RESUMEN

Postpartum hypertension can significantly increase maternal morbidity and mortality, and hence it requires prompt interdisciplinary evaluation and interventions. We present a case of a gravid patient with significant comorbidities who required multiple treatments and care from several specialists following a complicated vaginal delivery. The outcome of this case depended on a focused differential diagnosis and interdisciplinary consultation with the several teams involved. This case report illustrates the importance of effective communication and an interdisciplinary approach in the management of postpartum hypertensive emergencies. Such an approach is crucial in reducing maternal complications following postpartum hypertension, as well as reducing the length of hospital stay to improve maternal and fetal outcomes.

While young, healthy individuals without underlying medical conditions have generally not suffered catastrophic health consequences from the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), gravid patients appear to be at much higher risk of complications from this virus. A 29-year-old G3P2 patient at 30 weeks and three days presented with worsening dyspnea and chest pain after testing positive for coronavirus disease 2019 (COVID-19) infection two days prior. Notably, she had not received COVID-19 vaccination. A non-reassuring fetal tracing and fetal bradycardia were discovered on routine prenatal monitoring during admission, and an urgent caesarean section was performed. She subsequently required supplemental oxygen due to respiratory distress and remained hospitalized. She clinically deteriorated from a respiratory standpoint. Several days later, she experienced cardiac arrest with a return of spontaneous circulation (ROSC) in nine minutes. While the baby was discharged home and is doing well, the patient, unfortunately, expired from hypoxic encephalopathy secondary to COVID-19 pneumonia and complications of cardiorespiratory arrest. This case highlights the severe sequelae of COVID-19 infection in a postpartum patient, including ventilator-dependent respiratory failure, sudden cardiac death, hypoxic encephalopathy, and coma.

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) responsible for the coronavirus disease 2019 (COVID-19) pandemic has rarely impacted neonates. When infection does occur, it is typically asymptomatic. We describe a case of a neonate born to a 25-year-old mother who was COVID-19 positive but asymptomatic. An emergent cesarean section was performed during week 30 of gestation due to category three fetal heart tracings. The neonate, unfortunately, died on the day of life 12 from respiratory distress secondary to severe COVID-19 pneumonia. This is an important case that illustrates the deleterious impact COVID-19 infection can have on neonates. It is a unique case of the compassionate use of remdesivir for a neonate. The patient's respiratory decline soon after birth, lends support that the virus responsible for COVID-19 can be transmitted vertically.

Pelvic inflammatory disease (PID) and tubo-ovarian abscess (TOA) are serious diagnoses to consider in reproductive-age women presenting with abdominal or pelvic pain. Management can be medical or surgical depending on severity. This case report outlines the unique presentation of TOA in a 15-year-old female presenting with acute abdominal pain with a recent past surgical history of appendectomy. A discussion of the approach to similar presentations and the importance of maintaining a broad differential diagnosis follows.

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has important implications for gravid patients as they are more likely to experience severe complications of pregnancy such as multisystem inflammatory syndrome if infected with coronavirus disease 2019 (COVID-19). Due to normal physiological adaptations of pregnancy, COVID-19 may strain an already stressed respiratory system, making delivery a viable treatment option. We present a case of a gravid patient infected with COVID-19 pneumonia who delivered vaginally in the intensive care unit (ICU) at our hospital. Further research into clinical progress and management of pregnancy complicated by COVID-19 is necessary.

The risk of postpartum hemorrhage (PPH) and placental adhesion anomalies, including placenta previa, may be increased in pregnancies conceived by in vitro fertilization (IVF) and other forms of assisted reproduction technologies. The uterine compression suture, known as the "uterine sandwich method," may be useful in pregnancies complicated by placenta previa. We report an unusual case of placenta previa complicated by velamentous cord insertion, which was treated by a B-Lynch suture, a Bakri balloon tamponade, and vaginal packing.

A pesar de la importancia del ecosistema de páramo en el ciclo global de carbono no se dispone de una síntesis que permita diferenciar si es la biomasa vegetal o el suelo el componente que acumula la mayor cantidad de carbono. En este documento se busca integrar los aspectos fundamentales relacionados con el carbono almacenado en el ecosistema de páramo, a través de: 1) Revisar las estimaciones de carbono en biomasa aérea y suelo reportadas por la literatura para el ecosistema de páramo; 2) Examinar los efectos producidos por la actividad agropecuaria en el carbono almacenado en el páramo y 3) Identificar prácticas que reducen las emisiones de carbono en el páramo. Se revisaron artículos en bases de datos como Science Direct Springerlink, Willey Online Library y Google Scholar El componente que almacena mayor cantidad de carbono en páramo es el suelo, con contenidos entre 119 y 397 t/ha en los primeros 40 cm de profundidad. Mientras que la biomasa aérea varía entre 13,21 y 183 t/ha. Es necesario incrementar las investigaciones sobre carbono orgánico en suelos de páramo. Esta información podría contribuir a apoyar acciones tendientes a vincular los ecosistemas de paramo al mercado de carbono.


Despite the importance of the paramo ecosystem in the global carbon cycle, there is not an available synthesis for identifying if plant bio-mass or soil accumulates the maximum amount of carbon. The purpose of this document is to focus on paramo ecosystem stock carbon, throught: 1) to review estimates for the abovegroundbiomass and soil organic carbón; 2) to examine the agricultural production effects on carbon storage, and 3) to identify management practices for reducing carbon emissions in the paramo ecosystem. We searched papers about carbon storage in paramo in databases like Science Direct, Springerlink, Willey Online Library y Google Scholar. Soil stored more carbon than aboveground biomass. Soil carbon organic (SOC) between 119 and 397 t/ha was storaged in the upper 40 cm. Meanwhile, aboveground biomass carbon varied between 13,21 y 183 t/ha. More studies about SOC are requiered. This information could contribute to support actions over the entailment of paramo ecosystem in carbon markets.


Apesar da importância do ecossistema de páramo no ciclo global do carbono não está disponível uma síntese para diferenciar se é a biomassa vegetal ou solo, o componente que acumula a maior parte do carbono. Este trabalho procura integrar os aspectos fundamentais do carbono armazenado no ecossistema páramo, por meio de: 1) Rever as estimativas de carbono na biomassa e no solo relatado pela literatura para o ecossistema páramo; 2) Analisar os efeitos da atividade agrícola no carbono armazenado no páramo e 3) Identificar práticas que reduzem as emissões de carbono no páramo. Artigos foram revistos em bases de dados, tais como Science Direct SpringerLink Wiley Online Library e Google Scholar. O componente que armazena a maior parte do carbono é o solo, com contidos entre 119 e 397 t/ha nos primeiros 40 cm de profundidade. Enquanto a biomassa varia entre 13,21 e 183 t / ha. É preciso acrescentar a pesquisa sobre carbono orgânico no solo em páramo. Esta informação poderia ajudar a apoiar as ações para vincular os ecossistemas de Páramo ao mercado de carbono.
