backwardness. The lung perfusion scanning has the advantage of not injecting iodine but is not specific. Fortunately, some medical examinations such as plethysmography or Doppler echography are safe and can also guide the clinician. As far as therapy is concerned, intravenous heparin is the first intention treatment, it can be replaced subsequently by subcutaneous heparin (low molecular weight heparin). In case of heavy pulmonary embolism endangering the vital prognosis of the patient, in case of clinical or biological resistance to the medical treatment, it could be necessary to perform a pulmonary embolectomy with, if necessary, vena cava interruption with insertion of a mechanical filter.

RESUMEN

A stillborn female with Hanhart's syndrome in association with hydrocephalus owing to stenosis of the aqueduct of Sylvius is presented. Neuropathological findings are suggestive of an acquired pathophysiological mechanism.

RESUMEN

The authors describe a case of fetal sinusoidal cardiac rhythm which was observed during a feto-maternal transfusion. This type of cardiographic tracing is found similarly in certain cases of severe fetal anaemia or of isolated fetal hypoxia. It is also found in amnionitis and associated with various iatrogenic factors such as the administration of general anaesthesia and fetoscopy. The literature does not show a consensus when trying to find the criteria for defining this type of rhythm. The aetiology of the pathological condition is much in dispute. The prognosis does not have to be all that serious. In conclusion, the authors suggest the attitude of management to lessen the poor prognosis of these sinusoidal rhythms and help towards a favourable outcome.

RESUMEN

The authors report an unusual case of major lung hypoplasia in both of twins without any other associated malformations. This case has given an opportunity to make a hypothesis on the aetiology and pathogenesis in order to try to explain the pathology and better to appreciate fetal respiratory function.

RESUMEN

When repeated measurements of the circumference of the thorax of the fetus were carried out systematically the author could show from the beginning of the third trimester of the pregnancy that there was a close correlation between this measurement and the birth weight so that slow-for-dates growth could be detected very early.

RESUMEN

It would seem that observing the speed at which the circumference of the thorax of the fetus grows can be a rapid and reliable indication for monitoring whether the pregnancy is normal or abnormal. In 38 cases admitted to hospital because of troubles in fetal growth 20 showed that normal growth of the circumference of the thorax was associated with absence of pathology. On the other hand when growth was altered or absent the fetal or maternal pathology was serious.

RESUMEN

The object of this work is to demonstrate a new rapid method of working out the thoracic and skull perimeters iK the fetus using real time ultra-sound. Normal growth curves have been established as a result of 1955 measurements of the perimeters of the thorax and 729 measurements of the perimeters of the skull in 1344 patients pregnant with a single fetus. the gestational age had been checked by an ultra-sound examination between the 12th and the 20th week of the pregnancy. 43 newborn were light in weight below the 10th percentile on Lubchenco's curve. An analysis of these results shows that the diagnosis of growth abnormalities is made most efficiently by using the perimeter of the thorax. It is suggested that systematic and repeated use of this measurement in the third trimester of pregnancy is of value.

RESUMEN

The author analyses the different intra-uterine growth curves that are described in the literature and shows how in use as reference norms they have a restricted value. The author draws attention to the fact that no other author cited in the literature has used an objective method to control the gestational age of the fetus when making curves. The author demonstrates that in 17.3 p. 100 of cases the gestational age has to be corrected when making his own growth curve and this is done by measuring the bi-parietal diameter early in the pregnancy. He studies the repercussions of this high figure of non-correspondence with the dates at different levels of the curve and the percentage of infants defined as hypotrophic, macrosomic or post-mature.

RESUMEN

After having reviewed our present knowledge of the pharmacology of bromocriptine, its action on the pituitary, centrally and peripherally, the authors detail the results they have obtained in inhibiting lactation in two groups of patients who had delivered normally or had Caesarean sections. In the first group where bromocriptine was started in the first 24 hours with dosages of 2.5 mg every 12 hours for 14 days, clinical signs of breast engorgement and of milk secretion were insignificant and the level of prolactin had dropped to normal menstrual cycle levels from the third day after delivery. In the second group where the first dose of bromocriptine was given at least 48 hours after Caesarean section, the levels of prolactin dropped just as sharply from the third day after delivery, but on the other hand the clinical signs of breast engorgement and milk secretion were marked and completely comparable to those which are found in patients who are not going to breast feed but who have received no treatment to inhibit lactation.
