OBJECTIVE: To describe the prevalence of underweight and obesity indicators among individuals registered as traditional peoples and communities in the Food and Nutrition Surveillance System, across Brazil, in 2019. METHODS: This was a descriptive study using individual secondary data from participants receiving care in the Primary Health Care within the Brazilian National Health System. RESULTS: In the study population (N = 13,944), there was a higher prevalence of short stature among male children and adolescents (14.2%), when compared to their female counterparts (11.8%); in the adult female population, there was a higher prevalence of obesity (23.0%), when compared to the male population (11.3%); the prevalence of low height-for-age in riverine communities (18.5%) and obesity in the adult faxinalense population (75.1%) stood out. CONCLUSION: Anthropometric disparities between different communities require tailored responses, emphasizing targeted primary health care and programs to ensure food and nutrition security. MAIN RESULTS: It was possible to identify a higher prevalence of low weight and height-for-age in the child population of riverine communities, while faxinalense communities showed a higher prevalence of obesity in adults. IMPLICATIONS FOR SERVICES: Continuous improvement in the implementation of SISVAN protocols is recommended, as well as strengthening the assistance provided to communities with the worst indicators. PERSPECTIVES: Studies assessing the potential impact of interventions to mitigate the effects of malnutrition in communities, through in-depth qualitative research or probabilistic quantitative studies using primary data.

This study aimed to analyze whether community food environments are associated with individual food consumption among nutrition students and newly graduated nutritionists. This cross-sectional study used data from the Nutritionists' Health Study cohort, which included 357 undergraduate nutrition students from the city of Fortaleza, Ceará, Brazil. Exposure to the food environment was defined as the proximity and availability of food outlets within a 500 m buffer from the participants' homes. Food consumption was assessed using a Food Frequency Questionnaire and analyzed according to the NOVA classification. Multi-level linear regression models with fixed effects were used to estimate the presence of food outlets within the buffer and their association with food consumption. The presence of mini-markets in the buffer in the fourth quartile was associated with lower consumption of ultra-processed foods (UPF) when observing socioeconomic and lifestyle conditions (ß = -3.29; 95% CI = -6.39 to -0.19). The presence of bakeries and coffee shops was related to lower consumption of ultra-processed foods among participants when observing socioeconomic conditions (ß = -3.10; 95% CI = -6.18 to -0.02). There was no clear evidence of an association between the type of food outlet and UPF consumption. The community food environment seemed to influence food consumption among study participants, although clearer and more consistent evidence on this subject is needed.

The present study aimed to evaluate factors associated with adherence to the Food Guide for the Brazilian Population (GAPB) among food service workers. A cross-sectional study was conducted with 421 employees from 43 food service establishments located in a capital of Brazil. Health and lifestyle data were collected, including the continuous use of medication, smoking, physical activity, and alcohol consumption. An instrument based on the GAPB was utilized, covering the domains of planning, household organization, eating habits, and food choices, according to GAPB recommendations. The findings revealed a direct association between the quality of life scores and the total GAPB adherence score (ß = 1.17; p < 0.001), as well as the domains of planning (ß = 0.53; p < 0.001), household organization (ß = 0.22; p = 0.001), and eating habits (ß = 0.38; p < 0.001). Adherence to the GAPB among food service workers was strongly linked to their perception of quality of life, in addition to other factors such as gender, age, education, type of work activity, and variables of lifestyle. Therefore, the eating practices of these employees need to be assessed comprehensively, and enhancing their quality of life can encourage proper and healthy eating.

OBJECTIVE: to analyze the temporal trend of Food and Nutrition Surveillance System (Sistema de Vigilância Alimentar e Nutricional - SISVAN) coverage and the nutritional status of older adults, and its correlation with indicators of social inequality in Brazil between 2008-2019. METHODS: this was an ecological study using records from SISVAN, related to the population aged 60 years and older; the temporal trend of coverage and the correlation between indicators of social inequality and increment rate of nutritional status were analyzed; slope index of inequality and concentration index were used to measure absolute and relative inequalities. RESULTS: 11,587,933 records were identified; national coverage increased from 0.1% (2008) to 2.9% (2019), with a statistically significant upward trend; a moderate inverse correlation with an annual increment rate of overweight between human development index and gross domestic product per capita, was found. CONCLUSION: there was an increasing trend in SISVAN coverage; the increase in overweight was associated with social inequality.

Objetivo: Describir la prevalencia de indicadores de bajo peso y obesidad entre individuos registrados como pueblos y comunidades tradicionales en el Sistema de Vigilancia Alimentaria y Nutricional, en todo Brasil, en 2019. Métodos: Estudio descriptivo, con datos secundarios individuales de participantes atendidos en atención primaria de Salud del Sistema Único de Salud. Resultados: En la población estudiada (N = 13.944), hubo mayor prevalencia de talla baja en niños y adolescentes en el sexo masculino (14,2%), cuando se compara con el femenino (11,8%); en la población adulta femenina hubo mayor prevalencia de obesidad (23,0%), en comparación con la masculina (11,3%); se destacó la prevalencia de baja talla para la edad en comunidades ribereñas (18,5%), y obesidad en la población adulta faxinalense (75,1%). Conclusiones: Las heterogeneidades antropométricas entre comunidades requieren respuestas adaptadas, con énfasis en la atención primaria de salud y en garantizar la seguridad alimentaria y nutricional.


Objetivo: Descrever a prevalência de indicadores de baixo peso e de obesidade entre os indivíduos registrados como povos e comunidades tradicionais no Sistema de Vigilância Alimentar e Nutricional, de todo o Brasil, em 2019. Métodos: Estudo descritivo, com dados individualizados secundários de participantes atendidos na Atenção Básica do Sistema Único de Saúde. Resultados: Na população estudada (N = 13.944), houve maior prevalência de altura baixa entre crianças e adolescentes do sexo masculino (14,2%), quando comparados aos do sexo feminino (11,8%); na população adulta do sexo feminino, observou-se maior prevalência de obesidade (23,0%), quando comparada à do sexo masculino (11,3%); destacou-se a prevalência de altura baixa para a idade nas comunidades ribeirinhas (18,5%), e de obesidade na população adulta faxinalense (75,1%). Conclusões: As heterogeneidades antropométricas entre as comunidades exigem respostas adaptadas, com ênfase na atenção primária à saúde e na garantia da segurança alimentar e nutricional.

Objetivo: analizar la tendencia temporal de cobertura del Sistema de Vigilancia Alimentaria y Nutricional (SISVAN), y el estado nutricional de adultos mayores, correlacionándolos con indicadores de desigualdad social, en el período 2008-2019. Métodos: estudio ecológico mediante registros del SISVAN sobre la población ≥60 años. Se realizaron análisis de correlación entre indicadores de desigualdad social y la tasa de incremento del estado nutricional y análisis de desigualdades absolutas y relativas para obtener el índice de desigualdad angular y el índice de concentración. Resultados: se identificaron 11.587.933 registros. La cobertura nacional evolucionó del 0,1% en 2008 al 2,9% en 2019, con una tendencia ascendente estadísticamente significativa. Se encontró una correlación inversa moderada con la tasa de incremento anual de sobrepeso para IDH y PIB per cápita. Conclusión: hubo una tendencia de crecimiento en la cobertura del SISVAN. El aumento del sobrepeso se asoció con la desigualdad social.

Resultados: foram identificados 11.587.933 registros de idosos; a cobertura nacional evoluiu de 0,1% (2008) para 2,9% (2019), com tendência de aumento estatisticamente significativa; foi encontrada correlação inversa moderada com taxa de incremento anual de sobrepeso, para índice de desenvolvimento humano e produto interno bruto per capita. Conclusão: houve tendência de crescimento da cobertura do SISVAN; o aumento de sobrepeso esteve associado à desigualdade social.

ABSTRACT Objective Characterize the community food environment through the different types of food outlets in the city of Fortaleza and associate their distribution according to sociodemographic indicators. Methods This is an ecological study carried out in the city of Fortaleza in which data from the Health Surveillance Service were used with the location of all licensed food stores in the city in the years 2018 and 2019. Georeferenced maps were set up to illustrate the spatial distribution of the establishments. Correlation analyses were performed to verify the association between food outlets and socioeconomic data. Values of p≤0.005 were considered significant. Results We identified a greater concentration of food stores in the neighborhoods with better socioeconomic levels. Snack bars (n=2051; 27.7%) and restaurants (n=1945; 26.3%), were in greater quantity and exhibited a positive correlation with the Human Development Index and average income. Supermarkets and hypermarkets (n=288; 3.9%) and street markets (n=81; 1.1%) were in a smaller number and had the worst spatial distribution. Conclusion We observed socioeconomic inequalities in the distribution of different types of food outlets. The little diversity and the limited number of establishments in peripheral neighborhoods, besides the centralization of outlets that sell food that is harmful to health, constitute obstacles for the population to make healthy food choices.


RESUMO Objetivo Caracterizar o ambiente alimentar comunitário por meio dos diferentes tipos de estabelecimentos de venda de alimentos existentes na cidade de Fortaleza e relacionar sua distribuição de acordo com indicadores sociodemográficos. Métodos Trata-se de um estudo ecológico realizado na cidade de Fortaleza em que foram utilizados dados da Vigilância Sanitária com a localização de todos os comércios de alimentos licenciados para funcionamento no município nos anos de 2018 e 2019. Foram construídos mapas georreferenciados para ilustrar a distribuição espacial dos estabelecimentos. Análises de correlação foram realizadas para verificar associação entre os estabelecimentos de venda de alimentos e dados socioeconômicos. Consideraram-se significativos valores de p≤0,005. Resultados Foi possível identificar maior concentração de comércios de alimentos nos bairros com melhores níveis socioeconômicos. Lanchonetes (n=2051; 27,7%) e restaurantes (n=1945; 26,3%) apresentaram-se em maior quantidade e obtiveram correlação positiva com o Índice de Desenvolvimento Humano e a renda média. Supermercados e hipermercados (n = 288; 3,9%) e feiras livres (n=81; 1,1%) existiam em menor proporção e apresentaram a pior distribuição espacial. Conclusão Desigualdades socioeconômicas foram observadas na distribuição dos diferentes tipos de pontos de venda de alimentos. A pouca diversidade e a limitada quantidade de estabelecimentos em bairros periféricos, além da centralização da oferta de comércios que vendem alimentos prejudiciais à saúde, constituem-se obstáculos para que a população faça escolhas alimentares saudáveis.

Objetivo: Analisar o conhecimento de médicos e enfermeiros da Estratégia Saúde da Família sobre as orientações nutricionais para diabetes mellitus tipo 2 (DM2) e hipertensão arterial sistêmica (HAS). Métodos: Estudo transversal e descritivo realizado com 67 médicos e enfermeiros de 11 Unidades de Atenção Primária à Saúde da Regional II do município de Fortaleza, Ceará, entre dezembro de 2019 e janeiro de 2020. Foram incluídos profissionais contratados, concursados ou participantes de programas de educação em serviço, aplicando-se um questionário fechado do tipo Likert, elaborado pelos pesquisadores, contendo 29 questões relacionadas às recomendações nutricionais das sociedades de referência em DM2 e HAS, e do Guia Alimentar para a População Brasileira. A análise dos dados ocorreu de forma descritiva, utilizando frequências absolutas e relativas e teste qui-quadrado. Resultados: A concordância com as diretrizes nutricionais das sociedades de referência de DM2 e HAS pelos profissionais foi de 17,9% (n=12) para a proporção de carboidratos na dieta do paciente diabético; 22,4% (n=15) para o consumo de café e controle da pressão arterial; 34,3% (n=23) para o consumo de açúcar pelo diabético e 37,3% (n=25) para o consumo de leite e controle da pressão arterial. Em relação às orientações do guia alimentar, a totalidade (n=67) dos entrevistados concordou que os alimentos ultraprocessados devem ser evitados, e que deve haver regularidade e atenção no consumo das refeições. Conclusão: Os profissionais apresentaram conhecimento insuficiente a respeito das recomendações nutricionais estabelecidas pelas principais sociedades nacionais e internacionais de DM2 e HAS.


Objective: To analyze the knowledge of doctors and nurses from the Family Health Strategy on nutritional guidelines for type 2 diabetes mellitus (DM2) and systemic arterial hypertension (SAH). Methods: Cross-sectional and descriptive study was carried out with 67 physicians and nurses from 11 Primary Health Care Units in Regional II in the city of Fortaleza, Ceará, between December 2019 and January 2020. service, applying a closed Likert-type questionnaire, prepared by the researchers, containing 29 questions related to the nutritional recommendations of reference societies in DM2 and SAH, and the Food Guide for the Brazilian Population. Data analysis was descriptive, using absolute and relative frequencies and the chi-square test. Results: The agreement of the nutritional guidelines of the reference societies of DM2 and SAH by the professionals was 17.9% (n=12) for the proportion of carbohydrates in the diabetic patient's diet; 22.4% (n=15) for coffee consumption and blood pressure control; 34.3% (n=23) for the consumption of sugar by the diabetic and 37.3% (n=25) for the consumption of milk and blood pressure control. Regarding the guidelines of the Food Guide, all (n=67) of the interviewees agreed that ultra-processed foods should be avoided and that there should be regularity and attention in the consumption of meals. Conclusion: The professionals showed a lack of knowledge about the nutritional recommendations established by the main national and international societies of DM2 and SAH.

Se ha incluido los profesionales contratados, los concursantes o los participantes de programas de educación en servicio aplicándose un cuestionario cerrado del tipo Likert elaborado por los investigadores con 29 preguntas relacionadas con las recomendaciones nutricionales de las sociedades de referencia en DM2 y HAS, y del Guía Alimentario para la Población Brasileña. El análisis de los datos se dio de modo descriptivo utilizándose las frecuencias absolutas y relativas y la prueba de chi-cuadrado. Resultados: La concordancia con las directrices nutricionales de las sociedades de referencia de DM2 y HAS por los profesionales ha sido del 17,9% (n=12) para la proporción de carbohidratos de la dieta del paciente diabético; el 22,4% (n=15) para el consumo de café y el control de la presión arterial; el 34,3% (n=23) para el consumo de azúcar por el diabético y el 37,3% (n=25) para el consumo de la leche y el control de la presión arterial. Respecto las orientaciones de la guía alimentaria, la totalidad (n=67) de los entrevistados estuvo de acuerdo de que se debe evitar a los alimentos ultra procesados y tener regularidad y atención con el consumo de las comidas. Conclusión: Los profesionales presentaron conocimiento insuficiente respecto las recomendaciones nutricionales establecidas por las principales sociedades nacionales e internacionales de DM2 y HAS.
