INTRODUCTION: Colorectal cancer (CRC) ranks as the second leading cause of cancer-related deaths. The PREHAB trial revealed that prehabilitation in colorectal surgery leads to a reduction of severe complications and enhanced functional capacity. Nevertheless, risk selection for prehabilitation and the potential benefits for patients without postoperative complications remains unclear. This study aims to assess postoperative functional capacity, also in patients without postoperative complications. MATERIALS & METHODS: This study was a secondary analysis of the PREHAB trial. Functional capacity tests, including cardiopulmonary exercise testing (CPET), steep ramp test (SRT), 6-min walking test (6MWT), stair climb test (SCT), 30" sit-to-stand test (STS), timed-up-and-go test (TUG), and muscle strength assessments, were conducted at baseline (T0) and 4 weeks postoperatively (T3). The primary outcome was the relative change in functional capacity from baseline to postoperative (ΔT0-T3) per group (i.e., prehabilitation vs control). Secondary, identical analysis were performed for patients without postoperative complications in each group. RESULTS: Intention-to-treat analysis included 251 patients. For postoperative functional capacity, prehabilitation patients showed improvements in VO2peak (p = 0.024), VO2AT (p = 0.017), SRT (p = 0.001), 6MWT (p = 0.049), SCT (p = 0.012), and STS (p = 0.001) compared to the control group. Regarding muscle strength, prehabilitation patients showed improvements in estimated 1RM lateral pull down (p = 0.016), 1RM chest press (p = 0.001), 1RM leg press (p = 0.001) and HGS (p = 0.005) compared to controls. Additionally, prehabilitation patients more often reached baseline levels at T3 in VO2AT (p = 0.037), SRT (p = 0.008), 6MWT (p = 0.013), STS (p = 0.012), estimated 1RM lateral pull down (p = 0.002), 1RM chest press (p = 0.001) and 1RM leg press (p = 0.001) compared to controls. Moreover, even patients without postoperative complications in the prehabilitation group showed better postoperative functional capacity and more often reached baseline levels at T3, compared to controls. CONCLUSION: Multimodal prehabilitation in CRC surgery is associated with improved postoperative functional capacity, even in patients without postoperative complications.

SIGNIFICANCE: Partial improvement in stereoacuity may be achieved by balancing the contrast input to the two eyes of patients with bilaterally asymmetric keratoconus. PURPOSE: Interocular differences in image quality, characterized by dissimilar contrast loss and phase shifts, are implicated in stereoacuity loss in keratoconus. This study determined whether contrast balancing improves stereoacuity in this disease condition and, if so, whether it is dependent on the baseline interocular contrast imbalance. METHODS: Interocular contrast imbalance and stereoacuity of 43 subjects (16 to 33 years) with bilaterally asymmetric keratoconus were tested with spectacle correction as baseline using a binocular rivalry paradigm and random-dot stereograms, respectively. Stereoacuity measurements were repeated in a subset of 33 subjects at their contrast balance point (i.e., contrast level in stronger eye allowing balanced rivalry with 100% contrast in weaker eye) and with contrast levels biased in favor of stronger or weaker eye, all conditions in randomized order. RESULTS: Contrast imbalance level was significantly correlated with the subject's stereoacuity at baseline ( r = -0.47, P = .002). The median (25th to 75th interquartile range) stereoacuity improved by 34.6% (19.0 to 65.1%) from baseline (748.8 arc sec [261.3 to 1257.3 arc sec]) to the contrast balanced condition (419.0 arc sec [86.6 to 868.9 arc sec]) ( P < .001), independent of their baseline stereoacuity or contrast imbalance levels ( r < 0.2, P > .26 for both). Contrast bias in favor of the weaker eye (881.3 arc sec [239.6 to 1707.6 arc sec]) worsened stereoacuity more than a bias toward the stronger eye (502.6 arc sec [181.9 to 1161.4 arc sec]), both relative to the contrast balanced condition ( P < .002). CONCLUSIONS: Interocular contrast balancing partially improves stereoacuity in bilaterally asymmetric keratoconus, independent of their baseline contrast imbalance level. Cyclopean viewing may be inherently biased toward the input from the stronger eye in keratoconus.

PURPOSE: Optical imperfections of the eye, characterized by higher-order wavefront aberrations, are exaggerated in corneal disease (e.g., keratoconus) and iatrogeny (e.g., keratorefractive surgery for myopia correction, keratoplasty for optical clarity restoration). This article reviews the recent advances on this topic for a comprehensive understanding of how optical degradations in disease models impact retinal image quality and monocular and binocular visual performance. METHODS: Published literature over the last decade on retinal image quality and/or monocular and binocular visual functions with corneal irregularity was reviewed based on their relevance to the current topic, study population and strength of study design. The literature was summarized into four themes: 1) wavefront errors and retinal image quality of highly aberrated eyes, 2) monocular and binocular vision loss consequent to degraded optics and visual strategies to optimize performance, 3) impact of optical correction modalities on visual performance and 4) implications for clinical management of patients. RESULTS: Across the 46 articles reviewed, the results clearly indicated that an increase in higher-order aberrations across these conditions had a significant negative impact on the patient's retinal image quality, and monocular and binocular visual functions. Interocular differences in retinal image quality deteriorated visual performance more than an overall worsening of image quality bilaterally. Minimizing optical degradation using rigid contact lenses and adaptive optics technology significantly improves retinal image quality and monocular and binocular vision, but performance remains sub-optimal relative to age-similar healthy controls. CONCLUSION: Corneal disease and iatrogeny are useful models to understand the impact of optical degradation on retinal image quality and visual performance. Clinical management will greatly benefit from equalizing retinal image quality of both eyes of these patients. Future studies that deepen our understanding of the structure-function relation in these conditions are desirable for advancing vision science in this area and for developing novel clinical management strategies.

Purpose: Keratoconus results in image quality loss in one or both eyes due to increased corneal distortion. This study quantified the depth of monocular suppression in keratoconus due to this image quality loss using a binocular contrast rivalry paradigm. Methods: Contrast rivalry was induced in 50 keratoconic cases (11-31 years) and 12 age-matched controls by dichoptically viewing orthogonal Gabor patches of 5 cycles per degree (cpd) and 1.5 cpd spatial frequency for 120 seconds with their best-corrected spectacles and rigid gas permeable (RGP) contact lenses. The dwell time on each eye's percept was determined at baseline (100% contrast bilaterally) and at varying contrast levels (80-2.5%) in the stronger eye of keratoconus or dominant eye of controls. The contrast reduction needed in the stronger eye to balance dwell times on both eyes was considered a measure of suppression depth. Results: At baseline with 5 cpd stimuli and spectacle correction, the rivalry switches were less frequent and biased toward the stronger eye of cases, all relative to controls (P < 0.001). The contrast balance point of cases (20.51% [10.7-61%]) was lower than the controls (99.80% [98.6-100%]; P < 0.001) and strongly associated with the overall and interocular difference in disease severity (r = 0.83, P < 0.001). The suppression depth reduced for 1.5 cpd (70.8% [21.7-94%]), relative to 5 cpd stimulus (P < 0.001) and with contact lenses (80.1% [49.5-91.7%]), relative to spectacles (P < 0.001). Conclusions: The eye with lesser disease severity dominates binocular viewing in keratoconus. The suppression depth of the poorer eye depends on the extent of bilateral disease severity, optical correction modality, and the target spatial frequency.

Sex workers have been one of the marginalized groups that have been particularly affected by India’sstringent lockdown in response to the coronavirus disease 2019 (COVID-19) pandemic. The suddenloss of livelihood and lack of access to health care and social protection intensified the vulnerabilities ofsex workers, especially those living with HIV. In response, Ashodaya Samithi, an organization of morethan 6000 sex workers, launched an innovative programme of assistance in four districts in Karnataka.Since access to antiretroviral therapy (ART) was immediately disrupted, Ashodaya adapted its HIVoutreach programme to form an alternative, community-led system of distributing ART at discreet,private sites. WhatsApp messaging was used to distribute information on accessing government socialbenefits made available in response to the COVID-19 pandemic. Other assistance included advisorymessages posted in WhatsApp groups to raise awareness, dispel myths and mitigate violence, andregular, discreet phone check-ins to follow up on the well-being of members. The lessons learnt fromthese activities represent an important opportunity to consider more sustainable approaches to thehealth of marginalized populations that can enable community organizations to be better prepared torespond to other public health crises as they emerge.

BACKGROUND: Serratiopeptidase is an alkaline metalloendopeptidase, which acquired wide significance because of its therapeutic applications. The present study was undertaken for purification, characterization, and structural elucidation of serratiopeptidase produced from Streptomyces hydrogenans var. MGS13. RESULT: The crude enzyme was purified by precipitating with ammonium sulfate, dialysis, and Sephadex gel filtration, resulting in 34% recovery with a 12% purification fold. The purified enzyme S.AMP13 was spotted as a single clear hydrolytic band on casein zymogram and whose molecular weight was found to be 32 kDa by SDS-PAGE. The inhibitor and stability studies revealed that this enzyme is metalloprotease, thermostable, and alkaline in nature. The maximum serratiopeptidase activity was observed at 37 °C and pH 9.0. The partial amino acid sequence of the purified enzyme S.AMP13 by LC-MS/MS analysis shows the closest sequence similarities with previously reported alkaline metalloendopeptidases. The amino acid sequence alignment of S.AMP13 shared a conserved C-terminus region with peptidase-M10 serralysin superfamily at amino acid positions 128-147, i.e., ANLSTRATDTVYGFNSTAGR revealed that this enzyme is a serralysin-like protease. The kinetic studies of the purified enzyme revealed a Km of 1 mg/mL for its substrate casein and Vmax of 319 U/mL/min. The 3D structure of the purified enzyme was modeled by using SWISS-MODEL, and the quality of the structure was authenticated by assessing the Ramachandran plot using PROCHECK server, which suggested that the enzyme was stable with good quality. CONCLUSION: Inhibitor, stability, electrophoretic, and bioinformatic studies suggested that the purified enzyme obtained from S. hydrogenans var. MGS13 is a serralysin-like protease.

Introduction and Purpose: The aim of this study is to evaluate binocular vision parameters such as near point of accommodation (NPA), near point of convergence (NPC), fusional vergence ranges in subjects with orthotropic Duane retraction syndrome (DRS) and to compare with normal age-matched controls Patients and Methods: A total of 24 subjects (14 Type 1, 2 Type 2, 8 Type 3 DRS) who were diagnosed to have DRS and 15 normals were included in the study. The following binocular vision parameters were assessed in all subjects: amount of the deviation (for near and distance in all gazes), NPC, NPA, positive and negative fusional vergence (PFV and NFV) reserve and stereopsis. Results: Accommodative amplitude was normal in subject with DRS (median (95% confidence interval (CI):15D (11.9-18.1)) compared to age-matched controls. However, NPC was significantly receded (median [95% CI]:17 cm [14.4-19.6]). PFV reserve and NFV reserve were reduced in subjects with DRS. Subjects with Type 3 DRS have poor PFV reserve and NFV reserve for distance when compared to Type 1 DRS (p = 0.009 and 0.006 respectively). Stereopsis was poor in the subjects with DRS (p < 0.001). Conclusion: Subjects with DRS have good accommodation, but have poor convergence and fusional vergence. Stereopsis was noted to be poor as well.

The presence and the relative amount of 4-amino-L-arabinose in lipopolysaccharides of members of the Enterobacteriaceae family and in a single strain of Chromobacterium violaceum has been studied with regard to growth-temperature dependent variations. Changes in the presence and the amount of 4-amino-L-arabinose (4-AA) were observed in almost all cases, but the variations observed were not consistent among different species. While Salmonella minnesota and Proteus mirabilis showed higher levels of incorporation at higher temperatures, the S- and R-forms of Yersinia enterocolitica showed the opposite effect, i.e. only marginal incorporation by growth at 10 degrees C. Chromobacterium violaceum, however, showed no significant alteration in the 4-amino-L-arabinose content when growth either at 14 or at 37 degrees C. DOC-PAGE pattern of isolated lipopolysaccharides showed characteristic profiles indicating that the O-chain-synthesis of distinct Enterobacteriaceae is also differently influenced by changes in growth temperature.
