RESUMEN

Hepatitis E virus (HEV) infection is one of the enterically transmitted types of hepatitis. The present study was undertaken to estimate the occurrence of HEV infection in sporadic acute hepatitis in Thailand. Serum samples were obtained from 614 suspected acute hepatitis patients at two large hospitals in Bangkok during 2008, 2009, and 2011. Acute hepatitis E was identified by the presence of anti-HEV IgM (4.8%) using indirect ELISA kits and/or HEV RNA (4.5%) by a semi-nested reverse transcription-polymerase chain reaction assay. HEV IgM was the most common marker for detection (77%) at diagnosis, either by positive HEV IgM alone or together with HEV RNA, whereas HEV RNA alone was detected in 23% of patients. Overall, 4.2% of cases (26 out of 614) were acute HEV infection with the highest attack rate in the elderly age group. In addition, nucleotide sequence analysis of five HEV samples revealed 92.8-99.8% homology. All viruses were clustered into HEV genotype 3 and were similar genetically to swine HEV strains previously detected in the same area. Therefore, the occurrence of HEV infection with closely related to swine genotype 3 was approximately 4-5% of acute hepatitis cases in Thailand. Anti-HEV IgM was the most common marker at diagnosis.

A 1995 outbreak of dengue haemorrhagic fever (DHF) occurred on Samui Island in Thailand with an incidence of almost 500 cases/100,000 population. To find and develop effective strategies to control this disease through cost-effective vector control programs, entomological studies were carried out on the island between 1996 and 1998. There were two species of DHF vectors, Aedes aegypti and Ae. albopictus prevailing on the island, and the population of Ae. aegypti remained relatively constant throughout the year while the abundance of Ae. albopictus increased substantially during the rainy season (May-December) and then declined drastically in the dry season (January-April). The ranges of the three Aedes larval indices, Breteau index (BI), house index (HI) and container index (CI) were 93-310, 43-89 and 16-50 respectively. The ceramic or earthen jars both inside and outside the dwellings and concrete water storage tanks (mostly in toilets and bathrooms) served as the main breeding places of Ae. aegypti whereas coconut husks and coconut floral spathes found outdoors were the major breeding sites of Ae. albopictus. The number of washing water jars, concrete tanks and natural sites infested with Aedes larvae increased significantly in rainy season, with 60% of ovitraps become positive for Ae. albopictus eggs with an average number of 26 eggs/trap in 3 days of setting. There was a complete lack of oviposition by Ae. aegypti in outdoor ovitraps (15 m away from the houses). The indoor biting rate ranged from 1.5 to 8.1 mosquitoes/man-hour, while the outdoor rate was between 5 and 78 mosquitoes/man-hour. Of the indoor biting mosquitoes, 75.4% were identified as Ae. aegypti and 99% of the outdoor ones were Ae. albopictus. The diel biting activity of Aedes during the period from 0800 h to 1700 h in the houses was higher in the morning than in the afternoon period, with a low prevalence between 1300 h and 1400 h.

One hundred and thirty-nine strains of Cryptococcus neoformans were isolated in Thailand from clinical specimens including 97 AIDS patients: 67 from Northern, 48 from Central, 17 from Northeastern and 7 from Southern regional hospitals. Six out of the 139 strains were serotype B and the remaining 133 were A. There was no correlation between serotypes and regional distribution. To our knowledge, this is the first report of serotyping studies on C. neoformans in Thailand. Studies on random amplified polymorphic DNA (RAPD) analysis showed that this method is useful for the differentiation of C. neoformans var. gattii (serotypes B and C) and C. neoformans var. neoformans (serotypes A and D). They also indicated that Thai isolates of C. neoformans var. gattii (serotype B) were a homogeneous group on the basis of their genotypes. Antifungal susceptibility tests using 5 antifungal agents including amphotericin B, fluconazole, flucytosine, itraconazole and micronazole against 50 selected strains of C. neoformans showed that they were sensitive to all of the antifungal agents tested except for one strain that was resistant to flucytosine.

RESUMEN

The in vitro activity of D0870, a new triazole, was compared with that of fluconazole, itraconazole, miconazole, amphotericin B and flucytosine against recent clinical isolates of Penicillium marneffei in Thailand. The minimum inhibitory concentrations (MIC) values were determined by a microbroth dilution method using morpholinopropanesulphonic acid (MOPS)-buffered RPMI-1640 and brain heart infusion (BHI) medium. Yeast nitrogen base (YNB) medium with glucose was also used for the assay of flucytosine. D0870 was less active against P. marneffei than itraconazole, but its activity was similar to that of miconazole, superior to that of amphotericin B and markedly superior to that of fluconazole and flucytosine. Much lower MIC values of D0870 were observed with BHI medium.

Bacillus cereus has been classified into 23 types by immunochemical analyses of flagella antigen, but a common antigenic determinant of flagella had not been determined. When the immunochemical method of classification had changed from "agglutination method" to "enzyme-linked immunosorbent assay (ELISA)," the cross-reactivity between each flagella type was increased. These results indicated that the application of ELISA method would enable detection of the common antigenic determinant of B. cereus. Therefore, we attempted to make monoclonal antibody against common flagella antigen that could be detected by ELISA. Monoclonal antibody provided was specific for B. cereus (H-1 to H-23) and did not show the cross-reactivity with Escherichia coli NIH and Bacillus subtilis.
