RESUMEN

The influence of "running" impulse magnetic field, antianginal drugs and their combination on physical load tolerability, hemodynamics and functional state of hormonal system in patients with stable angina pectoris was studied. "Running" impulse magnetic field therapy produced marked antianginal effect in patients with I-II class angina and it was effective in combination with antianginal drugs in patients of III class angina. The reduction of attack frequency and significant physical load tolerability improvement was determined. The correcting influence of "running" impulse magnetic field on hypophysis-thyroid system hormones (TTH, T3, T4) was revealed, that correlates with physical load tolerability and myocardial contractility improvement. As monotherapy, the "running" impulse magnetic field can be administered to the patients with stable angina of I-II class and in combination with antianginal drugs- to the patients with severe angina.

RESUMEN

The effects of the running pulse magnetic field, antianginal drug therapy and their combinations on the physical capacity were compared in 60 patients with Functional Classes I-III stable angina pectoris. Monotherapy with the running pulse magnetic field was found to produce an antianginal effect in patients with Functional Classes I-II angina pectoris, but the efficacy of drug therapy increased when antianginal drugs were used in combination with running pulse magnetic field in patients with severe angina.

The blood flow in the gastrocnemius muscle of healthy men was studied during 6-hour antiorthostatic hypokinesia at different angles (-7, -15, -22, -30 degrees) and also the possibility of correcting changes in hemodynamics during antiorthostatic hypokinesia was determined. The muscle blood flow was studied by the clearance method. A total of 72 tests were performed. The tested men received obsidan and octadine 30 minutes before taking the antiorthostatic position in doses of 80 and 12.5 mg for one intake respectively. The analysis of the clinical data showed that typical symptomatology develops during antiorthostatic hypokinesia with a significant decrease of the tissue blood flow in the gastrocnemius muscle at an increase of the angle of tilt. The results of the studies suggested that obsidan fails to improve the muscle blood flow and octadine leads to a greater slowing of the muscle blood flow that negatively influences the condition of the gastrocnemius muscle.

Plasma renin activity and aldosterone levels were measured radioimmunologically in 30 normal individuals subjected to antiorthostatic hypokinesia (at a - 15 degrees angle). Hormonal effects of some drugs (beta-blockers, trental, venotonics) were examined. The short-term antiorthostatic position was associated with a disordered hormonal control, reduced plasma aldosterone and enhanced plasma renin activity, as compared to the respective parameters of the reclining position. The use of beta-adreno-blockers alone or in combinations with other drugs decreases plasma renin activity in conditions of antiorthostatic hypokinesia which can be related to the inhibition of beta-adrenergic activity.

From the results of the examination of 176 patients with hypertensive disease the authors compared 49 signs, including sex, age, the findings of medical history, inspection, X-ray, ECG and consultations of an ophthalmologist with the power of physical exertion on a bicycle ergometer. Fifteen signs with a marked correlative connection with the power of exertion (r greater than 0.5) were distinguished. Systolic arterial pressure (170 mm Hg and more), circulatory insufficiency, concomitant ischemic heart disease, atherosclerosis of the peripheral arteries and hypokinetic type of circulation proved to be the most important factors limiting the physical working capacity of patients with hypertensive disease. A table for prognosticating the working capacity was compiled.

An examination was conducted of 45 healthy individuals 23 to 28 years of age who received nitroglycerine sublingually in a dose of 0.5 mg (15 persons) and by intravenous drip for one hour in doses of 3, 10, and 20 mg (groups of ten persons). It was shown that the drug had no essential effect on the rate of cardiac contractions and the diastolic arterial pressure. The systolic arterial pressure decreased with the increase of the nitroglycerine dose. The trend and the severity of changes in central hemodynamics were, in most cases, inversely dependent on the volume of cardiac ejection and did not depend on the dose and route of administration of the drug. With a cardiac index of 3 to 5.5 1/min/m2, this index in 71% of individuals reduced by 26% on the average. With an initial cardiac index of 2 to 2.9 1/min/m2, in 63% of individuals this index increased by 27% on the average. The authors distinguish three types of the reaction of hemodynamics to the administration of nitroglycerine: hypo-, hyper-, and eucardiac.

RESUMEN

The article discusses the results of treatment of 38 patients suffering from ischemic heart disease with hemoclar given in a dose of 200 mg for 25-34 days. Ergometry was carried out and the values of lipid metabolism and creatine phosphokinase activity were studied before and after treatment. The drug caused noticeable decrease in the lipid level, mainly in types IIb and IV hyperlipoproteinemia. Medication with hemoclar was attended by a decrease in the frequency and strength of the anginal attacks and by a lesser increase in the activity of creatine phosphokinase during a load. No side-effects were recorded.

RESUMEN

Rheopneumographic examinations were conducted in 54 patients with chronic pneumonia (stage I and II pulmonary insufficiency) prior to and following exercise tests. It was demonstrated that in normal individuals physical exercises cause a compensatory increase of the pulmonary blood flow without elevating the pressure in the pulmonary circulation system. The progression of pulmonary insufficiency is accompanied by a reduction of the adaptation capacity of the heart, of the pulmonary vessels, by an increase of pulmonary hypertension, and impairment of the contractile function of the right ventricular myocardium. The severity of changes in the pulmonary system haemodynamics, as shown by rheopneumography, correlates with the disorders in the oxygen regimen (oxygen consumption and oxygen pulse).
