RESUMEN

Hypertension guidelines recommend initiating treatment with single pill combination (SPC) antihypertensive medications, but SPCs are used by only one-third of treated hypertensive US adults. This analysis estimated the cost-effectiveness of initial treatment with SPC dual antihypertensive medications compared with usual care monotherapy in hypertensive US adults.The validated BP Control Model-Cardiovascular Disease (CVD) Policy Model simulated initial SPC dual therapy (two half-standard doses in a single pill) compared with initial usual care monotherapy (half-standard dose when baseline systolic BP < 20 mmHg above goal and one standard dose when ≥20 mmHg above goal). Secondary analyses examined equivalent dose monotherapy (one standard dose) and equivalent dose dual therapy as separate pills (two half-standard doses). The primary outcomes were direct healthcare costs, quality-adjusted life-years (QALYs), and incremental cost-effectiveness ratio (ICER) over 10 years from a US healthcare sector perspective.At 10 years, initial dual drug SPC was projected to yield 0.028 (95%UI 0.008 to 0.051) more QALYs at no greater cost ($73, 95%UI -$1 983 to $1 629) than usual care monotherapy. In secondary analysis, SPC dual therapy was cost-effective vs. equivalent dose monotherapy (ICER $8 000/QALY gained) and equivalent dose dual therapy as separate pills (ICER $57 000/QALY gained). At average drug prices, initiating antihypertensive treatment with SPC dual therapy is more effective at no greater cost than usual care initial monotherapy and has the potential to improve BP control rates and reduce the burden of CVD in the US.

Objective: To examine trends in child sleep, physical activity, and screen use during the COVID-19 pandemic in New York City with a prospective, longitudinal online survey of parents recruited from a large medical center. Methods: Data was collected Spring 2020 ("Complete Shutdown") and Fall 2020 ("Partial Shutdown"). Outcomes were parental perceptions about changes in child sleep, physical activity, and screen time compared to before COVID-19; and contemporaneous measures of these child behaviors. We report contemporaneous responses and paired analyses to describe longitudinal changes. Results: Two hundred seventy-seven participants were surveyed during Complete Shutdown and 227 (81.9%) filled out a follow-up survey during Partial Shutdown. The largest percentage of parents at both time points perceived no change in child sleep, decreases in child exercise, and increases in child screen time. In paired analyses, perceptions shifted toward less sleep, more physical activity and less screen time from Complete Shutdown to Partial Shutdown. Conclusion: COVID-19 had negative impacts on child health behaviors that did not resolve over a 6-month period despite partial reopenings.

Introducción: El aislamiento absoluto es un procedimiento odontológico que garantiza la bioseguridad y la calidad de los tratamientos restaurativos; sin embargo, no existe un instrumento adecuado para evaluar el tema en la realidad peruana. Objetivo: Diseñar y validar una escala para evaluar las actitudes de odontólogos peruanos sobre el aislamiento absoluto. Material y métodos: Estudio de tipo instrumental y corte transversal. La escala fue administrada mediante un cuestionario virtual y posteriormente distribuida por redes sociales a 279 odontólogos (143 mujeres y 136 hombres) mediante un muestreo no probabilístico intencional. Las edades comprendidas estuvieron entre 22 y 67 años. Resultados: Los valores del coeficiente V de Aiken fueron favorables para todos los ítems. El análisis factorial exploratorio indicó la existencia de un solo factor. El análisis factorial confirmatorio sugirió eliminar los ítems 1 y 7. La confiabilidad de la escala fue buena y se calculó con el coeficiente α de Cronbach = 0,71. Conclusiones: La escala sobre las actitudes de los odontólogos peruanos acerca del aislamiento absoluto (Act-AA-Cov19) es una medida breve, válida y confiable, que puede ser utilizada para fines investigativos(AU)


Introduction: Rubber dam isolation is a dental procedure that ensures biosafety and the quality of restorative treatments; however, no appropriate tool is available to evaluate the topic in Peruvian reality. Objective: Design and validate a scale to evaluate the attitude of Peruvian dentists toward rubber dam isolation. Methods: An instrumental cross-sectional study was conducted. The scale was applied through a virtual questionnaire and then distributed on social networks to 279 dentists (143 women and 136 men) selected by intentional nonprobability sampling. Age ranged from 22 to 67 years. Results: Aiken's V coefficient values were favorable for all items. Exploratory factor analysis showed the presence of a single factor. Confirmatory factor analysis suggested deletion of items 1 and 7. Reliability of the scale proved to be good. It was estimated with a Cronbach's α coefficient of 0.71. Conclusions: The scale about attitudes of Peruvian dentists toward rubber dam isolation (Act-AA-Cov19) is a brief, valid and reliable measurement which may be used for research purposes(AU)

RESUMEN Introducción: La región de la cabeza y el cuello es una de las áreas más complejas del cuerpo y el carcinoma epidermoide es la neoplasia maligna más frecuente en esta localización. Objetivo: Caracterizar, desde el punto de vista clínico, epidemiológico y terapéutico, los pacientes con carcinoma epidermoide de cavidad bucal. Métodos: Estudio observacional, descriptivo y transversal en pacientes tratados en un servicio de cirugía maxilofacial entre el 1 de enero de 2018 y el 31 de diciembre de 2020. Se analizaron variables clínicas, epidemiológicas y terapéuticas. La información se obtuvo mediante la entrevista y el examen clínico. Como medidas de resumen se utilizaron frecuencias absolutas y porcentajes. Resultados: Se estudiaron 29 pacientes, con predominio del sexo masculino (69,0 %). La edad media fue de 60,3 años (rango entre 41 y 80 años). La lengua fue la región anatómica más afectada (34,5 %) y la úlcera hundida o infiltrante la variante clínica de mayor presentación (27,6 %). El 48,3 % de las neoplasias fueron diferenciadas histológicamente. El 65,5 % de los diagnósticos se realizó en estadios avanzados de la enfermedad (etapas III y IV). Los tratamientos más utilizados fueron la cirugía (31,0 %) y la combinación de quimioterapia más radioterapia (27,6 %). Conclusión: Predominan los pacientes masculinos, las lesiones linguales, de aspecto ulcerado y diferenciadas histológicamente. La modalidad terapéutica más utilizada fue la cirugía.


ABSTRACT Introduction: Head and neck region is one of the more complex areas of the body and the squamous cell carcinoma is the most frequent malignant neoplasm in this location. Objective: To characterize, from a clinic, epidemiologic and therapeutic point of view, patients with squamous cell carcinoma of the oral cavity. Methods: Observational, descriptive and cross-sectional study with patients treated in maxillofacial surgery service between January 1, 2018 and December 31, 2020. Clinic, epidemiologic and therapeutic variables were analyzed. The information was obtained through interview and clinical examination. Absolute frequencies and percentages were used as summary measures. Results: Twenty-nine (29) patients, with a predominance of males (69.0 %), were studied. Mean age was 12 years (range between 41 and 80 years). The tongue was the most affected anatomical region (34.5 %) and the sunken or infiltrating ulcer was the most common clinical variant (27.6 %); 48.3 % of the neoplasms were differentiated histologically. 65.5 % of the diagnoses were made in advanced stages of the disease (stages III and IV). The most used treatments were surgery (31.0 %) and the combination of chemotherapy plus radiotherapy (27.6 %). Conclusion: Male patients, lingual lesions, ulcerated appearance and histologically differentiated predominated. The most used therapeutic modality was surgery.

Introducción: Publicar artículos desde el pregrado permite al estudiante culminar el proceso de investigación y desarrollar habilidades útiles para el posgrado. Objetivo: Describir la producción científica estudiantil en las revistas odontológicas indexadas en la colección SciELO durante los años 2018 y 2019. Métodos: Se realizó un estudio observacional, descriptivo, de corte transversal, considerando como unidad de análisis los artículos. Se consideraron las revistas indexadas en la colección en el momento de las búsquedas, así como las que incluyeron en sus artículos información relacionada con el grado académico de los autores. Se estudiaron las siguientes variables: nombre de la revista, factor de impacto, artículos con participación estudiantil, año de publicación, idiomas, tipologías, temáticas, número de estudiantes por artículo, tipos y modalidad de autoría, países, universidades y citas. Resultados: Se evaluaron nueve revistas y en tres no se encontraron artículos con autoría estudiantil. Se publicaron 889 artículos, de los cuales el 3,49 por ciento tuvo participación de estudiantes. International Journal of Odontostomatology (5,41 por ciento) fue la revista con más artículos con autoría estudiantil. El 67,74 por ciento de los artículos fueron originales. Los autores estudiantiles brasileños y peruanos publicaron igual número de artículos (n = 8). El 19,35 por ciento de los artículos investigó temas de cirugía maxilofacial. Tres artículos estuvieron afiliados a la peruana Universidad Nacional Mayor de San Marcos. Conclusiones: La producción científica estudiantil en las revistas odontológicas indexadas en SciELO es baja y se concentra principalmente en los artículos originales(AU)


Introduction: Publishing articles during undergraduate training makes it possible for students to complete the research process and develop skills useful for graduate education. Objective: Describe students' scientific production in dental journals indexed in the SciELO collection during the years 2018 and 2019. Methods: A cross-sectional observational descriptive study was conducted in which articles were the units of analysis. The journals considered were those indexed in the collection when the search was performed and those including articles containing information related to the academic degree of authors. The variables considered were journal name, impact factor, articles with students' participation, year of publication, languages, typologies, topics, number of students per article, authorship types and modes, countries, universities and citations. Results: Nine journals were evaluated, three of which did not contain any article of student authorship. A total 889 articles were published, with students participating in 3.49 percent of them. The International Journal of Odontostomatology (5.41 percent) was the journal with the largest number of articles of student authorship. 67.74 percent of the articles were original. Brazilian and Peruvian student authors published the same number of articles (n = 8). 19.35 percent of the articles dealt with topics related to maxillofacial surgery. Three articles were affiliated to the Peruvian National University of San Marcos. Conclusions: Students' scientific production is low in dental journals indexed in SciELO, and is mainly composed of original articles(AU)

RESUMEN Introducción: El aislamiento absoluto es una técnica odontológica que aumenta la probabilidad de éxito del tratamiento, más en esta pandemia. Objetivo: Caracterizar las actitudes de odontólogos peruanos hacia el aislamiento absoluto en tiempo de pandemia por la COVID-19 e identificar factores asociados. Métodos: Estudio observacional y transversal con empleo de técnicas analíticas, realizado a través de una encuesta virtual a odontólogos del Perú. Se usó la escala Act-AA-Cov19, al puntaje se le dividió en tercil superior (los de acuerdo) versus los otros puntajes (categoría de comparación); se obtuvieron estadísticos de asociación. Resultados: En los 279 odontólogos, la importancia del aislamiento absoluto y el uso de equipos de protección personal fue lo más aceptado (96 % de acuerdo siempre y 4 % casi siempre). La necesidad de actualizar los conocimientos sobre aislamiento absoluto se asoció con una mejor actitud hacia el proceder (RPc: 1,52; IC 95 %: 1,18-1,95; valor p= 0,001); mientras que el resultado fue opuesto en los graduados en universidades privadas (RPc: 0,79; IC 95 %: 0,66-0,95; valor p= 0,011). Conclusión: La premisa más aceptada es sobre la importancia del aislamiento absoluto y uso de equipos de protección personal, seguido por el hecho de que un campo de trabajo libre de saliva/ sangre facilita el procedimiento.


ABSTRACT Introduction: Rubber dam isolation is a dental technique that increases the probability of successful treatment, especially in this pandemic. Aim: To characterize the attitudes of Peruvian dentists towards rubber dam insolation in times of COVID-19 pandemic and to identify factors associated with them. Methods: Observational and cross-sectional study using analytical techniques, carried out through a virtual survey of dentists in Peru. The Act-AA-Cov19 scale was used, the score was divided into the upper tertile (those in agreement) versus the other scores (comparison category); association statistics were obtained. Results: Of the 279 dentists, the importance of rubber dam isolation and use of personal protective equipment was the most accepted (96 % always agreed and 4 % almost always). The need to update knowledge about rubber dam isolation was associated with a better attitude towards the procedure (PRc: 1,52; 95 % CI: 1,18-1.95; p value = 0,001); while the result was the opposite in graduates from private universities (PRc: 0,79; 95 % CI: 0,66-0,95; p value = 0,011). Conclusion: The most widely accepted premise is about the importance of rubber dam isolation and use of personal protective equipment, followed by the fact that a saliva / blood-free work area facilitates the procedure.

OBJECTIVES: Correct electrode placement is a challenge of cochlear implant surgery, which occurs because electrode position cannot be directly visualized. This work aims to 1) develop a protocol for a practical, consistent, single view plain radiograph able to be used to confirm cochlear implantation, 2) confirm its usefulness on patients, and 3) confirm its usefulness for identifying misplaced electrodes in cadaveric ears. STUDY DESIGN: Imaging procedure and quality improvement initiative. SETTING: Tertiary academic hospital. PATIENTS: Cadaveric ears, and patients undergoing cochlear implantation. INTERVENTIONS: An intraoperative imaging protocol was developed specifying patient head position, machine position, and exposure setting. This was tested to confirm proper cochlear implantation in patients, including one revision case. This technique allowed the electrode placement to be reliably identified in patients of all ages. Its usefulness for identifying maligned electrodes (partial insertion, and insertion into the vestibule or hypotympanum) was confirmed using four cadaveric hemi-heads. MAIN OUTCOME MEASURES: Ability to accurately identify correct or incorrect electrode insertion based on radiographic images. RESULTS: After adjusting radiographic exposure to account for the embalming process of the cadaveric heads, this new protocol was confirmed to be able to identify incorrect placement. This was also successfully used to confirm proper placement of cochlear implants in patients. CONCLUSIONS: Following a standardized radiographic protocol for cochlear implantation is a quick and easy method for checking electrode position.Supplemental Digital Content, http://links.lww.com/MAO/B253.

OBJECTIVE: To determine if there is a benefit of 2-stage cleft lip repair in regard to improving facial symmetry and facilitating definitive lip, nose, and palate repair. STUDY DESIGN: Retrospective chart review of patients born with complete, unilateral cleft lip deformity that underwent a two-stage repair described as a stage 1 straight line repair and a stage 2 modified Millard repair, for which a complete set of records, and peri-operative and post-operative photos were available. All cases were performed by a single surgeon. SETTING: Tertiary care center craniofacial team. METHODS: Measurements were taken from intraoperative, perioperative, and postoperative images of patients before and after each stage. Ratios were then created comparing the affected size to the unaffected side, and these were averaged between observers. RESULTS: A 19% increase in the width of area of the presumptive C flap was obtained between the unrepaired and the post-stage I images. The nostril width of the cleft side was 1.2× the width of the unaffected side, demonstrating a 140% decrease in nostril width at the completion of stage II. The cleft side nostril width was maintained slightly larger than the noncleft side as desired. Symmetry of the upper lip length was achieved, as the length of the cleft side lateral lip after stage II was 92% of the unaffected side. CONCLUSION: We believe this study provides evidence for our observations that a two-stage repair can be performed with functionally and aesthetically pleasing outcomes as an alternative to presurgical nasoalveolar molding.

Metallothioneins (MTs) have been identified in a wide variety of organisms from bacteria to humans. The biological functions of these MTs have a key role in metalloregulatory metabolism and its expression is induced in response to different stimuli, particularly by divalent metal cations. Also, the action of MTs have been implicated in the survival of pathogens in presence of microbicidal concentration of divalent cations, which allows the establishment of the infection. Trichomonas vaginalis is a protozoan parasite that adapts to the microenvironment of the male urogenital tract, where cations such as zinc (Zn2+) and cadmium (Cd2+) are present. Nevertheless, the molecular mechanisms of metal tolerance and homeostasis is not yet dilucidated in this parasite. In this study, we have identified 4 potential MT-like sequences (tvmt´s) in T. vaginalis genome. Because tvmt-2, -3, and -4 corresponds to truncated partial genes, we characterized the trichomonad tvmt-1 gene. The bioinformatic analyses and the predicted protein (TvMT-1) show similar properties to the reported in other MTs. The expression patterns of tvmt-1 in the presence of several divalent cations (Fe2+, Mn2+, Zn2+ and Cd2+) were analyzed and we demonstrated that Cd2+ induce significantly their expression. By indirect immunofluorescence assays, we corroborated this positive regulation of TvMT-1 in the cytoplasm of parasites grown in the presence of Cd2+. The tvmt-1 promoter contains putative metal responsive elements, which are probably the responsible for the Cd2+-dependent expression of this gene. Our results suggest that tvmt-1 gene encode a metallothionein that may be responsible for the homeostatis and detoxification of Cd+2 in T. vaginalis.

Reactivation of Kaposi's sarcoma-associated herpesvirus (KSHV) from latency requires the viral transactivator Rta to contact the host protein Jκ recombination signal-binding protein (RBP-Jκ or CSL). RBP-Jκ normally binds DNA sequence-specifically to determine the transcriptional targets of the Notch-signaling pathway, yet Notch alone cannot reactivate KSHV. We previously showed that Rta stimulates RBP-Jκ DNA binding to the viral genome. On a model viral promoter, this function requires Rta to bind to multiple copies of an Rta DNA motif (called "CANT" or Rta-c) proximal to an RBP-Jκ motif. Here, high-resolution ChIP/deep sequencing from infected primary effusion lymphoma cells revealed that RBP-Jκ binds nearly exclusively to different sets of viral genome sites during latency and reactivation. RBP-Jκ bound DNA frequently, but not exclusively, proximal to Rta bound to single, but not multiple, Rta-c motifs. To discover additional regulators of RBP-Jκ DNA binding, we used bioinformatics to identify cellular DNA-binding protein motifs adjacent to either latent or reactivation-specific RBP-Jκ-binding sites. Many of these cellular factors, including POU class homeobox (POU) proteins, have known Notch or herpesvirus phenotypes. Among a set of Rta- and RBP-Jκ-bound promoters, Rta transactivated only those that also contained POU motifs in conserved positions. On some promoters, POU factors appeared to inhibit RBP-Jκ DNA binding unless Rta bound to a proximal Rta-c motif. Moreover, POU2F1/Oct-1 expression was induced during KSHV reactivation, and POU2F1 knockdown diminished infectious virus production. Our results suggest that Rta and POU proteins broadly regulate DNA binding of RBP-Jκ during KSHV reactivation.
