RESUMEN

In orthodontics, incisor overbite has always been considered as an anomaly difficult to correct but also as the one most hindering the solving of the problems resulting from other associated malpositions. The recent concept of unlocking, introduced by the bioprogressive School, proves that the profession has become aware of its importance in any orthodontic treatment plan. Due to the fact that overbite also hinders the setting up of inferior brackets or forces the practitioner to bond them in a position liable to burden the parodontal health of the lower incisors has therefore induced the author to put in place, at the beginning of the treatment with the Tip-Edge technique, an anterior bite raiser thus generating space in the lateral sectors. Considering how fast the anterior problem is solved once the occlusion is lifted, the bite raiser can be suppressed within three months. Another advantage resides in the possible adjunction of an expansion screw also aimed at unlocking the occlusion in transverse direction. The question then raised in to know whether that approach which ought to be compared to the one of the functional appliances is not the ideal solution to unlock the occlusion in the three dimensions of space when the use of fixed appliances alone might be unsuitable or too slow, regarding current therapeutic aims. Attributing that spectacular therapeutic result to an incisor intrusion or a molar extrusion is of little interest as far as those alterations do not lead to an increase of the vertical dimension of the lower portion of the face. A cephalometric study, published concomitantly, has evidenced that the use of an anterior bite raiser together with the Tip-Edge technique only leads to perfectly similar alterations to those observed with the Ricketts bioprogressive technique or the Schudy edgewise technique, without the help of a bite raiser. From now on, integrating the bite raiser in our therapeutic armamentarium in conjunction with fixed techniques is highly recommended.

If incisor overbite in the matter of orthodontic treatment entails difficulties with the appliances due to excessive overbite of the incisors, involving an anterior bite raiser to enable the unlocking of the occlusion and the bonding of the brackets on the anterior mandibular teeth poses the problem of the vertical behavior of the masticatory apparatus during treatment. A profile teleradiograph was therefore taken at the beginning of treatment and again once the occlusion had been lifted due to the presence of the bite raiser and within a period of three months. The sample was separated in two groups according to the value of FMA; a first group was composed of 44 cases exhibiting an FMA angle inferior to 25 degrees therefore similar to the cases analyzed by Dake and Sinclair in 1989, called "reference group" and a group of cases exhibiting an FMA angle superior to 25 degrees. The aim of this study is indeed to confront, as regards vertical behavior, the therapeutic approach of the authors with the one studied by Dake and Sinclair dealing with cases treated with Ricketts and Tweed technique (Schudy modified). Following Dake and Sinclair's approach, the authors managed to find out in cases with FMA inferior to 25 degrees that the vertical alterations in the study group were not different from those in the reference group. This means that in spite of the presence of an anterior bite raiser the mandibular plane angle had only increased by 1.8 degree, compared to 1.8 degree for the Ricketts group and 1.1 degree for the Tweed/Schudy group. As for the study sample with an FMA angle superior to 25 degrees, the vertical alterations in the study group show an increase of the angle of the mandibular plane equal to 1.2 degree, here again the increase is similar to the one observed in the reference group. It can thus be concluded that the use of an anterior bite raiser in conjunction with the Tip-Edge technique is not only advisable but strongly recommended both to unlock the occlusion but also to enable bracket bonding at the very beginning of treatment.

RESUMEN

The body posture has been studied according to the craniofacial morphology. A cephalometric analysis (lateral cephalometric radiographs) and a stabilometric recording of the posture with a vertical force platform have been done for 45 individuals: 17 boys and 28 girls aged 9 to 16 years. Cephalometric sagittal and transverse variables show no correlation with the sagittal position of the center of foot pressure. It seems that mild to moderate dentofacial deformities do not reflect on the posture of the body as a whole.

Nine clinical centers using the Brånemark System participated in a prospective study of 159 partially edentulous patients between 18 and 70 years of age. Clinical parameters evaluated were Plaque Index, gingivitis, pocket depth, Bleeding Index, tooth mobility, prosthesis stability, and stomatognathic function. Change in bone height around fixtures was measured radiographically. After 3 years, 460 loaded fixtures, supporting 174 prostheses in 139 patients, remain in the study. The complications, failures, and technical problems are presented. After 3 years of this 5-year study, results suggest that implant-based treatment of partially edentulous patients may achieve a success rate comparable to that obtained in edentulous patients.

RESUMEN

A new curriculum with a common contents of 75%, leaving 25% for electives has been developed for a three year programme for specialty education in orthodontics. The result is a joint effort of the original eight participants and invited professors from Italy and Ireland as well as from five non-EEC countries (Austria, Finland, Norway, Sweden and Switzerland). Unanimity among the 15 participants was reached on all essential matters and on the contents of the final report. The main objective of the programme has been defined and the different courses have been formulated in terms of goals to be accomplished in three levels of comprehension and by the minimal hours students must devote to each subject. Furthermore, procedures that should be mastered have been specified.

RESUMEN

A boy with MCA/MR and a fragile site (FS) at 8q22 opens the discussion of a possible association between a rare autosomal FS and an abnormal phenotype. The child was born after prenatal diagnosis of ureterohydronephrosis. He had facial dysmorphia and mental retardation (IQ = 40). The karyotype showed 8q22 FS in 12% of the cells obtained after addition of FUdR to the culture medium. No other etiologic factor was shown to be responsible for the MCA/MR syndrome. Several authors have reported a variety of neurodevelopmental abnormalities and mental retardation in individuals with rare FS expressed on chromosomes 2, 9, 10, 16 and 19. If rare FS predispose to phenotypic abnormalities what are the mechanisms?

This longitudinal study of 39 patients who underwent treatment involving osseointegrated implants examined problems in oral and psychosocial functioning, expectations and experiences of difficulties with surgery, satisfaction with surgery, body image, neuroticism, self-concept, and extroversion. Patients completed six questionnaires from before phase 1 surgery to the final recall appointment for the new prosthesis (12 to 18 months after phase 1 surgery). The most common problems reported before treatment were those associated with eating; esthetics was less of a concern. Significant improvements in all problem areas were observed immediately after phase 2 surgery. Expectations of surgery-related problems were generally consistent with experiences immediately after phase 1 surgery, but more negative than experiences following phase 2 surgery. Body image before treatment was most negative vis-à-vis teeth. Significant improvements were found not only regarding teeth, but also on facial, mouth, and even overall body image. Satisfaction scores were generally high, but showed continued improvements through the final assessment. The only group experiencing negative outcomes consisted of patients scoring high on neuroticism.

Nine clinical centers using the Brånemark System participated in a prospective study of 159 partially edentulous patients between 18 and 70 years of age. Clinical parameters evaluated were plaque index, gingivitis, pocket depth, bleeding index, tooth mobility, and stomatognathic function. Initially, 558 fixtures were placed and 521 remained in the study following prosthesis placement (199 prostheses in 154 patients). Fixtures were lost or unaccounted for because of nonintegration prior to prosthesis fabrication (19), patient withdrawal (11), prosthodontic reasons (6), and failure during prosthetic procedures (1). Failure was primarily attributable to unfavorable bone quality, sex (more in males), and smaller fixture size. Complications and failure related to other patient characteristics are presented. After 1 year of a 5-year study, preliminary results suggest that a success rate equal to or better than that obtained with edentulous patients may be expected.

A detailed description of the laboratory processing procedures for Molloplast-B-lined dentures that varies from those offered by the manufacturer has proved to be effective. The processing technique for Molloplast-B-lined dentures is very sensitive, consequently, no steps should be omitted. Because these procedures are time-consuming, the minimal time required is 3 full working days in the laboratory.
