RESUMEN

The Buruli ulcer (BU) rages in a lot of tropical and subtropical countries. In Democratic Republic of Congo, some BU cases were reported between the 50s and 70s. This disease offers resistance to the usual chemotherapy. The only alternative for the treatment remains the surgical one (trimming, wound dressing, graft). The overarching aim of this study, conducted in the province of Lower Congo, is to put forward a surgical therapeutic approach adapted to the precarious conditions met on the ground in our rural areas. The evidence of a Mycobacterium ulcerans infection has been bacteriological and histopathological. The slight skin autograft or "of Davis", in open air or with dressing was performed in 37 patients. After three months this skin autograft in open air has experienced 78.5% of complete healing against 70% for the skin autograft with dressing. The exposure of skin graftings to open air favors an hyperoxygenation which also fights against the Mycobacterium ulcerans, for this latter develops better in an hypoxical area.

The electromyogram (EMG) median power frequency of the calf muscles was investigated during an exhausting treadmill exercise and a 20-min recovery period. The exercise was an uphill run at a speed of 5 km.h-1 and a gradient of 20%. During exercise there was no decrease of EMG median power frequency. In contrast, EMG median power frequency in isometric contractions of the same muscles decreased by 7% for the soleus muscle and 16 to 18% for gastrocnemius muscles immediately after the exercise. During the recovery period the isometric median power frequency of the gastrocnemius muscles increased to pre-exercise levels in about 5 min. The isometric median power frequency of the soleus muscle also increased but had not reached pre-exercise values by 20 min. The observations from this study and from a previous uphill treadmill investigation at a steeper gradient gave evidence that two types of exhaustion can be distinguished during dynamic exercise; exhaustion at lower exercise intensities without a decrease in frequency during exercise and exhaustion at high intensities accompanied by a decline of frequency. The reason for this difference remains unclear.

Par des methodes simples d'extraction; de chromatographie sur colonne de SEPHADEX de filtration G50; et par des reactions colorees; les auteurs ont mis en evidence la presence unique de phytosterols dans la fraction ethanolique (F2) d'un extrait foliaire de mitracapus verticillatus; valke; (Rubiacee). L'activite antifongique de ces phytosterols a ete testee sur cryptococcus neoformans; un germe fongique implique dans la redoutable meningite a cryptocoque. La culture du cryptocoque sur agar Sabouraud/chloramphenicol incorpore a F2 en double dilution a donne une concentration minimale fongique (CMF) egale a 12 gammas (12 ug/ml) quand l'extrait total (Et) donne 768 ug/ml et sa fraction 80acqueuse (F1) 2500 ug/ml. Un dosage chimique des ions a revele egalement la presence d'iode (I-) et de baryum (Ba++)qui confirme le pouvoir antiseptique et medical de la fraction (F2)

EMG median power frequency of the calf muscles was investigated during an exhausting treadmill exercise. This exercise was an uphill run, the average endurance time was 1.5 min. Median power frequency of the calf muscles declined by more than 10% during this exercise. In addition EMG median power frequency of isometric contractions of the same muscles was measured before and in one minute intervals for 10 min after this run. Immediately after the run isometric median power frequency had declined by less than 5% for the soleus muscle, more than 10% for the gastrocnemius medialis and gastrocnemius lateralis muscles. In the 10 min following exercise the isometric median power frequency increased to pre-execise levels. Maybe the median power frequency shift to lower frequencies during dynamic exercise can be interpreted as a sign of local muscle fatigue.
