RESUMEN

The objective is to assess the influence of sociodemographic, professional and clinical variables on the choice of treatment of work-related carpal tunnel syndrome (CTS). An exhaustive and trans-sectional study was conducted over a period of eight years, from 1st January 2006 to 31 December 2013 in the Department of Occupational Medicine at University Hospital of Mahdia, Tunisia. The study population was represented by patients with work-related carpal tunnel syndrome. Data collection was based on a questionnaire sheet, describing social, occupational and medical characteristics of patients. The study population was characterized by a large female dominance, representing 95.3% with an average age of 42±7.8 years. Patients medically treated represented 38.7% and 61.3% had had surgical treatment. After binary logistic regression, surgical indication of CTS was significantly correlated to diabetes (p=0.017), other musculoskeletal disorders (p=0.02), functional signs of CTS (acrocyanosis p=0.05; muscle weakness p=0.015; radiating pain p=0.01; painful discomfort of the hand, the forearm or arm p=0.027) and to the atrophy of thenar muscles (p=0.018). According to this study, the choice of therapy for occupational CTS depends only on clinical data. More detailed studies will be needed to refine these results.

INTRODUCTION: Ramsay Hunt syndrome is a viral infection that combines facial palsy and pinnal vesicular rash. OBJECTIVES: To detail diagnostic criteria, treatment and prognosis in Ramsay Hunt syndrome. PATIENTS AND METHODS: A retrospective study of 15 patients with Ramsay Hunt syndrome admitted to the ENT department of La Rabta Hospital (Tunis) from 2003 to 2009. RESULTS: The main presenting symptom was facial palsy associated with conchal vesicular rash. House-Brackmann classification of facial nerve function ranged from III to V. All patients were treated with acyclovir and corticosteroids. Mean follow-up was 8 months. Ten patients showed improvement in facial palsy, four had permanent palsy and one moved from grade IV to grade III. CONCLUSION: Ramsay Hunt syndrome involves severe dysfunction, with poorer facial nerve prognosis than in Bell's palsy.

BACKGROUND: Cutaneous metastasis of colorectal cancer is rare. We report a case of fistular lesions of the buttocks revealing a mixed tumour of the appendix involving mucinous cystadenocarcinoma and carcinoid tumour. CASE REPORT: A 67-year-old woman was admitted for four skin fistulae of the right buttock present for 6 years. Histological examination of skin biopsy specimens identified infiltration of the dermis by metastatic mucinous adenocarcinoma while colonoscopy showed a caecal tumour measuring 4 cm. Surgical excision was performed involving right hemicolectomy, evacuation of retroperitoneal mucin collection and excision of fistulae. Histopathological examination of surgical specimen confirmed mixed tumour consisting of perforated mucinous cystadenocarcinoma and carcinoid tumour of the appendix. Recurrence of the fistular lesions was seen. The patient was hospitalized several times for surgical drainage of mucin. She died one year later. DISCUSSION: Cutaneous metastasis of colorectal cancer is an uncommon event that usually occurs after identification of the primary tumour and generally indicates advanced-stage disease and an ominous prognosis. This case is particular and underlines the need to rule out a metastatic origin of cutaneous fistulae, even in patients otherwise apparently in good health.

RESUMEN

Objectif : Les traumatismes externes du larynx sont rares. Ces traumatismes; ouverts ou fermes; peuvent etre isoles; ou s'integrer dans la cadre d'un traumatisme cervical complexe ou d'un polytraumatisme. Leurs aspects etiologiques; cliniques; diagnostiques et therapeutiques sont tres varies. Les principes generaux de leur prise en charge et de leur traitement chirurgical dependent de la gravite des lesions Materiel et methodes : Nous rapportons une etude retrospective portant sur 30 patients ayant ete suivis et traites pour un traumatisme externe du larynx dans le service d'ORL de la Rabta. La serie a ete colligee sur une periode de 25 ans allant de 1983 a 2008. Resultats : L'age moyen de nos patients est de 30 ans. Les etiologies sont dominees par les accidents de la circulation (53de nos patients). Tous nos malades ont eu un examen endolarynge par le nasofibroscope ou le miroir larynge. La laryngoscopie sous AG a ete realisee chez 25 patients. La tomodensitometrie cervicale a ete pratiquee chez 16 patients. Au decours de ces examens; tous nos patients ont ete classes selon la classification de Schaeffer modifiee par Fuhrman. Plus que la moitie des patients (54) sont classes en stade I et II. L'exploration chirurgicale a ete indiquee pour 16 patients. Le traitement a ete conservateur dans tous les cas. Le recul moyen est de 2 ans. Trois de nos patients ont developpe une stenose sous glottique. Deux d'entre eux ont eu une laryngoplastie d'agrandissement type Rethi. Conclusion : Tout traumatisme larynge decouvert; doit etre pris en charge correctement afin d'eviter des complications pouvant mettre en jeu aussi bien le pronostic vital que le pronostic fonctionnel

RESUMEN

Le kyste de la vallecule est une malformation laryngee rare pouvant parfois mettre en jeu le pronostic vital. Diagnostique et traite a temps; le risque de complications et de morbidite peut etre ecarte. Les auteurs rapportent 2 cas de kystes de la vallecule. La tomodensitometrie a confirme la presence d'une formation kystique prenant origine au niveau de la vallecule. Une laryngoscopie directe a ete faite pour les 2 patients sous anesthesie generale. Le premier malade a eu une resection endoscopique complete du kyste et le 2eme une large marsupialisation au Laser. L'examen anatomopathologique a confirme la benignite de la formation kystique dans les 2 cas

BACKGROUND: Cutaneous metastases from gastric carcinoma are uncommon and are exceptionally the first sign of disease. CASE REPORT: A 33-year-old man presented with nodular lesions that had developed three months earlier on the trunk, scalp and limbs. A skin biopsy suggested a metastatic origin. The upper digestive fibroscopy with biopsy demonstrated gastric linitis. Due to the presence of bone metastasis, treatment was not initiated, the patient died four months later. DISCUSSION: Metastatic dissemination of gastric carcinoma to the skin usually occurs in advanced stage disease. Generally, the skin lesions are painless hard nodules on the anterior abdominal wall. Our patient developed unusual inaugural skin metastases with an exceptional localization.
